The so-called stroke (Zhong Feng) is caused by wind pathogen invading a person with deficiency. Wind is the Qi of the four seasons and eight directions, usually starting from the Kan position (North) on the day of Winter Solstice; one must observe the direction of the wind from the eight directions.
For those suffering from stroke presenting with opisthotonos (Jiao Gong Fan Zhang), wind-bi causing inability to move limbs freely, hemiplegia with locked jaw (Pian Ku Kou Jin), deviation of mouth and eyes (Kou Yan Pian Xi), wind-dizziness with headache (Feng Xuan Tou Tong), blood-wind causing mental palpitations and mania (Xue Feng Xin Shen Jing Ji Dian Kuang), as well as phlegm-stroke (Zhong Tan) and Qi-stroke (Zhong Qi), all can cause excessive saliva, dizziness, and obstruction. If stroke is treated as Qi-stroke, none out of ten will survive. Therefore, when in doubt between stroke and Qi-stroke, initially use Suhe Xiang Wan (Styrax Pill), Wu Ji San (Five Accumulations Powder), or Shen Su Yin (Ginseng and Perilla Decoction). If it is cold-stroke (Zhong Han), use Lizhong Tang (Regulate the Middle Decoction); if it is heat-stroke/summerheat (Zhong Yue), use Baihu Tang (White Tiger Decoction); if it is clearly identified as wind-pathogen, use Paifeng Tang (Dispel Wind Decoction), Xuming Tang (Continue Life Decoction), Fengyin Tang (Wind Attractant Decoction), Da Zhuli Tang (Large Bamboo Juice Decoction), Xiao Zhuli Tang (Small Bamboo Juice Decoction), Da Bafeng Tang (Large Eight Winds Decoction), Zhibao Dan (Precious Treasure Pill), or Niuhuang Qingxin Wan (Calvescent Gall and Heart-Clearing Pill). Differentiate based on cold, heat, deficiency, and excess before administering medication; there will be no case that does not recover. However, this disease accumulates over a long period and is not caused in a single day. Letting people take three to five doses... (Original text has gaps here, context implies emphasizing the chronic nature or slow effect). For women with arm pain, tendon and vessel spasms, inability to flex and extend, worsening with cold, this is due to Liver deficiency allowing wind-cold pathogens to flow into the blood vessels and lodge in the meridians, causing binding. Sometimes symptoms shift to the other arm, which is due to latent phlegm at the Zhongwan point, where Spleen Qi stagnates and fails to descend, rising to bind with Qi. The four limbs belong to the Spleen; when Spleen Qi stagnates and Qi does not descend, it attacks upward to the arms, causing pain. The pulse is deep and fine; Fuling Wan (Poria Pill) and Kongyan Dan (Controlling Saliva Pill) are appropriate.
Another meridian system connects to the root of the tongue. Now, the Heart and Spleen meridians... (Original text is fragmented, likely referring to Heart opening to the tongue and Spleen meridian connecting to the tongue base) are the gate of voice, and the tongue is the mechanism of speech, hence called stroke, resulting in loss of voice and inability to speak. Deficient constitution receiving wind, wind entering the areas beside the mouth, causes muscle tension and irregularity, leading to deviation of the mouth and eyes.
