Within seven days postpartum, if lochia (postpartum discharge of blood) has not been completely expelled, decoctions should not be taken. Wait until the clots below the umbilicus dissipate before administering Mutton Soup. Cases with severe pain are exceptions to this rule.
After two or three days, Zelan Wan (Celandine Pill) may be taken according to the patient's condition. It is best to finish the pills by the end of the first month; otherwise, deficiency and damage will not recover. If the patient is extremely emaciated and incurable, Wushi Zelan Wan (Five-Stone Celandine Pill) should be administered. All women in confinement must take Zelan Wan for tonification. The method of administration requires waiting until after the seventh day; it must not be taken too early.
If postpartum recuperation was conducted properly, limbs are comfortable, and no diseases are present, Black God San (Heishen Powder) should still be taken for four doses. Slightly prepare some tonifying pills and powders. Do not take too much, lest medication cause illness. This must be guarded against. Examples include Siwu Tang (Four Substances Decoction), Sishun Lizhong Wan (Four-Compliant Regulating Middle Pill), Neibu Danggui Wan (Internal Nourishing Angelica Pill), and Danggui Jianzhong Tang (Angelica Building Middle Decoction). Or, if the parturient has abundant blood and qi, is giving birth for the first time, and feels chest oppression and discomfort, Qibao San (Seven-Treasure Powder) should be mixed and taken. If the patient is calm and comfortable, no medicine is needed. If after three days there is high fever and headache... (Text implies: if due to warm bed curtains, abundant qi, or frequent anger/joy, causing dizziness as if on a boat or in a carriage, and mental depression/dizziness, this is Blood Syncope/Xue Yun.) Immediately administer medicine for Blood Syncope for one or two doses. Careful supervision by attendants is still required. If porridge tastes bland and the patient feels weak and fatigued, take Sishun Lizhong Wan for one or two doses. If not so, no medicine is needed. If between two or three days there is intermittent pinching pain in the abdomen, this is "Child Pillow Pain" (Erzhen Tong). Medicine for treating Child Pillow Pain must be taken for one or two doses. If constipation occurs, or urination is difficult, do not use purgatives or diuretics because body fluids are depleted. Using such medicines would cause uncontrollable diarrhea, which is untreatable. Strictly avoid this. If constipation is severe and purgation is necessary, only then can Henuan Tang (Warming and Harmonizing Decoction) be taken to achieve passage.
Postpartum patients should frequently consume warm rice gruel. Eating too much generates heat, and heat causes blood stasis. After seven days, alcohol and small amounts of salt may be consumed. Mutton, pig trotters, and pig stomach should be boiled until soft. Sow pigs and white-hoofed pigs are contraindicated. Black chicken and eggs are suitable for warming the blood. Food intake should not be excessive, nor should raw, cold, fatty, or greasy foods be eaten, lest they form stagnation. Generally, normal delivery results only in the emptiness of the blood organs. Tonify the deficiency, generate good blood, transform bad blood, protect organ qi, and dispel wind pathogens.

Chanhou Cu Mo (Postpartum Vinegar-Ink) [from *Daquan Liangfang*]: Take pine-soot ink, heat over charcoal fire until red hot, quench in rice vinegar. Repeat quenching seven times. Grind extremely fine, sieve through silk gauze. Immediately after childbirth, take 3 qian of vinegar-ink, mixed with urine from a young boy.
Sishun Lizhong Wan (Four-Compliant Regulating Middle Pill) [from *Qianjin Fang*]: Treats postpartum injury to both qi and blood, sudden deficiency of the five zang organs, emaciation of limbs, shortness of breath, and excessive sweating.
From childbirth until 100 days postpartum, take daily. Strengthens qi, tonifies deficiency, regulates organ qi, eliminates residual diseases, promotes digestion and appetite. Also treats postpartum organ deficiency and incessant vomiting.
Licorice (2 liang), Ginseng (1 liang), Atractylodes Macrocephala (1 liang), Dried Ginger (1 liang). Make into powder, honey-pill sized like paulownia seeds. Take 10 pills initially, increase to 20 pills, with warm rice drink, before meals. If just delivered, dissolve in urine from a young boy. Within seven days, take three times daily. The *Chanru Fang* states this is Dali Zhong Wan with double the amount of Licorice. However, its efficacy is greater than the four ingredients in equal proportions. It uses the sweet flavor to moderate the middle and excels at unblocking meridians. This pill is indispensable during the first month postpartum but unnecessary after. Ancient formulas for women's tonification include Da Zelan Wan, Xiao Baiwei Wan, Shugan Dihuang Wan, and Dasheng San. These contain many herbs, making them hard to obtain for lower-class families or rural areas. Moreover, true herbs are rare in pharmacies. It is better to substitute with Sishun Lizhong Wan, which covers more indications than Zelan Wan.

Case of Yi'an, Fifth Son of Minister Liu, wife of Vice Director of Court of Imperial Sacrifices Shuqian, Ms. Li. In spring of Zhongtong Year 3, she wanted to visit her parents but could not, leading to emotional disturbance. Due to labor, she experienced fatigue in limbs, restlessness with heat and aversion to cold, intermittent pain, anorexia, vomiting after eating, weak and short breath, lethargy, and desire to sleep. Physicians treated it as Cold Damage, using exterior-releasing and sweating methods. The next day, the condition transmitted. They used Xiao Chaihu Tang and Da Chaihu Tang. After ten-plus days, symptoms worsened. The family asked if it was another disease. The physician said it was Cold Damage transmitting through six channels, expecting sweat for recovery. Next day, nails turned slightly blue-black, legs to waist felt ice-cold, eyes looked up and fixed, throat uncomfortable, cold qi from lower abdomen rushed to heart causing pain, incessant vomiting, breath nearly gone. Considered incurable. Summoned me. I diagnosed pulse as deep, fine, and faint. This is insufficient Middle Qi, wrongly treated as Cold Damage. Exterior-releasing and interior-purging further damaged Middle Qi. The ruined pattern was evident. The mother wept and stepped aside, saying the illness was critical, asking for my utmost effort. I first prepared 1 liang of pungent-hot herbs, cut, for one dose. At night, the medicine was ready but she couldn't drink. I dripped it slowly. By midnight, she groaned slightly, body warmed, suddenly asked for porridge. Drank until dawn, ate porridge twice. Decocted and gave more. By noon, other physicians arrived, diagnosed pulse recovering and symptoms returning. Everyone rejoiced and left. Three days later, the mother said the patient had bowel obstruction. Someone suggested using Pi Yue Wan (Spleen Restriction Pill) to moisten. I said the previous pattern was cured by pungent-hot agents generating yang and retreating yin. Using Rhubarb purgative might cause cold mismatch and other complications. Others disagreed, used 20 Pi Yue Wan. That night, profuse diarrhea occurred. Next day, face turned green, spirit weak, vomiting returned. I again used pungent-hot agents to warm and cured it.
