On the sixth day of Cold Damage (Shanghan), the Jueyin meridian is affected. The Jueyin meridian belongs to the Liver. Its vessels circulate through the genitalia and connect to the liver. Therefore, on the sixth day of illness, if there is irritability and fullness, and the pulse at the Cun position is slow, the testicles will not retract. The external symptoms must include fever and aversion to cold resembling malaria, indicating that the disease is about to resolve. Guizhi Mahuang Ge Ban Tang (Cinnamon Twig and Ephedra Half-Volume Decoction) should be used. If the pulses at both Cun and Chi positions are deep and short, the testicles will definitely retract, and toxic heat has entered the zang organs. Chengqi Tang (Gut-Purging Decoction) should be used to purge them. Generally, in Cold Damage diseases, the transmission between zang and fu organs follows a sequence where the Yang meridians are affected first, gradually transmitting to the Yin meridians. Yang governs generation; thus, Taiyang Water transmits to Yangming Earth, and Earth transmits to Shaoyang Wood, representing a mild pathogenic factor. Yin governs destruction; thus, Wood transmits to Taiyin Upper, Earth transmits to Shaoyin Water, and Water transmits to Jueyin Wood. By the sixth or seventh day, it should transmit to Jueyin Liver Wood. Since Qi shifts to overcome Spleen Earth, the Spleen becomes trapped and critical. Wei Qi and Ying Blood circulation is blocked, leading to deafness, retraction of the testicles, loss of consciousness, and death. In such cases, Chengqi Tang should be used rapidly. It is said: "Spleen Heat Disease endangers the five zang organs." It is also said: "When Earth fails and Wood attacks, death occurs." However, if by the sixth or seventh day, the Spleen and Stomach pulses indicate that Spleen Qi is fully restored and no longer overcome, the pathogen will have no place to reside. As adversity turns to prosperity, Ying and Wei Qi will flow freely, resulting in profuse sweating and resolution of the disease.
