Wei Qi (Defensive Qi) circulates in the Yang aspect during the day and in the Yin aspect at night. The Yin aspect governs the night, and the night governs sleep. When Yang Qi is exhausted and Yin Qi is abundant, the eyes close and sleep occurs. Currently, the pathogenic heat has not yet dissipated; it merges with the Qi of the various Yang meridians, causing Yang to be exclusively excessive and Yin to be relatively deficient. Therefore, the inability to sleep after illness is due to the fact that Yin Qi has not yet recovered. Ying Qi (Nutritive Qi) and Wei Qi (Defensive Qi), when circulating in the Yang aspect during the day, result in wakefulness; when circulating in the Yin aspect at night, they result in sleep. After recovery from Shanghan (Cold Damage disease), the Zang-Fu organs are all deficient, and the entry and exit of Ying and Wei Qi cannot follow their normal patterns. Because pathogenic heat has not dissipated and merges with Yang Qi, (leading to an imbalance of Yin and Yang).
The verse states: For insomnia associated with Kidney syndrome, there is diarrhea accompanied by vexation; excessive heat manifests in the Bladder and dryness in the Stomach; after vomiting, sweating, or purging, the resulting ao nao (restless discomfort) should be judged based on whether the heat is predominant or minor.
The classic says: "When the Stomach is disharmonious, sleep is uneasy." By "disharmonious," it means that body fluids are dried up and scorched, pathogenic heat causes vexation, Yang is exclusively excessive, and Yin is relatively deficient. Generally, the night is governed by Yin; if Yin Qi is sufficient, the eyes close and one sleeps peacefully. Only when Yin Qi is suppressed by Yang heat (or Yin is insufficient) does one become restless throughout the night without peace. This is what is meant by "Yin deficiency leads to nighttime disturbance" (referring to the restlessness caused by Yin deficiency and hyperactive fire disturbing the Shen). If Yang is deficient and Yin is excessive, there are symptoms of quietness at night and vexation during the day. Shaoyin syndrome with difficulty urinating, thirst, and insomnia is treated with Zhuling Tang (Poria Decoction), which indicates water retention. If for two to three days or more, there is vexation and insomnia, the primary formula is Huanglian Ejiao Tang (Coptis and Donkey-Hide Gelatin Decoction), which is very effective. In Taiyang syndrome, excessive sweating or profuse sweating leads to dryness in the Stomach, vexation, heat, and insomnia; if the patient desires water, give it to them, and once the Stomach Qi harmonizes, the condition resolves. If the pulse is floating, there is difficulty urinating, and thirst, Wuling San (Five-Ingredient Powder with Poria) can be used. After vomiting, sweating, or purging, if there is internal ao nao (restless discomfort) and empty vexation, leading to tossing and turning and inability to sleep, use Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction) or Suanzaoren Tang (Sour Jujube Seed Decoction). With excessive heat, dry retching, groaning, and confused speech, along with insomnia, use Huanglian Jiedu Tang (Coptis Decoction to Resolve Toxicity).
In all cases of Yang toxicity and heat diseases, insomnia occurs. After purging followed by sweating, if there is no significant heat, the pulse is faint and deep, there is no vomiting or thirst, and no exterior syndrome, but there is quietness at night and vexation during the day preventing sleep, Ganjiang Fuzi Tang (Dried Ginger and Aconite Decoction) can be used. When Yang overcomes Yin, there is manic sleep with chaotic dreams, treatable with Zhizi Chi Tang (Gardenia and Fermented Soybean Decoction). When Yin overcomes Yang, there is palpitations, dizziness, and lethargy, treatable with Suanzaoren Tang (Sour Jujube Seed Decoction). Insomnia after recovery is caused by heat merging with the various Yang aspects and Yin Qi failing to recover. One should use Zhizi Wumei Tang (Gardenia and Mume Fruit Decoction) or Yijian Wendan Tang (Simple Gallbladder-Warming Decoction) with Zhu Ru (Bamboo Shavings). These can be used generally.
Another verse on hypersomnia: Hypersomnia presents with mental sluggishness and confusion; spontaneous sweating and wind-warmth manifest silently. Huhe syndrome (Chancroid/Herpes-like disease) shows lip ulcers, with a deep and fine pulse indicating Kidney deficiency. Taiyang syndrome with a floating and fine pulse also presents with drowsy sleep.
Yin deficiency and Yang excess cause people to be drowsy with closed eyes because Yin governs closure. Silence indicates that Yin governs stillness. There are four syndromes of hypersomnia: Two involve the meridians. For example, Taiyang syndrome with aversion to cold and a floating, fine pulse indicates that Yang Qi has not fully recovered. Huhe syndrome with lingering heaviness and a history of diarrhea implies that Yang is not contraindicated. This is why it is called Yang excess and Wind-Warmth syndrome. If both Cun and Chi pulses are floating, with spontaneous sweating, panting, heavy body unable to hold posture, and silent desire to sleep, sweating methods are contraindicated; instead, use Weirui Tang (Reed Rhizome Decoction). Disease involving the Shaoyin and Jueyin meridians: Huhe syndrome involves heavy limbs, dry throat, hoarse voice, and ulcers on the upper and lower lips, causing severe harm, as detailed in its specific section. Shaoyin syndrome features deep and fine pulses at both Cun and Chi, with eyes wanting to sleep; urgently administer Sini Tang (Frigid Extremities Decoction) to restore Yang Qi. Additionally, in Shaoyin syndrome, there is a desire to vomit but nothing comes out, vexation, and hypersomnia. After five or six days, spontaneous diarrhea with thirst—Zhang’s text provides no treatment method. If the urine is white, Sini Tang (Frigid Extremities Decoction) is excellent. Zhang also states that in Shaoyin disease, if the pulse is faint, deep, and difficult to revive, careful examination is crucial. Taiyang syndrome involves headache and neck stiffness, aversion to cold, drowsiness, floating and fine pulse, or fullness in the chest and hypochondrium; Xiao Chaihu Tang (Minor Bupleurum Decoction) is suitable. If the pulse is floating, Mahuang Tang (Ephedra Decoction) is suitable. Wind attacking the Stomach causing jaundice and drowsiness has its own section.
Verse on post-recovery lethargy: Post-recovery lethargy resembles demonic possession; sometimes alternating chills and fever, sometimes tidal fever. Erratic speech and diminished spirit indicate toxin in the Heart and Chest, where sweat has not fully cleared.
Half a month after recovering from Shanghan (Cold Damage disease), if one remains unrefreshed, speaks erratically, lacks spirit, experiences alternating chills and fever resembling malaria, or tidal fever with red cheeks, treating it as wind-demon is incorrect. This is due to incomplete sweating, residual toxin in the Heart, and internal accumulation of heat affecting the collateral vessels. For post-recovery lethargy and unconsciousness, use Zhimu Mahuang Tang (Anemarrhena and Ephedra Decoction) to induce slight sweating. Post-recovery relapse due to labor or diet is discussed under the section on heat after purging. For residual heat in the epigastrium after recovery causing empty vexation and vomiting, use Zhuye Tang (Bamboo Leaf Decoction) with Sheng Jiang (Fresh Ginger). For post-recovery drowsiness due to cold above the diaphragm, Lizhong Wan (Regulate the Middle Pill) is suitable. For post-recovery fluid accumulation below the waist, Mushe Zexie San (Oyster Shell and Alisma Powder) is suitable. For mild evening vexation after recovery, it is because the illness has just resolved and forced consumption of grains led to indigestion; regulating diet will lead to recovery.

Heat dissipates accordingly. Generally, for Shanghan patients with thirst, do not give too much water; water accumulation in the chest leads to Jie Xiong (Chest Binding). However, moistening the lips is permissible. Treatment for Jie Xiong is found in the Lizhong Tang (Regulate the Middle Decoction) section.
