How to understand labor relapse (Laofu) after Shanghan? "Labor" refers to physical or mental exertion, and "relapse" refers to the recurrence of the disease. Labor relapse refers to the situation where the condition recurs due to excessive exertion after the initial recovery from Shanghan (Cold Damage).
After Shanghan has just healed, the body's blood and qi have not yet stabilized, and residual heat has not been completely cleared. At this time, if one engages in labor, it will cause a deficiency of true qi, allowing pathogenic heat to re-enter the meridians, thereby causing the disease to recur. There are mainly two types of this situation: one is external injury caused by labor (external contraction), and the other is internal injury caused by improper diet. Regarding external injury caused by labor, it is not only strong lifting, long-distance walking, and other physical consumptions that constitute labor; even small matters like combing hair and washing the face can disturb the qi mechanism; sorrow, grief, and excessive thinking will damage the spirit. These situations can all lead to relapse, let alone excessive food intake. As for internal injury caused by improper diet, it mainly manifests as symptom retention (Yi) caused by overeating, and relapse (Fu) caused by eating meat. The *Neijing* states: "Why does heat disease sometimes leave residual symptoms after healing?" This is because when the heat intensity is still high, forced eating occurs. Although the illness seems to decline, residual heat remains latent within the body. Due to the retention of grain qi from food and its struggle with pathogenic heat, the combination of two yangs leads to symptom retention. The classic also says: "If there is slight irritability and a feeling of discomfort after a major illness has healed, it is because the newly deficient body cannot withstand the transformation of grain qi, thus causing slight irritability. Reducing food intake will lead to recovery."
The transmission of Shanghan pathogens follows a sequence from the exterior to the interior; methods such as sweating, vomiting, and purging have a hierarchy from mild to severe. However, labor relapse is different. When seeing the return of pathogens, one must strike immediately without waiting for the pathogens to transmit. The classic states: "For labor relapse after a major illness has healed, Zhishi Zhi Zi Chi Tang (Bitter Orange, Gardenia, and Fermented Soybean Decoction) is the primary treatment. If there is retained food, add Dahuang (Rhubarb)." The Yellow Emperor asked: "Why is there occasional retention after heat disease has healed?" Qibo answered: "Various retention symptoms occur because forced eating happens while heat is intense, leading to retention. In such cases, although the disease trend has declined, residual heat is latent, and the combination of food and pathogenic heat causes retention." The Emperor said: "Good. How should retention be treated?" Qibo said: "Observe its deficiency or excess, regulate its reversal or smoothness, and the disease can certainly be cured." Eating meat causes relapse, and overeating causes retention; these are taboos. Moreover, Zhishi Zhi Zi Chi Tang has an emetic effect; why wait for symptoms of empty irritability and restlessness before using it? Adding Dahuang has a purgative effect; why wait for abdominal fullness and delirium before using it? The classic states: "For fever recurring after Shanghan has healed, Xiaochaihu Tang (Minor Bupleurum Decoction) is the primary treatment. If the pulse is floating, use sweating to resolve the exterior; if the pulse is deep and solid, use purging." This is also to quickly analyze and eliminate pathogens. Generally, Shanghan pathogens enter from the outside, while labor relapse pathogens emerge from the inside. Sweating, vomiting, and purging methods should be used according to appropriateness. Alas! Labor relapse can be triggered by various types of exertion. Sexual indiscretion can even lead to death. Exchange of clothes between men and women is called Yin-Yang Yi (Yin-Yang Transformation). Self-onset without exchange is called Nü Laofu (Female Labor Relapse). Because these diseases internally damage true qi and externally activate pathogenic heat, when true yuan is weak and pathogens are strong, they are difficult to cure. Previously, Gu Zixian, the District Commissioner, did not trust Hua Tuo's diagnosis and exhibited the sign of tongue protrusion several inches before death. From this, is it not a warning for future generations? Zhang Zhongjing said: "For fever recurring after Shanghan has healed, Xiaochaihu Tang is the primary treatment. If the pulse is floating, use sweating to resolve the exterior, preferably Chaihu Guizhi Tang; if the pulse is solid, use purging to resolve the exterior, preferably Da Chaihu Tang." He also said: "For labor relapse after a major illness has healed, Zhishi Zhi Zi Tang is the primary treatment." *Qianjin Fang* (Essential Prescriptions Worth a Thousand Gold) treats labor relapse reviving the dead with Renshen Maidong Tang (Ginseng and Ophiopogon Decoction). There is also food relapse: after a major illness has just healed, the spleen and stomach are still weak, and grain qi has not recovered. Forced overeating leads to stagnation and indigestion, causing fever, named Shifu (Food Relapse). Generally, new illnesses often present with epigastric fullness, dry belching with food odor, borborygmus, and diarrhea due to food injury, which can be treated with Shengjiang Xiexin Tang (Ginger Heart-Draining Decoction). Zhang Zhongjing stated regarding Zhishi Zhi Zi Tang: "If there is retained food, add Dahuang (Rhubarb) the size of five or six bo-chess pieces; take it and recover." *Guangji Fang* treats labor relapse with Xiongshufen Tang (Male Rat Feces Decoction), and Xu Renze's Qiwei Congbai Tang (Seven-Ingredient Scallion White Decoction) can also be selected. Alcohol-induced labor relapse involves extreme yin transforming into yang, cold generating heat due to excess. Ancients considered Shanghan a major illness; after a major illness, alcohol should not be consumed. Labor relapse fever and food relapse fever become worse with alcohol, with particularly severe heat. After Shanghan is resolved by sweating, if alcohol causes worsening with irritability, dry mouth, dry vomiting, groaning, delirium, and insomnia, Huanglian Jiedu Tang (Coptis Detoxification Decoction) can be used, or Longdancao Decoction (Gentian Decoction) with Chenpi (Tangerine Peel) as an adjuvant. Heat after sweating or purging due to labor or food relapse: heavy sweating damages qi, leading to weak yang, weak pulse, and aversion to cold; heavy purging damages blood, leading to weak yin, choppy pulse, and fever. Mistaken sweating or purging yields similar results. Yin is governed by yang, and yang is rooted in yin. Blood is yin, qi is yang. Blood cannot move without qi's propulsion. Purging leading to loss of yang means yang no longer governs yin. With yin having no master, cold qi attacks, resulting in a choppy pulse. Extreme cold and blood deficiency cause fever. How can prescriptions be taken lightly? After sweating, weak yang and aversion to cold can be treated with Sini Tang (Recovery Decoction) or Gancao Shaoyao Fuzi Tang (Licorice, Peony, and Aconite Decoction). After purging, weak yin and fever indicate internal heat, treatable with Tingli Kujiu Tang (Descurainia Vinegar Decoction). For fever after healing due to labor relapse, use Xiaochaihu Tang, Zhishi Zhi Zi Tang, or Shufen Tang (Rat Feces Decoction, specifically male rat feces with pointed ends). If the pulse is floating, use sweating with Mahuang Tang (Ephedra Decoction) or Congbai Tang (Scallion White Decoction); if the pulse is solid, use purging with Chengqi Tang (Order the Qi Decoction) or Da Chaihu Tang. *Zhengzhi Lun* states: For lingering residual heat after healing, use Chaihu Guizhi Tang; for diarrhea, borborygmus, and epigastric fullness, use Shengjiang Xiexin Tang; for difficult urination, use Maidong Tang; for sexual intercourse-induced labor relapse with swollen external genitalia and abdominal colic, use Chenpi Tang; for heart-mind labor-induced recurrent fever, use Zhishi Zhi Zi Tang and Xiaochaihu Tang. For food relapse fever after healing, use Zhishi Zhi Zi Tang plus Dahuang; for overeating rice/grains, meat, or hard foods like biscuits that do not digest, causing recurrent fever, food injury inevitably presents with epigastric fullness, belching, acid regurgitation, borborygmus, and diarrhea, treatable with Shengjiang Xiexin Tang. For body heat and chest pain after purging, use Zhizi Tang (Gardenia Decoction).
