For vomiting and hiccups caused by Shanghan (Cold Damage), the pathology lies in the Foot-Yangming Stomach meridian. The meridians of the Foot-Yangming ascend reversely, causing Qi rebellion; Qi rebellion leads to vomiting and hiccups.
Zhang Zhongjing stated: "If there is frequent vomiting, even if Yangming syndrome patterns are present, one must be cautious and absolutely avoid purgation." This is precisely for this reason. Furthermore, vomiting and hiccups in Shanghan may result from internal heat or from water retention after drinking. Since the syndromes differ, the treatments must vary accordingly; careful differentiation is essential.
Additionally, after the resolution of a Shanghan disease, disharmony of Stomach Qi may occur. This arises because during the initial infection, toxic heat was abundant, leading patients to take excessive cold medicines for purgation or drink cold water. Although the illness was temporarily suppressed and the heat subsided, cold pathogens subsequently activated. This results in hardness and firmness below the heart, belching with a smell of rotting food, borborygmus (rumbling in the abdomen), and diarrhea. This is due to deficiency-cold of the Spleen and Stomach.
It is also said that latent heat lodged in the Stomach causes chest fullness; chest fullness leads to Qi rebellion and hiccups. If severe purgation has been performed, resulting in deficiency-cold in the Stomach, hiccups will also occur.
Hiccups (Yue) in Shanghan are commonly known as ke-ni (convulsive hiccup). There is a condition similar to Yue called "KT" (likely referring to Yi or a specific ancient character denoting qi stagnation). KT manifests merely as qi stagnation between the chest and throat, with cold unable to descend, but without sound. In contrast, Yue produces audible "chi-chi" sounds. Sound corresponds to Metal; however, since the Stomach is afflicted, vomiting and hiccups arise. These are all considered diseases, differing only in severity.
When deficiency-cold interacts, forcing the patient to drink water which induces profuse sweating, the water encounters cold qi, and the cold mutually combats, causing the person to experience KT. This indicates exhaustion of Stomach Qi. After severe emesis in Shanghan, when the body is extremely deficient and sweating profusely, exterior Qi becomes constrained. If water is then given to induce sweating, hiccups ensue. This occurs because of cold in the Stomach. When the Stomach is deficient and cold, preventing food intake, drinking water triggers hiccups. Thus, it is evident that both Yue and KT are disorders of the Stomach.
The Classics state: "A floating pulse at the Fuyang point indicates Qi stagnation (KT); a slippery pulse indicates hiccups (Yue). This is the physician's fault, resulting from treating deficiency as excess." Generally, erroneous purgation leads to Stomach deficiency and Qi rebellion, forming hiccups.
The Classics further state: "In warm diseases, premature purgation causes hiccups; attacking heat in a constitutionally weak person causes hiccups; in Yangming disease, if the patient cannot eat and heat is attacked, hiccups inevitably occur." All such cases involve hiccups arising from Stomach deficiency following purgation.
However, KT is specifically caused by the interaction of water and cold; treatment requires Xiao Qing Long Tang (Minor Blue Dragon Decoction) with Mahuang removed and Fuzi added. Hiccups (Yue), on the other hand, are formed by heat oppression and stagnation, preventing Qi circulation. Mild cases have indications for harmonization; severe cases have indications for purgation.
The Classics state: "If there is tidal fever and occasional hiccups, administer Xiao Chai Hu Tang (Minor Bupleurum Decoction)." This represents a harmonizing indication. If hiccups accompany abdominal fullness, observe the anterior and posterior orifices; knowing which part is obstructed, unblocking it leads to cure. This represents a purgative indication.
When Shanghan progresses to hiccups, the disease has reached its extremity. It is not like mild symptoms such as thirst or irritability. For instance, in Taiyang Wind Strike, if fire therapy is used to force sweating, both Yin and Yang become exhausted and depleted. The body becomes dry and withered, sweat appears only on the head up to the neck, the abdomen is full with slight wheezing, the mouth and throat are dry and ulcerated, constipation may occur, and prolonged delirium may develop. Severe cases progress to hiccups, indicating the extremity of the disease.
If urination ceases, abdominal fullness increases, and hiccups appear, it is incurable. This is a true critical condition. Even with divine medical skill, facing such collapse, what can be done?
When Shanghan heat-toxins are vigorous, excessive use of cooling medicines and cold water causes the heat to subside temporarily, but the Stomach Qi becomes deficient. This leads to chest fullness, Qi rebellion, hardness and痞 (mass) in the heart and abdomen, inevitably causing vomiting and hiccups.
