Sudden Elimination Formula (Su Xiao Fang)
For carbuncles and sores on the back. Whether already erupted but not yet condensed, or already condensed but not yet ulcerated. Urgently use Huangming Niu Pi Jiao (Yellow-transparent Cowhide Glue), dissolve it in fresh water to a suitable consistency, then spread it onto glue paper.
Apply for a considerable time. Take two soft cloths, boil them in strong vinegar until hot, wring them out alternately, and place them over the glue paper for steaming and ironing. Continue until the swelling dissipates. If the sore itches, one must endure the itch and not stop ironing. Or if the sore breaks and pus flows out, stop ironing when the pus is exhausted. If there is no effect, repeat the previous method: apply the glue paper to the sore again and continue ironing with the hot vinegar cloth until the pus is exhausted. Afterwards, wash off the glue paper with Guan Zhong Tang (Cyatheae Spora Decoction), and apply powders such as Tui Zhong San (Swelling-Reducing Powder), Juan Du San (Toxin-Eliminating Powder), or Chen Shui Gao (Sunken-Water Ointment). Alternatively, use Ba Du San (Toxin-Pulling Powder) at will. Simultaneously, urgently take Wan Jin Yi Zui Gao (Ten-Thousand-Gold One-Drunkenness Ointment), or Nei Xiao San (Internal-Eliminating Powder), or Ru Fen Tuo Li San (Milk-Powder Supporting-Interior Powder). After rupture, wash the sore with Xi Chuang Fang (Sore-Washing Formula) to disperse toxic vapors.
Formula for treating various carbuncles, sores on the back, and carbuncles/furuncles that have already ulcerated and are painful.
Steam glutinous millet and alternate ironing on the affected area; recovery should be immediate. Another method says to use rose calyxes for moxibustion and ironing.
(Attached Discussion)
Carbuncles arise from disharmony of the Six Fu Organs. The Six Fu Organs govern the exterior; Qi moves through the meridians and floats outward. Unpredictable emotions and irregular diet lead to imbalance of Yin and Yang, causing disharmony of the Six Fu Organs. In individuals with deficient Ying (Nutrient) and Wei (Defensive) Qi, the interstices (Cou Li) open, allowing Cold pathogens to lodge in the meridians. When meridians are constrained by Cold, Ying and Wei Qi stagnate within the vessels. Ying refers to Blood, and Wei refers to Qi. When Ying Blood encounters Cold, it becomes stagnant and fails to flow; Wei Qi follows and contends with the Cold, leading to obstruction and blockage. Qi belongs to Yang; accumulation of Yang Qi generates stasis-heat. Prolonged retention of Cold and Heat leads to accumulation and formation of carbuncles. Since Fu Organs are where Yang Qi floats and governs the exterior, carbuncles are superficial, with thin, lustrous skin. Over time, Heat surpasses Cold, heat accumulates, damaging flesh and ruining muscle tissue, causing blood and flesh to rot and transform into pus. The pathology is located in the exterior; being superficial means the bone marrow remains unburnt and the Zang-Fu organs remain undamaged, thus it is treatable.
The *Treatise on the Great Normal Policies* states: Heaven tilts northwest, cold on the left, cool on the right; Earth sinks southeast, hot on the right, warm on the left. High places are cold, low places are hot. Thus, cold and cool causes distension, while warm and hot causes sores. Purging resolves distension; sweating resolves sores. In the Southeast regions, in humans, this corresponds to Bing Small Intestine Heat and Jia Gallbladder Wind. The Small Intestine and Gallbladder reside inferiorly, but their nature is inflammatory and ascending. Such sores present external manifestations of the Six Meridians but internal absence of urinary or fecal obstruction; diet remains normal, and excretions are regulated. Knowing the disease is not interior, it is not a deep abscess (Ju), but merely a carbuncle or furuncle. Small ones are called furuncles; large ones are called carbuncles. The pathogen originates from damp-wind areas below, entering from the exterior and invading the body. The Classics state: "When Ying Qi does not follow its course and reverses in the muscle tissue, carbuncles and swellings arise." The *Treatise on Pathogenesis* states: "All pain, itching, sores, and ulcers belong to Heart Fire." This type of sore enters from the exterior, moving from the Bing Small Intestine to the Jia Gallbladder, causing pain and itching. Both formulas primarily treat Blood disorders; blood diseases inevitably cause pain. This results from deficiency of Yuan Qi and reversal of Ying Qi. Initially, without transmission, it resides above the muscles and between the skin and hair, emerging only along the pathways of the Wind-Heat Six Meridians. It is appropriate to drain Wind-Damp-Heat. Physicians only know that when Yin restores Yang, sweating is indicated. Here, sweating is indicated because Damp-Heat stagnates in the Hand and Foot Shaoyang, causing blood vessel coagulation and reversal, weakening Ying Qi and whole-body Yuan Qi. Wind-Heat stagnates inferiorly; the face is red and swollen, slightly dark, showing the color of Wood stagnating in the East. The nature of Wind-Wood rushes upward, causing an angry expression; the person is prone to anger, and the sore is red, swollen, hard, and slightly dark. The sore's condition rises vigorously, forming hard knots that cause pain. The Lung is associated with the left hand governing the exterior; the Left Cun pulse is floating and rapid-slow, the Left Guan pulse is floating, rapid-slow, and wiry, indicating external pathogens above the blood vessels and between the skin. Emergency sweating is required to unblock Ying and Wei, thereby expelling the pathogen. Use Nei Tuo Ying Wei Tang (Internal-Supporting Ying-Wei Decoction) to disperse it.
Sores belong to Fire; one must distinguish internal and external treatments addressing the root. If the pulse is deep and solid, first unblock the interior to cut off the source. If the pulse is floating and large, first support the interior to prevent pathogen invasion. In cases involving both interior and exterior, the pathogen is extremely abundant, obstructing meridians, causing carbuncles and swellings. This results from failure to support the interior, failure to unblock, and failure to harmonize Ying and Wei. The general principle of treating sores requires understanding three methods: supporting the interior, unblocking, and promoting Ying-Wei circulation. For lesions originating internally and manifesting externally: the pulse is deep and solid, with fever, dryness, and irritability, but no external redness; pain is deep inside, indicating deep pathogens. Therefore, first unblock the Zang-Fu organs to cut off the source. For lesions originating externally and attacking internally: the pulse is floating and rapid, swelling is external, signs and symptoms are apparent externally, suggesting urgent internal attack; therefore, first support the interior. For lesions in the middle of both: externally no aversion to cold, internally Zang-Fu organs are unobstructed, knowing the disease is in the meridians, one should harmonize Ying and Wei. After using these three methods, even if not fully cured, complications will likely be avoided. Pathogens can be significantly reduced, facilitating recovery. A rapid pulse without body heat indicates internal carbuncle. Floating-rapid pulses usually indicate fever, but if accompanied by shivering and aversion to cold, and a painful spot, a carbuncle will form. A faint and slow pulse indicates fever. A weak and rapid pulse indicates chills, indicating impending carbuncle/swelling. A floating and rapid pulse with no body heat, quiet demeanor, slight stomach dryness, and unknown pain location indicates impending carbuncle/swelling. A slippery and rapid pulse: slippery indicates excess, rapid indicates heat; slippery indicates Ying, rapid indicates Wei. When Ying and Wei meet, they knot to form a carbuncle. Where heat passes, pus forms. Body pain and persistent sores occur.
Generally, high swelling indicates shallow origin; low swelling indicates deep origin. Large sores are easier to treat; small, hot sores are harder to treat. Severe pain early damages muscle; severe pain late damages bone. Uniform hardness indicates no pus; half-hard, half-soft indicates pus. Uniformly soft swelling is a blood tumor, not a carbuncle. Gradual swelling that persists without heat and causes dragging pain is a tumor, not a carbuncle. Qi stagnation can also cause swelling; prolonged non-resolution forms a carbuncle, treated by dispersing Qi. Once Qi is dispersed, if residual floating Qi remains, elderly people may develop carbuncles; treatment should occur during youth, and heat should be dissipated to avoid this worry.
Carbuncles hard as stone moving under the skin are rootless scrofula. Prolonged non-resolution due to other heat diseases may occasionally erupt as carbuncles.
Without internal consumption diseases or blood tumors, if a sore rises like a mound, straight and upright, moxibustion is contraindicated. Treatment with fire ironing causes burning and rotting; scraping off the eschar leads to uncontrollable bleeding and death.
For any carbuncle, regardless of size, upon detection, take glue palm-sized, soak in warm water until soft, measure size. Make a hole the size of a coin in the center, apply to the swelling. When dry and tight, if no pus has formed, growth stops. If pus has formed, it will discharge naturally. Puncturing the hole with a sharp needle to reach the pus is excellent. Remove the glue upon cure. For high, large carbuncles without significant heat, and flat, purple flesh without points, aggressive treatment is unnecessary; simply use Zhu Ye Shi Gao Tang (Bamboo Leaf and Gypsum Decoction) to promote Qi. Flat flesh indicates no pus. Sudden carbuncle pain: apply Ba Wei Huang Qi San (Eight-Ingredient Astragalus Powder). Large carbuncles: seven days; small carbuncles: five days. Naturally hard ones may rupture spontaneously. For breast carbuncles that are hot and intolerable to touch, first take Wang Bu Liu Xing San (Vaccaria Seed Powder) internally, and apply Fa Bei Gao (Back-Carbuncle Ointment) externally. If back sores are broken but not painful, breast sores should be allowed to ripen completely; when pressing elicits immediate rebound, the sore is ripe. Needle it. Needling must reach the pus, judged by intuition. Chest and back depth should not exceed one cun. If no pus, apply Shi Rou Gao San (Flesh-Nourishing Ointment Powder) to the opening. For internal mouth carbuncles, once body heat subsides, take Mu Zhan Si San (Xanthoxylum Powder). Five days later, if the carbuncle begins to scab, take Pai Nong Nei Sai San (Pus-Draining Internal-Sealing Powder).
After carbuncle rupture, the body feels limp and weak, with internal cold and external heat. Some swellings resemble carbuncles but are not; pressing reveals no connection, indicating Wind-Toxin. Do not needle. Take Sheng Ma Tang (Cimicifuga Decoction) and apply ointment externally. For ruptured carbuncles, leave the upper third unopened, needle the lower one-third. Ensure extreme heat, which alleviates pain. For post-rupture decay failing to heal, wash with Zhu Ti Tang (Pig Trotter Decoction), twice daily. Summer: two days; Winter: six to seven days. Half a dose of decoction suffices.
For carbuncles with foul flesh after breakdown, wash with Zhu Ti Tang to remove filth, then apply Shi Rou Gao San (Flesh-Nourishing Ointment Powder). After foul flesh is gone, apply Sheng Rou Gao San (Fresh-Flesh Ointment Powder) and massage edges to accelerate healthy tissue growth. Abstain from sexual activity, avoid wind and cold, do not overexert. Wait for tendons and vessels to recover before resuming normal activities. New flesh is easily injured; injury leads to internal collapse, re-eruption, and difficult rescue. Be cautious. White scabs are taboo. Long-standing carbuncles with non-healing wounds, white flesh, and scant pus/blood indicate cold stagnation at the wound site due to exhaustion of patient's Qi and Blood, failing to nourish the sore. Use one bunch of Ai Ye (Mugwort Leaves), decoct strongly in a clay pot. In a wind-shaded place, pour hot Mugwort decoction over the wound. Wrap clean flesh around Mugwort leaves with silk cloth, pour hot liquid over it. Once daily, avoid wind after washing. Burn Song Xiang (Rosin) and smoke the wound for a long time, then apply Shen Yi Gao (Divine-Efficacy Ointment). Do not let repulsive/unclean people see the sore; otherwise, healing is hindered and medicine ineffective.
