Kuzhi Shuicheng Gao (Mummifying Hemorrhoid Water-Clear Ointment), Rushen Qianjin Fang (Miraculous Thousand-Gold Formula), Sanpin Dingzi (Three-Grade锭状 Medicine), Yangdan Gao (Sheep Gallbladder Ointment), Xiongdan Gao (Bear Gallbladder Ointment), in addition to these, there are Shuiyin Zaozi Gao (Mercury Date-shaped Ointment) and Woniu Gao (Snail Ointment).
One prescription: Whenever hemorrhoids first appear, with unceasing pain and itching, take an old cloth shoe sole, heat it by roasting, and frequently apply it as a hot compress to the affected area. When it cools, roast and apply again; the pain and itching will then cease.
Mingmen (GV4, Gate of Life) acupoint moxibustion, seven moxa cones, treats five types of hemorrhoids and fistulas. Changqiang (GV1, Long Strong) acupoint moxibustion, using the number of moxa cones corresponding to the patient's age (Sui Nian Zhuang), is most effective for treating hematochezia from the five types of hemorrhoids.
One method: To treat hemorrhoids large like a cucumber, prolapsing from the anus, which upon onset causes severe pain and even collapse, first wash with Jingjie Tang (Schizonepeta Decoction), then apply moxibustion three to seven cones on the lesion. If one feels a stream of hot qi penetrating into the intestine, there will be a massive discharge of bright red blood and foul blood. After about an hour, when the pain intensifies, the disease will subsequently heal.
For all cases of traumatic injury or falling from heights, where evil blood flows into the interior, regardless of which meridian is injured, they are all governed by the Liver. This is because the Liver governs blood storage; thus, whenever defeated blood stagnates, following its association, it inevitably returns to the Liver. Locations such as the hypochondrium and lower abdomen are pathways of the Liver Meridian. If swelling and pain are severe, or if fever and spontaneous sweating occur, one must assess deficiency and excess, and treat with herbs that regulate blood and promote meridian circulation.
Pulse Diagnosis: As stated in *Neijing* (Classic of Internal Medicine): If the Liver pulse is firm and long, but the complexion is not green, it indicates a fall injury. If the pulsation is due to blood accumulation under the ribs, it causes vomiting and regurgitation. *Jingui Yaolue* (Essential Prescriptions of the Golden Cabinet) states: If the Cun (Inch) position pulse is floating, faint, and choppy, it indicates blood loss or sweating. If there is no sweating, there should be sores on the body caused by knife or axe wounds, due to blood loss. *Maijing* (Pulse Classic) states: For open wounds with excessive bleeding, if the pulse is deficient, fine, deep, and small, survival is likely; if floating, rapid, full, and large, death ensues. For hacking/stabbing wounds with unceasing bleeding, if the pulse comes in strongly/largely, death occurs within seven days; if slippery and fine, survival is likely. From high falls causing internal blood stasis and abdominal distension, if the pulse is firm and strong, survival is likely; if small and weak, death ensues. For broken injuries with internal blood stasis, if the pulse is firm, strong, and full, survival is likely; if deficient, small, and weak, death ensues. If blood loss is excessive, if the pulse is fine and small, survival is likely; if floating, large, rapid, and full, death ensues. These are all instances where the pulse does not correspond to the disease.
Treatment Methods: For chest fullness and hypochondriac distension, promoting blood circulation is appropriate. For the elderly and weak, promoting and invigorating blood circulation is appropriate. For abdominal pain, purging blood (removing blood stasis via purgation) is appropriate. For blood stasis that does not dissipate, or ulcers that do not heal, major tonification of Qi and Blood is appropriate. If the trauma or fall is slight, without other signs of blood stasis, yet pain persists, only harmonizing Qi and Blood and regulating meridians is needed, after which the pain will cease naturally. Further nourishing Qi and Blood and strengthening the Spleen and Stomach will ensure efficacy. In cases where pain injures Stomach Qi causing vomiting or anorexia, Sijunzi Tang (Four Gentlemen Decoction) added with Danggui (Angelicae Sinensis Radix) and Sharen (Amomi Fructus) can be used for regulation. If blood stasis is not first dissipated before adding tonics, it creates a "double excess" disaster (exacerbating excess conditions). If there is no blood stasis but aggressive purgatives are wrongly administered, it leads to a "double deficiency" disaster (exacerbating deficiency conditions). Therefore, for such cases, one must examine the severity of the condition, the presence of blood stasis, and the strength of Yuan Qi (Original Qi). One must not indiscriminately use purgative methods, lest they transform into complicated/ruined syndromes. Generally, for blunt trauma or falls where the skin and flesh are intact but the abdomen hurts, there must be internal blood stasis, treated with Fuyuan Huoxue Tang (Restore Vitality and Invigorate Blood Decoction). For the elderly and weak, Siwu Tang (Four Substances Decoction) added with Honghua (Carthami Flos), Taoren (Persicae Semen), and Chuanshanjia (Manis Squama) is used to tonify while moving blood. If there is excessive blood loss accompanied by irritability, this is Blood deficiency, termed Wangxue (Loss of Blood); tonifying Blood is appropriate. If ineffective, Dushen Tang (Single Ginseng Decoction) should be used. Furthermore, for all injuries regardless of age or presence of blood stasis, Tongbian (Child Urine) is suitable. Administered with wine to push out the old and generate the new, its efficacy is great. For hypochondriac distension, pain, fever, irritability, dry mouth, and preference for cold drinks, drinking one bowl of hot Tongbian is superior to taking other medicines. Although other medicines may also be effective, since the presence or absence of blood stasis might not be fully recognized, they could mislead patients. Only Tongbian does not disturb the Zang-Fu organs nor injure Qi and Blood, ensuring safety. I once asked military personnel who often suffered horse-fall injuries how they healed. All replied that drinking hot Tongbian led to recovery. This repeated verification is clear. However, those with Spleen/Stomach deficiency causing vomiting, or Cold-deficiency diarrhea, must not take it. Generally, for swelling, pain, or injuries, crushed scallions roasted hot can be applied as a compress; alternatively, ginger and scallion whites crushed together, mixed with flour and roasted hot, are excellent. Ginger and aged rice wine lees crushed together and roasted hot for application are also acceptable. External formulas for trauma, such as Mituan Zhenggu Dan (Secretly Transmitted Bone-Setting Pill), Moyao Jiangsheng Dan (Myrrh Descending Sage Pill), Danggui Daozhi San (Angelica Guiding Stagnation Powder), Hei Wanzi (Black Pills), Ben Shi Jiegu Fang (Ability to Set Bones Prescription), Shiwei Moyao Wan (Ten-Ingredient Myrrh Pill), and Xing Sunshang (Washing Trauma), among over ten others, have wonderful uses and should be carefully examined.
Lizhai (Xue Ji) said: In the Ren Shen year, I was run over by a heavy cart, remaining unconscious for a long time before reviving. My chest felt full and tight like being beaten, and my breath was obstructed. I immediately drank a bowl of hot Tongbian, after which my chest relaxed and Qi flowed smoothly, though my lower abdomen still hurt. Mr. Xu Donghao, a Yintai (Censor) from my hometown, prescribed one dose of Fuyuan Huoxue Tang. I passed several shengs of bloody stool, and the swelling and pain completely subsided. I then took herbs to nourish Qi and Blood and recovered. In the Wu Chen year, while on official business at Juyong Pass, I saw seven or eight people injured in a carriage overturn, lying on the ground groaning, one unconscious. I had them all灌 (force-fed) hot Tongbian, and all were unharmed.
