Sun Simiao stated: In the disease of intestinal abscess (Chang Yong), the lower abdomen feels heavy and stiff, pain upon pressure, urination resembles strangury (Lin syndrome) with difficulty and pain, frequent sweating occurs, accompanied by aversion to cold. The skin is dry and scaly like fish scales, the abdominal wall is tense as if swollen. In severe cases, the abdomen becomes distended and large, with a splashing sound heard when turning the body. There may be sores around the umbilicus, pus discharging from the umbilicus, or pus and blood in the stool. A surging and rapid pulse indicates that pus has already formed; if blood (or pus and blood) flows downward, the condition will stabilize. If treated erroneously, it will inevitably lead to death.
Chen Wuze stated: In the disease of intestinal abscess, the body skin is dry and scaly, the abdominal wall is tense but soft upon pressure, showing initial signs of swelling, without accumulations in the abdomen, and without fever. A rapid pulse indicates pus within the intestine, formed by long-standing accumulation of yin-cold. Therefore, *Jin Gui Yao Lue* (Essential Prescriptions of the Golden Cabinet) uses Fuzi (Prepared Aconite Root) to warm. A slow and tight pulse indicates that pus has not yet formed; purgation can be used, and blood should be discharged. A surging pulse indicates that pus has formed; purgation is contraindicated. This is caused by internal heat binding. Therefore, *Jin Gui Yao Lue* uses Dahuang (Rhubarb Rhizome) to promote drainage and clear heat.
The moxibustion method in *Qian Jin Fang* (Prescriptions Worth a Thousand Gold): Bend both elbows, locate the sharp bone at the tip of the elbow, and apply 100 moxa cones. After pus and blood are discharged, the condition stabilizes.
A male patient presented with tenesmus (urgent need to defecate with heavy sensation in the anus), discharge of pus and blood, and abdominal distension and pain. This was due to Spleen Qi sinking. He recovered after taking Paishao San (Drain Pus Powder) and Lafan Wan (Beeswax and Alum Pill). Later, due to overwork, he experienced alternating chills and fever and physical fatigue. He stabilized after taking Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
A female patient had abdominal distension and pain due to formed pus, with difficult urination and a slippery, rapid pulse. This indicated internal rupture of pus toxin. She took Taiyi Gao (Great One Ointment), three qian, resulting in the discharge of about one sheng of pus, with immediate relief of distension and pain. Subsequently, Shenxiao Gualou San (Miraculous Trichosanthes Powder) was administered for two doses, leading to complete resolution of symptoms. Then, Lafan Wan and Tuoli medicines (medicines supporting the healthy qi to expel toxins) were taken for more than ten doses, leading to stabilization.
A postpartum woman suffered from lower abdominal pain and difficult urination. She was treated with Yiyiren Tang (Coix Seed Decoction); after two doses, the pain ceased. Then, Siwu Tang (Four Substance Decoction) was modified with Taoren (Peach Kernel) and Honghua (Safflower). After discharging about one sheng of stasis blood, she recovered.
A postpartum woman had lingering lochia and painful lower abdomen. She took Gua Zi Ren Tang (Melon Seed Decoction) and recovered after discharging stasis blood. Whenever blood stasis stagnates, urgent treatment is required; delay leads to putrefaction into pus, which is extremely difficult to treat. If it flows into the joints, it causes Gu Ju (bone abscess). Failure to treat properly often results in a fatal condition. (Commentary by Xue Ji)
