Fu Gu Ju (Abscess Attached to Bone), a condition commonly known in popular terms as Tie Gu Yong (Bone-Adherent Carbuncle). Any case where toxic pathogens are deepest and accumulate at the bone interface can be termed Fu Gu Ju. However, this condition is particularly prevalent in the thighs, where muscles are thick. The etiology of this disease includes: injury to tendons and bones from overwork damaging the meridians; consumption of yin essence due to excessive alcohol and sexual indulgence; stagnation of qi due to worry, thought, depression, and anger; and obstruction of meridians by wind-cold-dampness. Whenever there is unexplained soreness and pain at the Huantiao (GB30) acupoint that persists for a long time without healing, it is a sign of this condition. One must promptly treat according to the syndrome; delay is inadvisable. Initially, the condition merely involves a slight reversal and stagnation in the Shaoyang Meridian. If this reversal does not dissipate, it gradually becomes obstructed. Obstruction leads to swelling, swelling leads to ulceration, and once it spreads, all three Yin and three Yang meridians may be involved, leading to ulceration of the entire leg. Fundamentally, this condition arises from a severe deficiency of Yuan Qi (Original Qi), which fails to circulate and transport, causing pathogens to stagnate and eventually rupture. It is indeed a critical condition. If the pulse is harmonious after ulceration, despite extreme weakness, life can be preserved primarily by strongly tonifying Qi and Blood. However, if the pulse becomes surging or hollow (Kou) after ulceration, accompanied by restlessness, fever, and thirst, the condition is incurable.
Regarding treatment methods: For cases caused by tendon and bone injury, Daying Decoction (Great Camp Decoction) combined with Dafeng Tang (Major Wind-Dispersing Decoction) is appropriate. For cases caused by alcohol and sexual excess damaging Yin, Bawei Wan (Eight-Ingredient Pill), Liuwei Wan (Six-Ingredient Pill), or Yougui Yin (Right-Restoring Infusion), combined with Dafeng Tang, should be used. For cases caused by emotional stagnation, Chu Ke Liuqi Yin (Wound Department Qi-Circulating Infusion) or Wuxiang Lianqiao Tang (Five-Aromatic Forsythia Decoction), combined with Dafeng Tang, are suitable. For cases caused by external wind-cold invasion, Wuji San (Five Accumulations Powder) combined with Dafeng Tang is indicated. Generally, at the onset, Daying Decoction should be used to warm and tonify Qi and Blood, or combined with Xianfang Huoming Yin (Immortal Formula for Preserving Life) to promote the circulation of toxic pathogens. If heat signs are present, Lianqiao Guiwei Jian (Forsythia and Angelica Tail Decoction) should be used urgently to resolve toxicity. Urgent application of Garlic-Separated Moxibustion (Ge Suan Jiu) or Fermented Soybean Cake Moxibustion on the head of the lesion is also recommended to rapidly disperse toxicity, which is the most expedient method. For cases involving damp-heat or phlegm-fluids, refer to subsequent methods to avoid severe consequences. If pain at Huantiao persists or mild swelling appears in the buttocks/thighs suggesting pus formation that cannot dissipate, urgent use of Tuoli Bu (Supporting Interior Tonification) is required to secure the root. This allows rapid development and drainage; once the root is solidified, even severe cases will heal easily. If the pulse is slippery and rapid, and soft upon pressure, indicating mature pus, immediate needle drainage is necessary to prevent deep erosion of the bone. Those who ignore these principles and use draining/dispersing drugs further damage Yuan Qi, or apply cold compresses to suppress toxicity, will see the condition worsen daily until the Qi collapses and ulceration becomes uncontrollable. Since classical texts do not detail this condition, its causes and treatment notes are provided below.
Xue Lizhai stated: Fu Gu Ju can result from sleeping outdoors with deep wind-cold invasion into the bones; from physical depletion failing to expel pathogens; from using draining medicines that deplete Yuan Qi preventing expulsion; or from applying cold topical agents causing blood and qi coagulation internally. In all such cases, moxibustion and warming plasters should be applied to the affected area to disperse toxicity and tonify Yuan Qi, focusing on warming and tonifying the Spleen and Stomach. If appetite is normal, start with Xianfang Huoming Yin to resolve toxicity and disperse stagnation, followed by Liujunzi Tang (Six-Gentlemen Decoction) to tonify Ying Qi. If fatigue and poor appetite exist, use only the former decoction to nourish the Zang organs, preventing pathogens from harming healthy Qi. Once pus forms, needle it to prevent internal invasion; using a raw needle is acceptable. Fire needles cause no pain and facilitate healing. Garlic-separated moxibustion resolves toxicity and moves Qi; Scallion-warming therapy assists Yang Qi and clears stagnation. Though not in standard formularies, I find them highly effective, except in cases of Qi and Blood collapse. He further stated: Although swelling depth and pathogen severity vary, the underlying cause is always weak True Qi allowing deep pathogen invasion. If True Qi is strong, pathogens cannot cause harm. Thus, Fu Gu Ju and Crane Knee Wind are common in Kidney Deficiency. Ancients used Fuzi (Aconite Root) to warm Kidney Qi, promote drug action, and dispel cold. Also, weak individuals sleeping outdoors in autumn/summer may contract cold qi, leading to latent cold accumulation, immobility, alternating chills/fever without sweat, and deep bone pain on pressure. If unresolved, extreme Yin generates Yang, transforming cold into heat and ulcerating. If injured by "thief wind" (sudden cold), with mild local heat but systemic chills and sweating, stopping pain with warming, mild cases require Dafeng Tang and Huolong Gao (Fire Dragon Ointment); untreated, this leads to deformity or hemiplegia. Hard, stone-like lumps are called Shi Ju (Stone Abscess). Slow-healing, red-purple, rotting flesh ulcers are called Huan Ju (Slow Abscess). Throughout, the aforementioned decoctions are used to dispel cold and tonify deficiency to support the interior.
He also noted: Postpartum cases may involve residual lochia causing sharp abdominal pain, flowing to limbs or thighs, causing cone-like pain or thigh swelling. This results from cold-heat imbalance or mental/physical strain causing Qi stagnation and blood stasis in meridians; Moyao Wan (Myrrh Pill) is indicated. Cases of amenorrhea with limb flow or thigh pain due to water-dampness affecting menstruation require Danggui Wan (Angelica Pill). Stagnant lochia is dangerous; delayed treatment leads to bone abscesses and death.
Case 1: A woman with knee swelling/pain worsening with cold, lasting over a month, unresponsive to drugs, with wiry-tight pulse. Diagnosis: Deeply latent cold. Treated with Dafeng Tang and Huolong Gao, resolved.
Case 2: A man with pain penetrating bone near Huantiao, normal skin, rapid-sluggish-slippery pulse. Diagnosis: Fu Gu Ju, pus forming. Treated with interior-supporting herbs for six doses; swelling/pain increased, pulse became slippery-rapid, confirming pus. Needle drained about one bowl of pus. Added tonics; healed in over a month.
Case 3: A man with Fu Gu Ju, hard swelling, fever, bone pain, tendon spasm, rapid-deep pulse. Cured with Danggui Niantong Tang (Angelica Pain-Relieving Decoction).
Case 4: A man with leg carbuncle, diffuse swelling/pain, cold extremities, throat blockage, chills/fever, unresponsive to other treatments. Diagnosis: Pathogen stagnation in meridians. Treated with Wuxiang Lianqiao Tang; symptoms improved after one dose. After two bowel movements following continued use, fully recovered.
Case 5: A man with initial leg pain spreading to limbs, migratory, worse at night, unresponsive to damp-clearing/toxin-resolving drugs. Pulse slippery-choppy. Diagnosis: Damp-phlegm and turbid blood. Treated with Erchen Tang plus Cangzhu, Qianghuo, Taoren, Honghua, Niuxi, Caowu; cured. Damp-phlegm or dead blood in joints requires acrid-warm formulas to open pores, unblock channels, and harmonize Qi/blood.
Case 6: Wife of Wang Shiheng had postpartum waist/leg swelling due to blood stasis in meridians. Untreated, would become bone abscess. Treated with Tao Ren Tang (Peach Kernel Decoction) for two doses, then Moyao Wan several times; recovered. (Xue's Note)
Case 7: Mr. Wei, 30+, overworked. Developed Huantiao soreness, later thigh swelling. Diagnosed as Fu Gu Ju. Given Huoming Yin for two doses without effect; swelling worsened. Panic led to random clearing/cooling drugs, causing vomiting, fever, anorexia, critical state. Saved by Shenqi Neituo San (Ginseng Interior-Supporting Powder) with large amounts of Paojiang (Prepared Ginger); vomiting stopped, appetite returned. Swelling softened, pus formed. Needled, little pus drained. Given Jiwei Yigong Jian (Nine-Ingredient Extraordinary Efficacy Decoction); massive ulceration occurred (5-6 sites), flesh lost, leaving skin and bone. Post-ulcer vomiting/fever/anorexia treated alternately with Shiquan Dabu Tang (Ten Complete Tonifying Decoction) and Jiwei Yigong Jian. Heat subsided, appetite returned, complexion improved. Leg tendons shortened; patient could only lie supine with knees upright, immobile due to pain, believing himself disabled. Treated with over 80 doses of Shiquan Dabu Tang and 3 jin of Ginseng; leg flesh regenerated, tendons relaxed, restored to robust health. Entirely due to saving the root.
Case 8: Mr. Chen, nearly 30, unrestrained in desires. Developed Huantiao soreness for a month. Warned of abscess risk; he ignored me, consulted a quack who dismissed it as wind-heat. Treated with wind-dispersing/heat-clearing drugs. Six months later, mild swelling appeared. I advised urgent interior support; he refused, consulting a surgeon who forbade tonification before ulceration. Continued cooling/clearing drugs. When massive ulceration occurred, he sought me again. Pulse/symptoms were depleted; he believed my words too late. Regret was futile.
Case 9: Young nobleman surnamed Mao, under 30, indulgent in wine/women. Developed this condition. Refused early medication. When swollen and pus formed, refused needling due to fear of pain. Listened to quacks, applied bitter-cold detoxifying topicals. Unaware that once pus forms, detoxification is contraindicated; condition worsened, waiting for spontaneous rupture, exhausting Yuan Qi beyond repair. (Xue's Note)
