Bi syndrome is caused by external pathogenic factors, with the disease root located in the meridians; if it penetrates deeply, it involves the zang-fu organs. Therefore, when located in the upper body, it presents with panting and vomiting, or regurgitation of food; when located in the middle, it manifests as distension and fullness, and pain; when located in the lower body, it results in undigested diarrhea (Sun Xie), constipation, and other diseases. These are all concurrent syndromes of Wind-Bi. For any such manifestations, one should weigh the urgency and sequence according to respective categories and treat based on the specific syndrome.
1. In Bi syndrome where Wind predominates, treatment should focus on dispersing. It is appropriate to use formulas such as Baidu San (Resolving Toxicity Powder) or Wuyao Shunqi San (Aucklandia Decoction for Regulating Qi). If Wind predominance is accompanied by slight Heat/Fire, Da Qin Jiao Tang (Great Notopterygia Decoction) or Jiu Wei Qiang Huo Tang (Nine-Ingredient Notopterygia Decoction) may be used.
2. In Bi syndrome where Cold predominates, provided there are no Heat signs in both the exterior and interior, treatment should employ warmth. Appropriate formulas include Wu Ji San (Five Accumulations Powder), Xiao Xuming Tang (Minor Chengming Decoction), or Gancao Fuzi Tang (Licorice and Aconite Decoction). If Cold is severe and Qi is deficient, Fuzi Tang (Aconite Decoction) from *San Yin Fang* (Treatise on Causative Factors of Diseases) is suitable.
3. In Bi syndrome where Dampness predominates, the patient’s body will necessarily feel heavy, often accompanied by aversion to cold, excessive phlegm, or profuse sweating. These are all syndromes of Spleen deficiency with Yin-Cold. Among them, Qianghuo Shengshi Tang (Notopterygia Decoction for Overcoming Dampness) is a formula that disperses Wind and eliminates Dampness; Wu Ji San (Five Accumulations Powder) warms the meridians and dissipates Dampness; Zhenwu Tang (True Warrior Decoction) warms the middle and removes Dampness; Fuzi Tang from *San Yin Fang* tonifies the Spleen and dries Dampness; Tiaoqi Pingwei San (Regulating Qi and Harmonizing Stomach Powder) promotes Qi movement and resolves Dampness; Wuling San (Five-Ling Powder) promotes urination to guide out Dampness; Erchen Tang (Two-Aged Powder) and Liujunzi Tang (Six-Gentlemen Decoction) resolve Phlegm and eliminate Dampness. Generally, treating Dampness requires drying; to dry, one should employ warmth. This is because the Earth element of the Spleen prefers dryness and dislikes dampness, prefers warmth and dislikes cold. Thus, warming the Spleen is equivalent to treating Dampness. However, there are cases of Damp-Heat pathology. Only when internal Heat signs and a slippery-rapid pulse are present can cooling methods be applied, using formulas such as Ermiao San (Two-Marvels Powder), Jiami Ermiao Wan (Modified Two-Marvels Pill), or Danggui Niangtong Tang (Angelica Decoction for Relieving Pain). For severe heat, Chuxin Yin (Drawing Out Fire Decoction) may be temporarily used to first clear the Fire before regulating Qi and Blood.
4. Wind-Bi syndromes are mostly due to deficiency and often involve Cold. Because Qi and Blood are not sufficient, Wind and Cold pathogens can invade; because Yin pathogens remain stagnant, the meridians become obstructed. This constitutes the main mechanism of Pain-Bi. Only formulas such as Sanqi Yin (Three-Qi Infusion) and Dafengfang Tang (Great Wind-Preventing Decoction) can achieve efficacy. Among all methods for treating Bi, this is considered the best. For those suitable for wine therapy, soaking Sanqi Yin in wine for administration is also excellent (see method in the original formula description); alternatively, Yilao Tianma Wan (Old Yi’s Gastrodia Pill) may be used.
