赤小豆是色赤心红的谷物。其性质下行，通达小肠，能进入阴分，治疗有形的病证。因此能运行津液，利小便，消除胀满去除水肿，止吐并治疗下痢肠澼。解除酒病，去除寒热痈肿，排脓散血并通乳汁，下胞衣治疗难产。这些都是针对有形病证的久服之法，但如果服用过度，会导致降泄太过，津液血液渗漏流失，所以会使人肌肉消瘦身体沉重。它吹鼻用的瓜蒂散以及辟瘟疫时使用的方剂中用到它，也是取其通气除湿散热的作用。对于水气病和香港脚是最为急需使用的。当今治疗水气病的人，只知道治水，却不知道补胃，这样就会造成壅滞。赤小豆能消水利气且健脾胃，这才是它的药效所在。
