The above-mentioned medicines are pounded and sifted into a powder. Each dose is four qian (approx. 12-15g), decocted with one zhong zhan (approx. 200ml) of water until reduced to six fen (approx. 120ml). Strain the dregs and take warm, regardless of meal times. (*Shenghui Fang*)
Shen Wangge suffered from jaundice; his entire body and face were yellow, with a slight blackish stagnation. He experienced irritability, thirst, and abdominal fullness, and sought treatment from Yu. The pulse on the left was wiry and rapid, while the pulse on the right was empty and large. This indicated jaundice due to internal injury, a sign of mutual damage between the Jueyin Liver Wood and the Taiyin Spleen Earth. Thus, the treatment strategy was formulated: to calm the hyperactivity of the Liver, support the deficiency of the Spleen, simultaneously resolve stagnant heat to clear the Qi pathway, and remove damp-heat steaming to harmonize the Middle Qi. The prescription consisted of Renshen (Ginseng Radix) 3 qian, Baizhu (Atractylodis Rhizoma) 2 qian, Baishao (Paeoniae Radix Alba) 1 qian, Huanglian (Coptidis Rhizoma) and Zhizi (Gardeniae Fructus) each 7 fen, Danggui Shen (Angelicae Sinensis Radix) [specifically the root body], Danpi (Moutan Cortex) Yinchen (Artemisiae Scopariae Herba), and Qinjiao (Gentianae Macrophyllae Radix) each 1 qian, Chaihu (Bupleuri Radix) 7 fen, Zhi Cao (Glycyrrhizae Radix et Rhizoma Preparata) 5 fen, and Banqu (Medicated Leaven) 1 qian. After taking thirty doses, all ailments vanished suddenly. (*Yinji Cao*)
Methods for treating interior syndromes: When jaundice arises from the stagnation and steaming of damp-heat, purgative methods should be employed to drain it. However, one must also emulate the methods for treating Cold Damage (Shanghan); purgatives are applicable only when there is definite interior heat. For severe cases, Dahuang Xiaoshi Tang (Rhubarb and Gypsum Decoction) is used to scour away damp-heat, similar to the usage of Dachengqi Tang (Major Order the Qi Decoction). For slightly milder cases, Zhizi Dahuang Tang (Gardenia and Rhubarb Decoction) is used to clear heat and resolve exterior symptoms while simultaneously purging, similar to Sanhuang Tang (Three Yellow Decoction). For even milder cases, Yinchenshao Tang (Artemisia and Coptis Decoction) is used as the principal formula to clear and resolve, with a slight addition of Dahuang (Rhubarb) as an assistant, analogous to adding Dahuang in Zhichi Tang (Gardenia and Fermented Soybean Decoction) in the size of a chess piece. Therefore, sweating methods dare not be used lightly, and purgative methods must also be applied cautiously, because jaundice syndromes often involve internal injury, necessitating protective measures. (*Yixue Xinfa* commentary) (Note: Zhizi Dahuang Tang contains one liang of Dahuang, whereas Yinchenshao Tang contains two liang, indicating their relative severity. Yu Jianyuan's statement is not entirely accurate. Moreover, the distinction between these two formulas lies in whether the pathology is in the Upper or Middle Jiao, rather than merely in dosage.)
Prescription for treating jaundice where the body, face, and eyes are golden yellow, urine resembles concentrated boiled Phellodendron bark juice, and other physicians failed to treat it:
Yinchen (Artemisiae Scopariae Herba) and Zhizi (Gardeniae Fructus) (each 2 liang), Huangqin (Scutellariae Radix), Chaihu (Bupleuri Radix), Shengma (Cimicifugae Rhizoma), Dahuang (Rhei Radix et Rhizoma) (each 3 liang), Longdancao (Gentianae Radix) (2 liang).
The above seven ingredients are cut, decocted in eight sheng (approx. 1600ml) of water until two sheng seven he (approx. 420ml) remain. Divide into three doses. If the body is emaciated, remove Dahuang and add Zhizi Ren (Gardeniae Semen) 5-6 liang and Shengdihuang (Rehmanniae Radix) one sheng. If jaundice has persisted for a long time and changed to a peach-skin color (Note: Xian Jiaoyu stated that peach-skin color refers to a dull, dark hue lacking luster), with hardness under the heart, vomiting and inability to eat, extremely red and scanty urine, cold limbs, and a deep, extremely faint, fine, and slow pulse, this prescription is contraindicated; administration will lead to adverse transformations. Instead, Da Yinchensheng Tang (Large Artemisia and Rehmannia Decoction) [composed of Yinchen, Huangbai, Dahuang, Huangqin, Gualougen, Gancao, Baizhu, Fuling, Qianhu, Zhishi, Zhizi] should be given, removing Dahuang and adding Shengdihuang 5 liang. After finishing the decoction, observe the pulse; if it becomes slightly floating and superficial, and the condition is no longer deeply sunken and faint, treatment may proceed. If the pulse floats, the yellowing will clarify, the peach-skin color will disappear, and the epigastrium will feel comfortable. (*Qianjin Yaofang*)
After administering the previous Mahuang (Ephedra) five-ingredient decoction (see sweating section above) to induce sweating, without resting, after three to five days, administer the Zhizi (Gardenia) five-ingredient decoction to promote urination or bowel movement.
Zhizi (Gardeniae Fructus) (20 seeds), Chaihu (Bupleuri Radix) (3 liang), Huangqin (Scutellariae Radix) (3 liang), Yinchen (Artemisiae Scopariae Herba) (3 liang), Mangxiao (Natrii Sulfas) (6 liang).
Cut the above medicines, decoct the first four in eight sheng of water until two sheng six he remain. Strain, add Mangxiao, stir until dissolved, and divide into three warm doses. If effective, administer again after walking the distance of ten li. (*Waitai Miyao*, citing Xu Renze)
Da Huang Wan (Rhubarb Pill) for treating jaundice with yellow complexion and emaciation:
Dahuang (Rhei Radix et Rhizoma) (cut, 1 jin), Xiong (Ligustici Chuanxiong Rhizoma) (0.5 jin).
Pound and sieve the above two ingredients into powder. Mix with honey to form pills the size of paulownia seeds. Take thirty pills per dose with cooked water, three times daily: on an empty stomach, before meals, and before bed. (*Shengji Zonglu*)
Wang Wenchuan’s son had originally injured his diet and then consumed cold water chestnuts, resulting in generalized yellowing, golden eyes, night fever that subsides at dawn, severe abdominal pain intolerable to touch, and screaming throughout the night. City physicians, seeing the jaundice, diagnosed it as "Hu" syndrome and proposed herbal remedies. Upon arrival, I urgently stopped them, saying, "You nearly killed his mother with herbs; do you wish to kill his son too?" The Spleen and Stomach prefer warmth and dislike cold. This case is jaundice caused by food stagnation fermenting into damp-heat. The method should be to strengthen the Spleen and use warm purgatives. Once damp-heat is removed, the jaundice will recede. Most herbs are cold in nature; using them damages Spleen Earth and exacerbates the disease. I prescribed Baohe Wan (Harmonize the Stomach Pill) 1 qian mixed with Beijiji Wan (Emergency Rescue Pill) 0.5 fen, taken once. Shortly after, he defecated once, then three more times consecutively. A large amount of accumulated matter was expelled, and abdominal pain ceased completely. Subsequently, Tiao Zhong Wan (Regulate the Middle Pill) was administered for one month. Not only did the jaundice disappear entirely, but his gait became light and swift. (*Chuishui Xuanzhu*)
Formulas for promoting bladder function: Jiawu Wu Ling Tang (Modified Five-Ingredient Powder with Poria) treats jaundice due to latent summer-heat stagnation, with irritability, thirst, and dysuria.
Chi Fuling (Poria with Red Skin, peeled), Zhuling (Polyporus), Zexie (Alismatis Rhizoma), Baizhu (Atractylodis Rhizoma), Yinchen (Artemisiae Scopariae Herba) (equal parts).
Cut into pieces. Each dose is four qian, decocted in one and a half zhan of water until eight fen remain. Strain and take warm, regardless of meal times. (*Jisheng Fang*)
Fuling Shen Shi Tang (Poria Damp-Draining Decoction) treats jaundice with alternating chills and fever, vomiting, desire for cold drinks, generalized yellowing of body and face, dysuria, anorexia, and insomnia.
Yinchen (Artemisiae Scopariae Herba) (6 fen), Bai Fuling (Poria) (5 fen), Mu Zhuling (Polyporus), Zexie (Alismatis Rhizoma) (each 3 fen), Huanglian (Coptidis Rhizoma), Huangqin (Scutellariae Radix, raw), Zhizi (Gardeniae Fructus), Han Fangji (Stephaniae Tetrandrae Radix), Baizhu (Atractylodis Rhizoma), Cangzhu (Atractylodis Rhizoma), Chenpi (Citri Reticulatae Pericarpium), Qingpi (Citri Reticulatae Pericarpium Viride) (each 2 fen).
Cut the above twelve ingredients. One dose is decocted in two zhan of water until one zhan remains. Strain and take warm, on an empty stomach before meals. (Effective for Kang Erge) (*Yixue Mingfa*). Additionally, for Grain Jaundice, Fuling Zhizi Yichen Tang (Poria, Gardenia, and Artemisia Decoction) adds Zhishi (Immature Bitter Orange) to this base formula. *Tongzhi* removes Huangqin and Qingpi, adding Qinjiao and Gegen. *Jishi Quanshu* Yinchens Wu Ling San (Artemisia Five-Ingredient Powder) removes Huangqin, Huanglian, Fangji, Chenpi, and Qingpi, adding Huashi (Talcum) and Guangui (Cinnamon Bark). *Huichun* Yinchens Tang (Artemisia Decoction) removes Huangqin, Fangji, Baizhu, Chenpi, and Qingpi, adding Zhishi and Houpo.
Tongtai Wan treats damp-heat in the five types of jaundice, with yellowing of the body, urine, and sweat, thirst, and epigastric fullness.
Zhizi Ren (Gardeniae Semen) (1 liang), Yinchen (Artemisiae Scopariae Herba) (2 liang), Haijinsha (Lygodii Spora), Mutong (Akebiae Caulis), Chi Fuling (Poria with Red Skin), Huashi (Talcum, fried), Huanglian (Coptidis Rhizoma) (each 1 liang), Dahuang (Rhei Radix et Rhizoma) (1 liang).
Grind into powder, make pills with thin paste the size of paulownia seeds. Take sixty pills per dose, washed down with Yichen decoction. (*Huoren Xintong*)
Warm-Middle Formulas: Lizhong Jia Fuling Tang (Li Zhong Decoction plus Poria) treats cold-damp jaundice. Although the urine is yellow-red, it is treated as a cold syndrome based on accompanying symptoms and pulse differentiation. (*Bencao Gangmu*) (Note: Originally cited Zhang Zhongjing, but the source is unclear and includes redundant cases; only key points are excerpted here.)
In the year Wuyin, during medical practice at Zhishan Temple, a man in his fifties suffered from jaundice. His whole body and eyes were yellow, urine was yellow-red and black, he felt hot yet desired proximity to fire. Medicines for clearing damp-heat worsened his condition. After taking Zhizi Yichen Tang (Gardenia and Artemisia Decoction) in one day, his hands and feet became cold, he experienced tremors and rigors, and his vision dimmed. Believing himself incurable, he was carried to the temple for consultation. His pulse was deficient, fine, and scattered in all six positions. I prescribed Lizhong Fuling Tang (Li Zhong Decoction plus Poria) for ten doses, which stabilized him. Feeling hot yet desiring fire indicates heat on the skin but cold in the marrow. Black urine and a deficient, fine pulse indicate overwhelming cold-damp. Tremors and rigors are symptoms of Cold Water in the Foot-Taiyin. This patient took too many cold-cooling medicines, causing excessive Yin qi and impending Yang collapse. Without urgent rescue using warm-middle agents, recovery would be impossible. This is precisely "driving out the bandit while injuring the lord." (*Yiyao Zhinan*) (Note: This case is adapted from a verification record in *Gangmu*, organized for clarity and preserved here.)
Sijunzi Tang (Four Gentlemen Decoction) (*Heji Ju Fang*) treats Color Jaundice.
Add Huangqi (Astragali Radix, prepared), Baishaoyao (Paeoniae Radix Alba), and Baibidou (Phaseoli Semen) (prepared, each 1 liang).
Cut finely. Each dose is three qian, decocted with five slices of ginger and two jujubes. (*Zhizhi Fang*)
If due to chronic illness, Yuanqi (Primordial Qi) is increasingly deficient, and the true color of the Spleen appears externally, use Buzhong Yiqi Tang (Middle-Supplementing Qi-Boosting Decoction) plus Yinchen and Fuling. If there is Middle Qi deficiency-cold with cold extremities, or edema with black-yellow discoloration, use Renshen Lizhong Tang (Ginseng Li Zhong Decoction) plus Fuling and Yinchen. (*Mingyi Zazhu* commentary)
Shuhuang Yin (Clearing Yellow Drink) treats cold-type jaundice.
Baizhu (Atractylodis Rhizoma), Renshen (Ginseng Radix), Ganjiang (Zingiberis Rhizoma), Yinchen (Artemisiae Scopariae Herba), Gancao (Glycyrrhizae Radix et Rhizoma) (*Yijing Huijie*)
Nourishing Formulas: Yangrong Tang (Nourish Ying Decoction) treats the five types of jaundice with weak legs, palpitations, bland taste, tinnitus, mild chills and fever, shortness of breath, and turbid white urine. It should be treated as Consumption (*Xulao*). (*Sanyin Fanglun*) (Note: *Jisheng Fang* states below: Do not recklessly administer cooling agents, as they further damage Blood and Qi. Instead, prescribe Qinjiao Yinzi (Gentiana Decoction), which is Yangrong Tang minus Huangqi, Wuweizi, and Renshen, plus Qinjiao, Chuanxiong, and Banxia.)
For jaundice where urination is spontaneous, urine is white like rice washings, caused by excessive alcohol consumption, Tu Gua Gen San (Trichosanthes Root Powder) is indicated.
Tu Guagen (Trichosanthes Radix) (1 liang), Baishizhi (Halloysitum Rubrum) (1 liang), Guixin (Cinnamomi Ramulus) (1 liang), Gualougen (Trichosanthis Radix) (1 liang), Tusizi (Cuscutae Semen) (1 liang, soaked in wine for one day, dried, separately pounded into powder), Muli (Ostreae Concha) (1 liang, calcined to ash).
Pound the above medicines, sieve finely into powder. Each dose is two qian, taken with barley porridge drink, three to four times daily. (*Shenghui Fang*) (Note: This originates from *Qianjin* Ningshuishi San (Calcined Water Stone Powder) for Flesh Jaundice, characterized by low fluid intake but high urine output, urine white like rice washings, caused by alcohol. Now, Ningshuishi and Zhimu are removed, and Tu Guagen and Muli are added.)
Lu Rong San (Deer Antler Powder) for treating "Fang Huang" (Jaundice due to sexual excess):
Lu Rong (Cervi Cornu Pantotrichum) (1 liang, de-haired, brushed with vinegar, lightly fried), Shudihuang (Rehmanniae Radix Preparata) (1 liang), Shanzhuyu (Corni Fructus) (1 liang), Wuweizi (Schisandrae Fructus) (1 liang), Huangqi (Astragali Radix) (1 liang, cut), Muli (Ostreae Concha) (1 liang, calcined to ash).
Pound the above medicines, sieve finely into powder. Take two qian with warm wine, regardless of meal times. (Ibid.)
Shengxue Runfu Tang (Generate Blood and Moisten Skin Decoction) treats Dry Jaundice, where blood fails to nourish the skin, resulting in dry, yellow, and withered skin.
Moistening Skin Generating Blood Wine Honghua (Carthami Flos), Wuweiizi (Schisandrae Fructus), Shuangmen [likely referring to specific herbs or typo], Erdi [Rehmannia], Ma [Hemp seed?], Gui [Angelica], Qin [Scutellaria], Kuo [Trichosanthes], Taoren (Persicae Semen) crushed. Several doses yield remarkable effects on the skin. (*Yidu*)
Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) treats Dry Jaundice. (Ibid.)
Nasal Administration Methods: In epidemic febrile diseases, internal stasis often leads to jaundice. Simply insert Guading San (Melon Seed Powder) into the nose. When yellow fluid discharges, the patient recovers and will not suffer from jaundice again. One must vigilantly monitor patients; if any slight yellowish tint appears on the limbs, body, or face, Guading San must be used immediately. Delaying treatment leads to critical danger. Alcohol and wheat products are strictly forbidden; violation results in death. (*Qianjin Yifang*) (Note: *Qianjin* and *Waitai* contain various formulas; only one is recorded here. Also, *Benshi Fang* has verification records for reference.)
Guading San (Melon Seed Powder) for persistent jaundice with yellow eyes:
Finely powder the melon seeds (Gua Di). Place an amount the size of a large soybean into the nose. Instruct the patient to inhale deeply. Discharge of yellow nasal fluid indicates cure. (Ibid.) (Note: *Lantai Guifan* cites *Jinxiao* Guadi San: For mild cases, the size of a small bean suffices. If ineffective, repeat frequently every other day. Alternatively, blow into the nose forcefully using a bamboo tube; this is invariably curative. Also, sniffing out yellow nasal fluid was practiced before the Tang Dynasty. Some used the white membrane inside a gourd, ground fine with a little musk, blown into the nose to induce discharge.)
Regulation Methods: Strictly avoid wheat products, meat, vinegar, fish, garlic, leeks, and hot foods. Violation leads to death. (*Qianjin Yifang*)
If the Spleen and Stomach are slightly robust, and rich flavors are abstained from, it is treatable. However, if caused by alcohol and sexual excess, or indulgence in diet, it is difficult to treat. (*Yixue Rumen*)
