Malaria (a periodic febrile disease characterized by chills, high fever, and headache) includes wind-summerheat malaria, mountain miasma malaria, food-stagnation malaria, chronic malaria (old malaria), malaria mass (splenomegaly), cold-dampness malaria, bi-syndrome type malaria, five-zang malaria, and six-fu malaria.
Dispersing Method: For cases with sweating where cessation of sweat is desired, the primary approach is to support healthy Qi while simultaneously dispersing pathogenic factors; Xiaochaihu Tang (Minor Bupleurum Decoction) added with Guizhi (Cinnamon Twig) or Baihu Jia Guizhi Tang (White Tiger Decoction with Cinnamon Twig) may be used. If headache, floating pulse, aversion to wind, and sweating are present, according to *Jiyao* (Essentials), Guizhi Qianghuo Tang (Cinnamon Twig and Notopterygium Decoction) should be used.
Harmonizing Method: For cases without sweating where induction of sweat is desired, the primary approach is to disperse pathogens while supporting healthy Qi; Guizhi Jia Huangqin Zhimu Shigao Tang (Cinnamon Twig Decoction with Scutellaria, Anemarrhena, and Gypsum), Renshen Chaihu Yinzi (Ginseng and Bupleurum Drink), or Liuhe Tang (Six-Harmony Decoction) may be used. The composition of Liuhe Tang is as follows: Renshen (Ginseng Root), Zhimu (Anemarrhena Rhizome), Caoguo (Tsaoko Fruit), Beimu (Fritillaria Bulb), Baizhi (Angelica Dahurica Root), Binglang (Areca Seed) (each 1 qian); Wumei (Mume Fruit), Chaihu (Bupleurum Root) (each 1 qian, soaked in wine); Changshan (Dichroa Root) (2 qian).
Warming Method: When chills predominate over fever, or when there is only chills without fever, it is called Muzhang (Male Malaria); Chaihu Jiangui Tang (Bupleurum, Ginger, and Cinnamon Twig Decoction) is indicated, which aims to warm the meridians.
Purgative Method: When heat predominates over chills, accompanied by eye pain, profuse sweating, and a large pulse, Chaihu Tang (Bupleurum Decoction) may be used to induce mild purgation as a limit. If residual pathogens remain, Baizhi, Shigao (Gypsum Fibrosum), and Zhimu (Anemarrhena Rhizome) Sanwu Tang (Three-Substance Decoction) may be used to eliminate remaining pathogens.
Regulating Meridians Method: When wind-summerheat pathogens penetrate deeply into the Yin level and manifest at night, it indicates that the Blood is affected; drugs such as Guizhi (Cinnamon Twig), Taoren (Peach Kernel), Danggui (Angelica Sinensis Root), Chaihu (Bupleurum Root), and Huangqin (Scutellaria Root) should be used.
Dispelling Miasma Method: Use Caodoukou (Black Cardamom), Houpo (Magnolia Bark), Caoguo (Tsaoko Fruit), Binglang (Areca Seed), Banxia (Pinellia Tuber), and Changshan (Dichroa Root).
Warming Spleen Method: Due to internal injury from raw and cold foods, Renshen (Ginseng Root) may be added to Pingwei San (Stomach-Pacifying Powder), or formulas containing Shoutian (Atractylodis Rhizoma, i.e., Cangzhu), Fuling (Poria), Caoguo (Tsaoko Fruit), Huoxiang (Patchouli Herb), Shengjiang (Fresh Ginger), and Wumei (Mume Fruit) may be used; Feilong Duoming Dan (Flying Dragon Life-Extending Pill) may also be used.
Eliminating Masses Method: Cough-type malaria often forms masses; those located under the left hypochondrium are called Muzhang (Malaria Mass); Yan Shi's Biejia Yinzi (Bivalve Shell Decoction by Dr. Yan) or Jingui's Biejia Jian Wan (Bivalve Shell Decoction Pills from the Golden Cabinet) may be used.
Digesting Food Method: Use Sanyin Hongwanzi (Red Pills of the Three Causes).
Exerting Vomiting Method: When external pathogens affect the Upper Jiao (Upper Burner) chest and diaphragm, phlegm and saliva often accumulate; thus, emesis is appropriate. Hezi's Hengshan San (Hengshan Powder), Lili San (Veratrum Powder), or Xionghuang San (Realgar Powder) may be used.
Intercepting Malaria Method: Chen Sha Dan (Cinnabar Pill) and Yijian Jin (One-Shear Gold) are essential medicines. The formula consists of: Zhusha (Cinnabar) (1.5 liang, reserving 0.5 liang for coating), Xinshi (Arsenic Trioxide), and Xionghuang (Realgar) (each 5 qian). These are ground into powder, mixed with 6 qian of white flour, and made into pills the size of mung beans. One pill is taken orally with water during the Wu Geng (early morning, around 3-5 AM) period.
Tonifying Method: In cases of excessive sweating, persistent chills and fever, reluctance to speak, and poor appetite, Xiaochaihu Tang (Minor Bupleurum Decoction) added with Guizhi (Cinnamon Twig), Baizhu (Atractylodes Macrocephala Rhizome), Chenpi (Tangerine Peel), Danggui (Angelica Sinensis Root), Caoguopi (Tsaoko Peel), Shaoyao (Peony Root), Huangqi (Astragalus Root), Zhimu (Anemarrhena Rhizome), and Beimu (Fritillaria Bulb) may be used. If symptoms persist, the following medicine may be taken concurrently.
For malaria occurring every three days, or once every month or two months, the secret formula Qingpi Wan (Clearing Spleen Pill) is applicable: Baizhu (Atractylodes Macrocephala Rhizome) (1.5 liang), Renshen (Ginseng Root), Binglang (Areca Seed), Caoguo (Tsaoko Fruit), Huangqin (Scutellaria Root), Shoutian/Cangzhu (Atractylodis Rhizoma, processed) (each 1 liang), Qingpi (Green Tangerine Peel) (1 liang), Pengzhu (Curcuma Zedoaria Rhizome, stir-fried with vinegar), Houpo (Magnolia Bark) (each 0.5 liang), Gancao (Licorice Root), Jianghuang (Turmeric Rhizome) (each 3 qian). These are ground into powder, made into pills the size of paulownia seeds using rice paste, and 50-60 pills are taken with plain hot water shortly after meals, twice daily.
Shifting Spirit and Changing Qi Method: Inducing fear through startling or striking, hiding fears, gazing into water mirrors, drinking talismanic water, or engaging in intense chess games can all stop malaria; however, if the pathogen has transmitted to the interior organs, it cannot be stopped by these methods.
Acupuncture Points: Hegu (LI4), Quchi (LI11), Gongsun (SP4).
Moxibustion: Regardless of gender, at the end of the first segment of the Dazhui (GV14) acupoint, acupuncture should be performed first, followed by moxibustion of 21 cones (San Qi Zhuang), yielding immediate results; alternatively, moxibustion at the third segment may also be used.
