*Chanru Ji* (Collection on Childbirth and Lactation) records: Fill the umbilicus with salt until level. Pound white parts of scallions (*Congbai*, Allii Fistulosi Bulbus), spread them to a thickness of about one finger, place them over the salt, and moxibustion is performed on top of the scallions using moxa cones. When heat sensation enters the abdomen, urination will immediately become unobstructed; this method is most effective.
Shen Yaofeng stated: If this method fails, it must be due to Qi deficiency failing to lift and support. Huangqi (Astragali Radix) possesses an ascending nature within its Qi-tonifying properties; use it as the Sovereign herb (Jun Yao) at 5 qian. Maidong (Ophiopogonis Radix) can clear the upper source (Lung); use it as the Minister herb (Chen Yao) at 1.5 qian. Baotongcao (Tetrapanacis Medulla) reaches the lower jiao; use it as the Assistant herb (Zuo Yao) at 0.8 fen. Decoct in water and take one dose for efficacy.
*Daquan* states: Postpartum hematuria, sallow complexion, hypochondriac distension, and poor appetite indicate Liver Wood overacting on Spleen Earth. Use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), decocted and taken orally, for cure.
Zhu Danxi said: I have observed that careless midwives may damage the postpartum woman's bladder, causing dribbling symptoms that eventually become a chronic disability. There was a young woman who suffered this after difficult labor. I reasoned that external muscle injuries can be repaired; although the bladder is internal, it should also be treatable. Upon examining her pulse, it was extremely deficient. I said: The cause of difficult labor is often Qi deficiency, and postpartum Qi and Blood are especially deficient; let us try vigorous tonification. Therefore, Renshen (Ginseng Radix) and Huangqi (Astragali Radix) were used as Sovereign herbs; Chuanxiong (Ligustici Rhizoma et Radix) and Danggui (Angelicae Sinensis Radix) as Minister herbs; Taoren (Persicae Semen), Chenpi (Citri Reticulatae Pericarpium), and Fuling (Poria) as Assistant herbs. Pig or sheep bladders were decocted in water, and the patient drank this when extremely hungry. However, the dosage was small, typically only 1 liang, and she stabilized after one month. This allowed Qi and Blood to increase rapidly, so the bladder healed itself; delay might have hindered success.
In another case where the urinary bladder was injured during delivery, causing dribbling urine, two silkworm cocoons were burned to retain their carbonized nature, ground into powder, and taken for one month for cure. (This is a verified treatment by Miao Deren).
Xue Lizhai stated: For Qi and Blood deficiency, Shiquan Dabu Tang (Ten Substance Great Tonifying Decoction) is indicated.
Xue Lizhai stated: For deficiency-heat in the Liver channel, Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) is indicated.
If bearing down too early causes injury to the birth canal, resulting in redness, swelling, ulceration, and unbearable pain, wash with decoction made from lotus leaves inside a steamer basket three times daily; healing occurs in two days. (This is a verified treatment by Miao Deren).
Chen Wuyue stated: Postpartum vaginal prolapse, resembling rectal prolapse, with downward protrusion of the vagina, oppressive swelling and pain, triggered by movement or sexual fatigue, continuous discharge of clear fluid, and dribbling urine. Treat with Liu Huang (Sulfur) and Hai Zhe Ke (Sepiae Endoconcha), each 2 liang, and Wuweizi (Schisandrae Chinensis Fructus), 2.5 qian, ground into powder and applied locally three times daily.
Zhu Danxi said: A woman presented with a mass protruding from below the vulva after childbirth, shaped like an inverted bowl with two branches. Her husband sought treatment. I reasoned: This is the uterus, likely prolapsed due to weak Qi and Blood. I prescribed several doses containing Shengma (Cimicifugae Rhizoma), Danggui (Angelicae Sinensis Radix), and Huangqi (Astragali Radix). Half a day later, the husband returned saying that after two doses, there was a popping sound, and examination showed the organ had retracted into the vagina. However, having been exposed overnight, it adhered dryly to the mat, tearing off a piece the size of a palm. The wife wept at home, fearing infertility due to the rupture. I reasoned this was not intestine but fatty membrane. Muscle tears can heal; if Qi and Blood are abundant, regeneration is certain. I prescribed Siwu Tang (Four Substances Decoction) with added Renshen (Ginseng Radix) for 100 doses. Three years later, she gave birth again.
Formula for Uterine Prolapse: Huangqi (Astragali Radix) 1.5 qian, Renshen (Ginseng Radix) 1 qian, Danggui (Angelicae Sinensis Radix) 7 fen, Shengma (Cimicifugae Rhizoma) 3 fen, Gancao (Glycyrrhizae Radix et Rhizoma) 2 fen. Prepare as one dose, decoct in water, and take before meals. Externally, wash with infusion of Wubeizi (Galla Chinensis) powder and apply the powder. Repeat several times. Oral medication should be continued extensively to prevent recurrence.
