Discussion:
Except for the conditions of gan erosion (nutritional deficiency ulceration) and ban chuang (macular lesions), all other eye affections are caused by accumulated heat-toxin within the five zang organs. Additionally, some cases arise from qi reception during fetal development, where the mother consumed heat-toxin substances; as the child grows slightly, the toxins interact and suddenly cause opacities and blurred vision. These do not manifest primarily through swelling, redness, or heat-toxin accumulation symptoms. Such cases are determined to be difficult to treat. If cold-cooling herbs are added, the harm is even more severe. In cases presenting with redness, swelling, purulent discharge, tearing, pain, and corneal opacity, one must strictly avoid heat-toxin inducing foods. The treatment principle involves clearing the heart and cooling the liver, regulating qi and promoting blood circulation, resolving heat and dispersing toxin, and purging heat from the relevant zang organs based on symptoms. Si Shun Qing Liang Yin (Four-Obedience Clearing and Cooling Decoction) should commonly be used. Adjust the dosage according to the severity of the wind-heat toxin manifestation. If efficacy is achieved, do not rigidly adhere to fixed formulas; instead, treat according to the specific symptoms. Generally, children do not suffer from "qi eyes" (a specific type of strabismus or nerve-related eye issue distinct from adults); pediatric eye affections differ from those in adults. In severe cases where redness and swelling are prominent, purgative methods should be employed to rapidly resolve the pathogenic factors. If there is blood stasis causing severe swelling around the eyelids, removing the stale blood via the nasal cavity can lead to immediate recovery. Since "qi eyes" do not occur in children, pterygium (fleshy growth crossing the cornea) will not develop. Even if conjunctival vessels extend across the pupil, it is also caused by heat-toxin and should not be treated with hook-knife surgery. Whenever purgation is indicated, using Da Huang-based formulas combined with Xi Xin San (Heart-Washing Powder) is preferable.
