Differentiation of the three sections of the infant's index finger is performed in the same manner for both boys and girls. It is known that the first section (Feng Guan, Wind Gate) corresponds to the pulse condition of wind-stroke, while the last section (Ming Men, Life Gate) is difficult to diagnose as it involves the gate of life. To understand the "Qi" (vital energy) and "Jiao" (twisted/knotted) pulses at the tiger's mouth, one must observe with the finger inverted.
Regarding morphology and color: Yellow and red indicate harmonious five zang organs and balanced qi and blood; a faint red-purple hue suggests minor excess or deficiency; cyan indicates food stagnation or qi deficiency with irritability; cyan-black signifies adverse and difficult-to-treat conditions. If black appears in the middle, it requires the discernment of an expert physician.
Coldness of the heart and gallbladder manifests when the fingerprint extends straight up to the Feng Guan, divided into two segments like a grain of rice, shooting forward like a spear, indicating approaching convulsions (Jing Feng). There are various other branching patterns; facial expression reversed internally but smooth externally is considered adverse. If adverse patterns interconnect, the disease is difficult to treat; however, if the head segment retains proper length, there is still hope. Thus, physicians must examine carefully.
Boys aged two are still considered infants; those aged three to four are young children; by five to six they grow sequentially; seven to eight years old, they begin to show temperament; nine years old, referred to as "Huang Kou" (yellow-mouthed child); ten years old, called "Zhi Zi" (tender child). When children fall ill, open their clothing to differentiate the cause. At eleven or twelve, epilepsy is called Dian Feng; Gan disease (malnutrition syndrome) attacks similarly to consumption diseases; Pi Xi (abdominal masses) are determined as sedimentary accumulations. Deficient heat differs from tidal fever.
Initially, if the palm center feels hot, body heat follows. Abdominal heat with cold feet indicates food stagnation excess; foot heat with forehead heat indicates wind invasion; forehead coldness with foot heat indicates fright acquisition. When rashes erupt, redness behind the ears occurs. For infants with accumulation, appropriate resolving agents should be given. For the three types of Shang Han (cold damage), resolution is suitable. During dietary diarrhea, accumulation must be present; cold diarrhea requires Wenyi Tang (Warm the Spleen Decoction). Watery diarrhea requires Se Zang San (Astringe Intestines Powder), first regulating the zang-fu, then administering Se Zang San.
If a healthy child suddenly cries out loudly, it is not convulsions but Tian Diao Feng (Heaven-hanging wind). Crying accompanied by rapid breathing and coarse long sounds suggests misconsumption of hot toxins stifling the mind orifices; urgent emesis and purgation are needed to harmonize the spleen and stomach. Using convulsion medicines in such cases is laughable. In dysentery, frowning during straining indicates symptoms; lack of frowning suggests wind in the intestines due to cold-heat disharmony causing red-and-white stools. Tuo Du (prolapse/dysentery collapse) results from toxic heat attack. Among twelve types of dysentery, which are malignant? Jin Kou Li (forbidden-mouth dysentery) and Gua Chang Li (scraping-intestine dysentery) are vastly different.
Children's illnesses should not be treated with purgatives lightly. If there is spontaneous sweating and diarrhea without fever, with mental fatigue, sunken fontanelle, cold limbs, dry vomiting, qi deficiency with spirit timidity, fear of vomiting worms, pale face, scorched hair, it belongs to Gan Qi (Gan qi deficiency) with tidal fever, undigested food, nasal congestion, cough, deficient phlegm, fine pulse, intestinal borborygmus, and irritability. If accumulation is present and cleared via purgation, unexpected complications will arise.
For children with excess heat, purgation is permissible. Red face and eyes, strong qi, large wiry and surging pulse, abdominal heat, swollen cheeks, sore throat, urine like boiling water, hard stool, abdominal distension, full hypochondrium, edema of four limbs, night crying, widespread sores, and hidden abdominal pain will all be cured by purgation. This has been passed down through generations as a good method.
