Xue said: Chi (chì, contraction of sinews and vessels) refers to the tightness and shortening of sinews and vessels; Zheng (zhēng, relaxation of sinews and vessels) refers to the laxity and extension of sinews and vessels. One contracting and one extending, with hands and feet pulling each other, and continuous convulsions, similar to infantile convulsions, is called Chi-Zong (chì zòng). This syndrome mostly belongs to Wind, as Wind governs tremors and movements. Luo Longji said: The Heart governs the vessels, the Liver governs the sinews; the Heart belongs to Fire, the Liver belongs to Wood; Fire governs Heat, Wood governs Wind. When Wind and Fire blaze together, Chi-Zong occurs. In terms of treatment methods: if caused by Wind-Heat (this Wind originates from internal Heat, so the prescriptions also address the interior), use Gouteng Tang (Uncaria Decoction) added with Chaihu (Bupleuri Radix), Shanzhi (Gardeniae Fructus), Huangqin (Scutellariae Radix), and Baizhu (Atractylodis Macrocephalae Rhizoma) to calm the Liver Wood, descend Heart Fire, and nourish Qi and Blood; if Phlegm-Wind surges upward, add Zhuli (Bambusa Textilis Sap), Nanting (Arisaematis Cum Bile), and Banxia (Pinelliae Rhizoma); if acute convulsions are caused by Wind pathogen, add Quanxie (Scorpio) and Jiangcan (Bombyx Batryticatus); if there is a deficiency of Qi and Blood, use Bazhen Tang (Eight-Treasure Decoction) added with Gouteng (Uncariae Ramulus cum Uncis) and Shanzhi as the main formula. If there are weak convulsions, Daiyan (eyes fixed upward), Fanze (opisthotonos), and sweating like pearls, this indicates Liver exhaustion, and all such cases are incurable. A pregnant woman had limbs that could not extend; after taking formulas for dispelling Wind and drying Blood, she suffered from fecal incontinence and profuse phlegm, with limb convulsions. I diagnosed it as Liver Fire and dry Blood, treated her mainly with Bazhen Tang added with stir-fried black Huangqin, assisted by Gouteng Tang, and she recovered. Later, due to anger, the previous symptoms recurred, with hematuria, alternating chills and fever, insomnia, and poor appetite. She was treated with Gouteng San added with Shanzhi and Chaihu, which stopped the bleeding; then Jiawei Xiaoyao San was used, which resolved the chills and fever and allowed sleep; finally, Liujunzi Tang added with Shaoyao (Paeoniae Radix Alba) and Gouteng Gou (Uncariae Ramulus cum Uncis) improved her diet and led to gradual recovery.
Gouteng Tang is listed under the section on Fetal Restlessness.
Daquan states: A pregnant woman's inability to speak is not a disease. Occasionally, this happens; medication is unnecessary. In the month before delivery, simply administering Bao Sheng Wan (Life-Preserving Pill) or Siwu Tang (Four-Substance Decoction) allows speech to return after childbirth. This is natural law, not the merit of medicine. Physicians do not disclose this truth to patients. They administer ordinary medicines near term; when the patient can speak postpartum, they claim credit for their medical skill. How can this be considered their merit?
The Yellow Emperor asked: There are cases where a person, while pregnant, loses their voice in the ninth month. What is the reason for this? Qibo replied: The collateral vessels of the uterus are obstructed. The Emperor asked: Why do you say so? Qibo said: The uterine collaterals connect to the Kidney. The Foot-Shaoyin Kidney channel penetrates the Kidney and connects to the root of the tongue; therefore, speech is impaired. (This is also due to insufficiency of Kidney Qi). The Emperor asked: How should it be treated? Qibo said: No treatment is needed. It will recover naturally in the tenth month.
