(Ji Zitong - Sub-pain)
Zhang Zhongjing stated: When a pregnant woman experiences various types of abdominal pain, she should be treated with Danggui Shaoyao San (Angelica and Peony Powder). The *Mai Jing* (Pulse Classic) records: If a pregnant woman suffers from abdominal pain and feels restless, to determine the viability of the fetus if it appears inactive, one should palpate the abdomen. If it feels like an inverted cup (smooth and round), it is a male fetus; if it feels uneven, resembling the protrusion of elbows or necks, it is a female fetus. If the area is cold, it indicates a dead fetus (the term "He Mian" refers to the left and right sides of the abdomen); coldness signifies death, while warmth signifies life.
Xue Ji stated: If there is intermittent abdominal pain or a sensation of heaviness and bearing down in the lower abdomen, it is called Taipain (Fetal Pain). It should be treated with Dihuang Danggui Tang (Rehmannia and Angelica Decoction). If there is no improvement, add Renshen (Ginseng), Baizhu (Atractylodes Macrocephala Rhizome), and Chenpi (Tangerine Peel) (suitable for Qi deficiency); if caused by Spleen Qi deficiency, use Sijunzi Tang (Four Gentlemen Decoction) added with Danggui and Shudihuang; if caused by Middle Qi deficiency, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
Dihuang Danggui Tang treats Taipain during pregnancy. (Excellent for pain due to Blood deficiency.)
Danggui (1 liang) Shudihuang (2 liang)
Grind the above into powder. Take as a single dose, decoct with 3 sheng of water until 1 sheng remains, strain, and take in one go. If ineffective, add Renshen, Baizhu, and Chenpi.
Jiawei Siwu Tang treats Taipain due to insufficient Blood.
Danggui Chuanxiong Baishyaoh Shudihuang Xiangfuzi (each in equal proportions)
Grind the above into powder. Take 3 qian per dose, mixed with Zisu Tang (Perilla Leaf Decoction). (The efficacy lies in using Zisu Tang.)
*Daquan* states: Lower abdominal pain during pregnancy is caused by deficiency of the Bao Luo (Uterine Collaterals), where Wind-Cold pathogens clash. Severe pain may also cause fetal movement.
Xue Ji stated: For the aforementioned conditions, if caused by Wind-Cold clashing, use Zisu Yin (Perilla Infusion) added with Shengjiang (Fresh Ginger); if due to Qi and Blood deficiency, use Bazhen Tang (Eight Treasures Decoction); if due to Spleen Qi deficiency, use Liujunzi Tang (Six Gentlemen Decoction); if due to Middle Qi deficiency, use Buzhong Yiqi Tang; if accompanied by abdominal distension and pain, use Antai Yin (Fetus-Calming Infusion) added with Shengma (Cimicifuga Rhizome) and Baizhu. If ineffective, combine with Buzhong Yiqi Tang.
A pregnant woman suffered lower abdominal pain with unstable fetus, Qi attacking the left and right sides, occasional rebellious Qi rising upward, and difficulty urinating. She was treated with Xiaochaihu Tang (Minor Bupleurum Decoction) added with Qingpi (Green Tangerine Peel) and Shanzhi (Gardenia Fruit) to clear Liver Fire, resulting in recovery. Later, due to anger, she experienced lower abdominal distension and fullness, difficulty urinating, a heavy dragging sensation in the urinary tract, and continued fetal instability. This was also caused by excessive Liver Wood Fire. One dose of Longdan Xiegan Tang (Gentiana Decoction to Drain the Liver) cured all symptoms immediately. Subsequently, Sijunzi Tang added with Chaihu (Bupleurum Root) and Shengma was used to nourish Spleen Earth and calm the fetus.
