In diseases of the throat and pharynx, one must know when to use slow or rapid treatments. In acupuncture, it is crucial to recognize the onset and progression of the disease. If there is trismus (lockjaw), Tongguan San can be applied. For wind-toxin and phlegm obstruction, Zhui Feng San should be used. San Huang Liang Ge San has the effect of resolving phlegm and descending fire. Er Chen Tang with Jing Jie has the function of clearing phlegm and expelling wind. Ulceration requires internal support (Nei Tuo), while recovery relies on flesh-regenerating agents. Anesthetics are used before needling, and astringent secret medicines are used after needling. Bandaging agents reduce redness and swelling; water decoctions promote the movement of stagnant phlegm. Washing agents remove old tissue and generate new ones; fumigation eliminates pathogens and preserves healthy Qi. Blowing agents are applied during pain, and knives/needles are used on swollen areas. Bilateral or unilateral tonsillitis (Ru E) arises at the entrance of the pharynx; if round and small without pus, apply the proprietary blowing agent and needle. If pus is present, puncture it for self-healing, and administer Liang Ge Tang. Throat carbuncle (Hou Ding) and flower carbuncle resemble boot-shaped carbuncles but are slightly longer. Apply anesthetic, hook the lesion, cut with a knife, cauterize with an iron brand, and blow in secret analgesic powder. Internal support leads to spontaneous resolution; red lesions are treatable, while black ones are difficult to cure. For bilateral or unilateral abscesses, swelling may occur below the ears and on the cheeks, treated similarly to tonsillitis; after ulceration, take internal support formulas. Que She (Sparrow Tongue) abscesses are small and pointed on both sides, with phlegm under the tongue in the throat; treatment is similar to throat carbuncle, using only San Huang San. Dian She and Lian Hua swellings arise from the root of the tongue and cannot be needled. Chan She Hou Feng involves lockjaw, curled tongue, and swelling; San Huang Liang Ge San must not be delayed, or recovery will be difficult. For bilateral or unilateral tonsillitis, regardless of vitality, if enlarged over time and painful at any moment, carefully cut away layers with a knife, apply corrosive medicine inside, ensure it is spat out and not swallowed, blow in secret medicine for a month to see results. Cauterization can be used, and San Huang San may be taken. Regarding Zou Ma Ya Gan (Gangrenous Gingivitis), Hou Gan (Throat Gangrene), Kou Gan (Oral Gangrene), and Jie Du (Condyloma/Lues), with purple-swollen gums and unbearable foul odor, the proprietary blowing agent must be used, along with flesh-regenerating medicine. Rinse mouth with aged water in the afternoon. Facial swelling, head shaking, dry throat, hoarseness, body heat, lip fissures, and tooth loss without bleeding are all incurable. Taking Tu Fu Ling powder yields miraculous effects. Hou Dan resembles tonsillitis but is sharper and smaller, like cow's milk. Qi Dan resembles a plum pit but is smaller; use its forty-nine needles. Hui Shi Dan is also known as Dian Qi, arising beside Di Ding, red and raised in the center, named Mei He (Plum Pit); unable to spit out or swallow. Mei He requires needling; Dian Qi requires San Huang San, combined with Er Chen Tang and Si Qi Tang for success. Chong She (Heavy Tongue) arises on the tongue; Jin Jin and Yu Ye acupoints can be pricked. After Di Ding swells, it is called Xuan Ding; blow secret medicine without needling, take San Huang San and Xi Di Tang. Si She (Dead Tongue) is called Mu She (Wood Tongue), hard and unable to extend. Za She Yong arises in the throat, causing shortness of the tongue and difficulty speaking. With phlegm, first use Zhui Feng San; the abscess head must be scraped open. Initially take San Huang San, later use internal support. White, red, or purple colors are still treatable; black swelling indicates impending death. Dou Sai Yong arises below the cheek; externally apply Jin Gu San, internally take Shi Ba Fang. Pus discharging from the mouth is easy to treat; from the cheek is difficult. Fire needle for pus formation; secret flesh-regenerating medicine for rotting flesh. Qin Hei should be picked off, followed by San Huang Liang Ge San, intermittently blown with the proprietary base, and finally using flesh-regenerating medicine for completion. Fe Yang appears instantly; Hou Bi resolves quickly; this requires inducing vomiting first. What harm is there in blowing agents and knife/needle therapy? Suo Hou Feng presents with lockjaw and splayed limbs; first prick four acupoints to distinguish life from death, then bathe hands and feet to open the spleen and stomach. Knife or needle therapy to remove blood and reduce swelling is the best strategy. Chan Hou Feng presents with white sclera, purple face, neck swelling, and mutism, resembling opisthotonos; life hangs in the balance. Probe phlegm and prick blood to remove the root of the disease. Hou Qiu feels like connected cotton; take Qi Yi Shu Feng formula, mixed with Moschus (She Xiang) for administration, and blow Zhui Feng proprietary medicine. Gu Cao Feng with lockjaw requires expelling wind-phlegm first; moxa seven moxibustion grains five fen below; Qing Yang San Huo therapy cures it. She Xia Tan Yong requires pricking blue veins, releasing gelatinous saliva like egg white; add Er Chen Tang and Re Qing Ru Sheng San, both effective. Meat obstructing the throat with overlapping congestion: Ju Ju Tang and Xiong Huang powder, drunk and applied, are best. Sweating causes carbuncles; black swelling causes carbuncles; yin sores on left and right are identical; Jin Gu San and Shi Ba Fang, applied and taken appropriately. Qi Yong Hou Bi and Jiu Du Hou Bi are treated similarly; Jin Suo Chi and San Huang San, blown and drunk for immediate cure. Generally, blowing agents are administered downwards; after knife/needle and secret blowing, if residual swelling remains, use Jun Mo; for difficult-to-heal knife wounds, apply flesh-regenerating medicine. For bad breath in the throat, use Ren Zhong Bai; to chase wind-phlegm, use Jin Suo Chi. Water decoctions are held in the mouth; Bing Mei is frequently swallowed to generate saliva. To suppress fire, use San Huang Liang Ge San; to reduce swelling, know Shi Ba Wei. Nei Tuo Qian Jin San supports internally; Er Chen Tang resolves phlegm. For deficiency-fire and blood deficiency, follow Si Wu Tang; for yang-deficiency and qi-weakness, use Si Jun Tang. For aversion to cold, resolve the exterior; for constipation, unblock the bowels. These are the essential secrets of throat medicine; students must devote themselves fully.
