Ru Yan (Breast Rock / Breast Cancer)
Chen Yuan Gong said: There are cases where a patient suffers from Ru Yong (Acute Mastitis), which has already healed, but due to imprudence in sexual activity, it relapses and ulcerates, transforming into Ru Yan (Breast Cancer). Countless small sores appear, resembling tubes, holes, or a honeycomb structure, with flesh growing outward. It persists for years without healing. Taking detoxifying agents makes the body more debilitated and the sores more rotten. People assume the toxin is deeply knotted in the breast, but who knows that it is actually a severe deficiency of Qi and Blood? Generally, the flesh inside the breast grows outward while tendons bind to the nipple; thus, injury to the breast is an injury to the tendons. When an abscess occurs here, it should be dispersed urgently; delay risks tendon laxity and failure to heal. Moreover, seminal loss damages Yuan Qi (Primal Qi), how could it not transform into such an unusual condition! After seminal loss, one should immediately use large doses of medicines to tonify essence and fill marrow, avoiding such severity. Now that Yin deficiency has formed Ru Yan, and detoxifying agents are used because of Ru Yan, is this not further damaging the deficiency? The treatment method must heavily tonify Qi and Blood to generate Essence, without attempting to drain toxins, as there is no toxin to drain. Use Huayan Tang (Transform Rock Decoction): Renshen (Ginseng Radix), Huangqi (Astragali Radix), Rendongteng (Lonicerae Japonicae Caulis), Danggui (Angelicae Sinensis Radix) each one liang; Baizhu (Atractylodis Macrocephalae Rhizoma) two liang; Qiancao (Rubiae Radix), Baijiezi (Sinapis Semen) each two qian; Fuling (Poria) three qian. Decoct with water and take orally. After two doses, flesh grows; after another two doses, pus ceases and pain stops; after another two doses, fistulas regenerate; after another two doses, complete recovery; after another two doses, no recurrence. This prescription fully tonifies Qi and Blood and does not remove toxic stasis. Although Rendongteng is a detoxifying herb, its nature is also tonifying, especially when combined with other tonics, making it purely tonifying. Why only tonify Qi and Blood when seminal loss caused Ru Yan, seemingly requiring essence tonification? Essence cannot be generated quickly, and its effect is slow. Tonifying Qi and Blood is easier to generate Essence. Furthermore, the breast belongs to the Yangming Channel. Since Ru Yan has occurred, the Yangming Channel must lack abundant Qi and Blood. Tonifying Qi and Blood strengthens the Yangming Channel, which naturally generates fluids to irrigate the breast. Why use essence-tonifying herbs to constrain the efficacy of Renshen and Huangqi? Hence, no essence-filling herbs are used. (From *Bingjian*)
Chen Shigong said: Ru Yan (Breast Cancer) results from melancholy injuring the Liver, overthinking injuring the Spleen, persistent thoughts in the heart, and unfulfilled desires, leading to obstruction and stagnation of meridians, accumulating into nodules. Initially like a bean, gradually like a chess piece, over half a year to one year, or three to five years, it is painless and itchless, gradually enlarging, then starting to hurt. Once painful, it cannot be resolved. Later, swelling resembles piled chestnuts or an inverted bowl, purple with foul odor, gradually ulcerating. Deep lesions resemble rock caves, protruding ones resemble floating lotuses. Pain radiates to the heart, bleeding emits a foul smell. At this stage, the Five Zang Organs are all衰竭 (declining), and the Four Da (limbs/body elements) cannot be saved. It is named Ru Yan. Those afflicted face certain death; if they can maintain a calm mind and rest quietly without attachment, forcing treatment is unnecessary, and life may be prolonged. The primary treatment is Yiqi Yangrong Tang (Qi-Nourishing and Blood-Sustaining Decoction). (From *Zhengzong*)
He also said: Women past middle age who are widowed are more likely to die from this disease.
He also said: When men suffer from this, it is called Ru Jie (Breast Knot), slightly different from women's cases. In women, the Liver and Stomach are injured; in men, the Liver and Kidney are damaged. This is generally due to excessive anger and sexual desire, leading to Liver deficiency and Blood dryness, Kidney deficiency and Essence weakness. Qi and pulses cannot ascend, the Liver Channel lacks nourishment, resulting in knotted swelling and pain.
He also said: Treatment should use Bazhen Tang (Eight-Treasure Decoction) plus Zhizi (Gardeniae Fructus), Danpi (Moutan Cortex); for dry mouth and thirst, use Jiawei Bawei Wan (Modified Eight-Ingredient Pill) or Shenqi Wan (Kidney Qi Pill); if already ulcerated, use Shiquan Dabu Tang (Complete Great Tonifying Decoction) to facilitate flesh regeneration and wound closure.
Wang Shengzhi said: Ru Yan in patients under forty is treatable; those under fifty are untreatable. Treating them leads to death; not treating them allows them to live out their natural lifespan. (From *Lili*)
Feng Luzhan said: Women suffering from worry, anger, and depression accumulate day and night, obstructing Spleen Qi, causing Liver Qi to rebel horizontally. Qi and Blood become deficient, tendons lose nourishment, stagnation combines with Phlegm to form hidden nodules, redness-free and painless. Accumulating and gradually developing over years, they enlarge, internally ulcerating and rotting deeply. Named Ru Yan because the sore shape resembles a rock cave. Do not treat it rashly. This is a syndrome of Liver Channel Qi and Blood exhaustion caused by the Seven Emotions. Treatment method: If chills and fever occur at the onset, release the exterior and scatter pathogens. While soothing the Liver, simultaneously tonify Qi and Blood with prescriptions like Yiqi Yangrong Tang or Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder). Use wind herbs to follow its nature, Qi-moving herbs to resolve stagnation, Renshen, Huangqi, Danggui, Baishao to tonify Qi and Blood, Wuyao and Mutong to unblock accumulation and relieve congestion, Chaihu, Fangfeng, Ziye to release the exterior, Baizhu to expel pus, Tongcao to harmonize Ying and Wei, Rougui to move Blood and harmonize vessels. Mild cases may self-heal with prolonged use; severe cases may only have their course prolonged. Using cooling, Qi-moving, and blood-breaking methods accelerates death. (From *Jinnang*)
Dou Hanqing said: Married or unmarried women may develop this condition, resulting from extreme Yin and declining Yang. Deficient Yang accumulates with Blood. Without Yang, how can it disperse? Thus, this blood penetrates into the Heart Channel, causing this disease. If not yet ulcerated, it is treatable; once ulcerated, it is difficult to treat.
Hu Gongbi said: Ru Yan often stems from suspicious and jealous temperaments, frustration with parents-in-law, or dissatisfaction with husbands, due to poor regulation, brewing anger, accumulated depression, and rich diet. This causes Jueyin Qi to fail to circulate and Yangming Blood to surge turbidly. Orifices remain blocked, forming hard nodules like chess pieces. Some do not manifest for five to seven years, or even ten-plus years. They may erupt due to seasonal changes or triggered by great anger. Once erupted, they ulcerate like pomegranate flowers, named Ru Li (Breast Chestnut). Patients within thirty years old with vigorous Qi and Blood are treatable; those over forty with declining Qi and Blood are difficult to treat. (From *Qingnang*)
