The *Yi Lun Xuan Yao* (Selected Essential Medical Discussions) states: Chan Hou Feng (Choked Throat Wind) refers to heat pathogen accumulating in the throat, with swelling surrounding the exterior, accompanied by numbness and itching, and significant enlargement. The treatment method for mild cases is to use salty substances to soften and dissipate; for severe cases, pungent substances are used to disperse, or to eliminate wind-phlegm, or to resolve heat-toxin. If the swelling is so severe that medicine cannot be swallowed, administer medicine into the nose to induce vomiting. Externally apply detoxifying agents; if fever subsides and swelling disappears, it is a favorable sign.
Dou Hanqing stated: Chan Hou Feng belongs to phlegm-heat syndrome, characterized by swelling both inside and outside the throat. If there is no external swelling, the body must be feverish and the face red, indicating extreme excess of heat-toxin qi. If there is external swelling, the body also has fever, which is the origin of evil fire manifesting outwardly. In cases where the jaw is not stiff, there is no external swelling, but the interior of the throat is red, it is called "Bao Gan" (Sudden Contraction), with the pathology in the Heart Meridian. If the condition on the left recedes and transmits to the right, this is due to residual toxin not yet cleared. There are several syndromes of the throat: accumulated heat, wind-heat, guest-heat, and residual toxin after illness that transforms into Shuang Ru E (Double Tonsillar Swelling). For instance, if during an illness there is redness and swelling in the throat lasting months, with a luster like a mirror, this is accumulated heat. If there is swelling in the throat, slightly white in color, shaped like an arm, this is Feng Du Hou Bi (Wind-Toxic Throat Obstruction). This occurs because heat-toxin combines with wind pathogen to erupt. Or if there is swelling in the pharynx with a purplish hue, this is guest-heat, meaning the person suddenly contracted heat-toxin qi, obstructing the throat. It is essential to use Tong Cao (Akebia Caulis), Xuan Shen (Scrophulariae Radix), Sheng Di Huang (Rehmanniae Radix), Huang Qin (Scutellariae Radix), Huang Lian (Coptidis Rhizoma), and Shan Zhi Ren (Gardeniae Semen) to drain fire from the Heart Meridian. Some may progress to Mu She (Wooden Tongue), all caused by heart-heat accumulating in the chest, resulting in foul-smelling phlegm in the mouth. Internal medication should prioritize Liang Ge San (Cool the Diaphragm Powder), assisted externally by Bing Pian San (Borneol Powder), or puncture with a small knife to release purple blood. (From *Quan Shu*)
It further states: The exterior signs of Chan Hou Feng resemble a snake coiling around the neck, with tidal fever, severe headache and eye pain. The swelling appears purple-brown. The interior signs show red lines and white swelling, blocking the view of swallowing. Add Xiong Huang (Realgar), Bing Pian (Borneolum Syntheticum), and She Xiang (Moschus) to the insufflated powder. Administer Jing Fang Nian Zi Er Chen Tang (Schizonepeta, Saposhnikovia, Perilla Seed, and Two-Aged Phlegm Decoction). Urgently dip a goose feather in the turbid oil from the lamp wick holder, stir to remove three to four bowls of phlegm and saliva for survival; if phlegm cannot be removed, recovery is difficult. Avoid wind.
It further states: Ya Zhang Hou Feng (Mute Miasma Throat Wind) occurs when wind-phlegm invades between the pharynx and diaphragm, causing inability to speak and locked jaw. Urgently grind Chan Su (Toad Venom) with water and drip into the nostrils to open the mouth. Immediately drip Tung You (Tung Oil) into the throat, then use a goose feather to stir the throat until all wind-phlegm is expelled. Then use Gan Cao Tang (Licorice Decoction) to neutralize the smell of tung oil, apply insufflated powder, and take Jing Fang Bai Du San (Schizonepeta, Saposhnikovia, and Detoxification Powder). If the face is purple, tongue cyanotic, lips black, nose runs cold mucus, nails are all blue, and eyes tear excessively, these are untreatable.
It further states: Nong She Hou Feng (Playing Tongue Throat Wind) presents as muteness, with the tongue protruding and being handled by hand. Urgently use a Three-Edged Needle to prick the nail beds on the sides of both thumbs, three times per finger. If blood flows, it is treatable; if not, it is untreatable. Pricking Shao Shang (LU 11) is also excellent. Use a copper spoon to pry open the mouth, insufflate Dan Xiao Dan (Calomel and Borneol Powder) into the throat. Use the oil residue from the lamp wick holder with a goose feather to stir out phlegm and saliva. Still administer seven pills of Xiong Huang Hua Du Wan (Realgar Detoxification Pill), taken with clear tea. Subsequently take Shu Feng Gan Jie Tang (Wind-Dispersing Licorice and Platycodon Decoction) and frequently insufflate Bing Pian San (Borneol Powder).
Dou Menglin stated: Chan Hou Feng arises because heat in the Kidney Meridian causes internal dryness unable to moisten upwards, leading to hyperactive Heart fire, thus triggering this syndrome. If thunderous sounds are heard in the throat, it is untreatable; careful observation is crucial.
It further states: Qiang Shi Hou Feng (Choking Food Throat Wind) occurs because heat-toxin is in the Heart Meridian, causing dryness in the throat without phlegm. If caused by choking on food, it is untreatable; if left untreated for a long time, it transforms into Fei Si Lao Du (Flying Silk Labor Toxin), which can be fatal.
It further states: Gen Jiao Hou Feng (Heel Throat Wind) originates from the heel and spreads to the throat, occurring once a year or every half year. Strictly avoid hot foods and emotional anger. The disease travels one acupoint per day, reaching seven acupoints in seven days. Although not immediately life-threatening, it will flare up. Use throat insufflation powders and still take Bai Du Yao (Detoxification Medicine). If the swelling becomes foul and blisters, death ensues.
A man suffered from Chan Hou Feng with swelling affecting both exterior and interior, making oral medication impossible. I administered Shu Feng Qing Re Yao (Wind-Dispersing Heat-Clearing Medicine) via gavage more than sixty times. Externally, I mixed Yang Qi Shi (Stibnite) calcined and powdered with Fu Long Gan (Dragon's Liver/Colored Earth) in equal parts, blended with fresh drawn water, and applied/swabbed the area one hundred times. After three days, the fever subsided and swelling decreased.
Hu Jingzhou stated: If after inducing vomiting with tung oil or expelling phlegm via insufflation, the swelling does not subside or the patient loses consciousness, urgently needle the affected area to evacuate bad blood, and the patient will revive.
Kui Guang stated: Chan Hou Feng erupts due to mental agitation. Two days prior, there must be tightness in the chest and diaphragm, shortness of breath, followed by sudden throat swelling and pain, cold limbs, neck stiffness as if twisted, heat knotting internally, swelling disturbing externally, numbness and itching, red filaments winding in the throat, white fingernails, intense heat in palms, large throat swelling, abundant wind-phlegm obstruction, and a voice like sawing wood. These are its signs, representing a critical emergency. In the first day and night of onset, if the eyes stare fixedly and the throat sounds like thunder, it is untreatable; if a lamp flame near the patient's mouth is blown out, it is untreatable; if panting is urgent and sweat appears on the forehead, danger is imminent.
It further states: Chan Hou Feng is heat knotting in the throat, swelling surrounding the exterior, with numbness and itching.
It further states: "Chan" (Entangle) refers to winding from the mandible upward; redness indicates alternating chills and fever.
It further states: For early-stage Chan Hou Feng, insufflate Jin Dan (Golden Pill) and Bi Dan (Green Pill) in equal parts frequently. Internally take decoction with added Niuhuang (Calculus Bovis). (Formula recorded in the Throat Gate section.)
