Tie Gu San (Iron Hoop Powder) (Wu Danyuan): Applicable to yang-type sores and ulcers where the swelling is diffuse and the base is loose.
Ingredients: Fermented Bean Curd Shell (Wubeizi, 50g, lightly stir-fried), Raw Rhubarb (Sheng Dahuang, 12g), Autumn Hibiscus Leaf (Qiu Furong Ye, 18g).
Preparation and Usage: Take one cup of vinegar, boil in a ladle until boiling, add the powdered herbs, stir well, and apply to the affected area, leaving the apex open for ventilation. Cover with paper. If it dries, moisten with vinegar. Another version adds Cold Food Noodles (Hanshi Mian, 15g) to the original formula.
Contraindications: Do not use for yin-type abscesses or cases where the skin color remains unchanged, with diffuse swelling and no head.
Ruyi Jinhuang San (Ruyi Golden Yellow Powder) (from *Waike Zhengzong*): Applicable to carbuncles on the back, various types of carbuncles and swellings, traumatic injuries, phlegm-dampness flowing accumulation, epidemic swelling of the head, lacquer dermatitis, fire blisters, wind-heat blistering disease, red and swollen skin, damp and dry tinea pedis, breast abscess in women, and erysipelas in children. It is effective for all kinds of stubborn and malignant external sores upon immediate application, truly an excellent and convenient prescription for treating sores.
Ingredients: Trichosanthes Root (Tianhuafen, premium white variety, 5kg), Phellodendron Bark (Huangbai, dark colored), Rhubarb (Dahuang), Turmeric (Jianghuang), Angelica Dahurica (Baizhi, each 2.5kg), Tangerine Peel (Chenpi), Pinellia Tuber (Tiannanxing), Atractylodes Rhizome (Cangzhu), Purple Magnolia Bark (Zi Nongpu), Licorice Root (Gancao, each 1kg).
Preparation: Grind into coarse slices, sun-dry until extremely dry, grind three times using a large donkey mill, sieve through fine silk gauze, store in porcelain containers, and seal tightly to prevent loss of volatile components.
Usage: For cases with redness, swelling, heat, pain, and fever before suppuration, as well as during the summer fire season, mix with tea soup and honey for topical application. For mild heat and swelling, or large sores that have formed and are about to suppurate, mix with scallion juice and honey. For diffuse swelling without a head, unchanged skin color, phlegm-dampness toxic flow, deep-seated bone abscesses, and knee deformity resembling a crane's leg, mix with scallion wine. For conditions caused by wind-heat evil toxins, characterized by intense skin heat, bright red luster, and migratory shape, mix with honey water. For blistering diseases like fire blisters, wandering erysipelas, yellow fluid sores, lacquer dermatitis, and blood toxicity attacks, crush Isatis Root Leaves (Da Lan Gen Ye) or Water Banana Root Leaves (Shui Bajiao Gen Ye) to extract juice for mixing; honey may be added. For burns and scalds causing broken skin, mix with sesame oil. The rationale behind these different vehicles is to regulate cold and heat, warmth and coolness according to the pathogen. The key lies in adapting to the seasons, understanding the disease progression, and ensuring the correct guiding agents are used.
Si Hu San (Four Tigers Powder) (from *Waike Zhengzong*)
Ingredients: Pinellia Tuber (Tiannanxing), Aconite Root (Caowu), Pinellia Tuber (Banxia, raw), Wild Swallowwort Root (Yelangdu, equal parts).
Preparation and Usage: Grind into fine powder, mash together with pig brain, and cover evenly over the sore, leaving the center open for ventilation. Suitable for carbuncles and sores that are hard and swollen, with thick pus-like consistency resembling ox neck skin, failing to suppurate and rot.
Tie Tong Gao (Iron Bucket Ointment) (from *Waike Zhengzong*): Applicable when a back carbuncle is about to rupture or has ruptured, and the base is spreading and cannot be contained.
Ingredients: Fermented Bean Curd Shell (Wubeizi, lightly stir-fried, 50g), Alum (Mingfan, 12g), Blue Vitriol (Danfan, 9g), Verdigris (Tonglv), Bletilla Rhizome (Baiji, each 25g), Calomel (Qingfen), Turmeric Tuber (Yujin, each 6g), Musk (Shexiang, 1g).
Preparation and Usage: Grind into extremely fine powder. Boil one bowl of aged rice vinegar slowly in a ladle until reduced to one small cup, stopping when golden-yellow bubbles appear. When warm, mix in one qian (3g) of the powder into the paste. Warm slightly before use, apply with a new brush around the base of the sore, cover with cotton paper. The goal is to induce wrinkling, gradual contraction, and reduction in size without expansion. Herbs must be ground very finely; coarse particles cause pain.
Efficacy: Effective for any spreading base that cannot be contained, helping to gather the root and confine the toxin.
Note: For treating nuchal carbuncles, do not wash off old medicine and add new daily, as this may disturb the wound and invite wind pathogens.
Furong Gao (Hibiscus Ointment): Applicable to red and swollen yang sores, capable of gathering the root and confining the toxin.
Ingredients: Hibiscus Leaf (Furong Ye, harvested in autumn, 18g), Elm Flour (Yumian, 60g), Raw Rhubarb (Sheng Dahuang, 15g), Mirabilite (Pinxiao, 30g).
Preparation and Usage: Grind fine, mix with scallion juice and child's urine for topical application, leaving the apex open. Applied at the onset, it can resolve the condition, not just containing the base.
Two Methods for Applying Medication to Major Toxins:
1. Apply thick sludge from a latrine around the periphery, leaving the center hollow for ventilation; it will dissipate spontaneously.
2. Or grind Adzuki Beans (Chixiaodou) and Perilla (Zisu) together and apply; it dissipates immediately.
Method for Reducing Swelling and Detoxifying All Major Toxins: Grind Fritillaria Bulb (Chuan Beimu) into powder, mix with White Betel Nut Juice (Bai Binglang Zhi) and apply. Blisters will form on diseased areas, while red spots like sores will appear on healthy areas.
Yuanzhi Gao (Polygala Ointment) (Cheng Shanling):
For all initial carbuncles, sores, and swellings, take 60-90g of Polygala Meat (Yuanzhi Rou), remove the heart, boil in clear oil, mash into a paste, and apply to the affected area. Cover with oiled paper and cloth bandage. After one night, the toxin dissipates spontaneously. This method is frequently tested and proven effective.
Furong Juhua Gao (Hibiscus and Chrysanthemum Ointment)

Another Formula
Ingredients: Fresh Crucian Carp (Xian Jiyu, one tail), Yam (Shanyao, one segment), Alum (Mingfan, 3g).
Preparation and Usage: Mash and apply, leaving the head exposed. Itching occurs briefly, then subsides spontaneously. (Another version omits Alum).
Another Formula
Ingredients: Frankincense (Ruxiang), Myrrh (Moyao), Musk (Shexiang, each 0.9g), Large House Centipede (Dayouyan, two pieces).
Preparation and Usage: Mash and apply, leaving the head exposed; it dissipates spontaneously.
Another Formula
Ingredients: Angelica Sinensis (Danggui), Phellodendron Bark (Huangbo Pi), Notopterygium Rhizome (Qianghuo).
Preparation and Usage: Grind fine. Mix with fresh Honeysuckle Juice (Sheng Jinyinhua Zhi) and apply around the periphery, leaving the center empty.
Another Formula
Ingredients: Raw Rhubarb (Sheng Dahuang, 30g), Bletilla Rhizome (Baiji, 9g), Pangolin Scales (Chuanshanjia, 9g), Musk (Shexiang, 0.3g).
Preparation and Usage: Mix with vinegar and apply, leaving the apex exposed; or apply with yellow honey or scallion juice for better effect. This formula provides immediate pain relief upon application.
Another Formula (from *Waike Zhengzong*)
Ingredients: Plantain Herb (Cheqian Cao, with roots and leaves), [Missing Character], Five Dragon Grass (Wulongcao), Honeysuckle Flower (Jinyinhua, equal parts).
Preparation and Usage: Mash four fresh herbs together, add three-year-old rice flour (commonly used for sizing). For early stages, add 0.6-0.9g of sublimed salt powder. Form a thick paste, apply evenly over the sore, leaving one apex open, cover with plaster, avoid wind, and pus/toxin will naturally drain. In winter, if fresh herbs are unavailable, collect and air-dry beforehand, grind to powder, and mix with aged rice vinegar; equally effective. Since Five Dragon Grass may be scarce, one may fumigate the apex, cover the surrounding swelling with plaster, and substitute with Ruyi Jinhuang San.
Another Formula
Ingredients: Buckwheat Flour (Qiaomai Mian), Sulfur Powder (Liu Huang Mo, each 60g).
Preparation and Usage: Grind into powder. Mix with well water to form cakes, sun-dry. Use one cake per application, grind with water, and apply. It stops pain in painful areas and prevents pain in non-painful areas.
Another Formula: Hold a live loach firmly by head and tail, place it on the base of a red carbuncle/abscess. For larger sores, surround with several loaches; they will suck and draw out the toxic qi.
Another Formula
Ingredients: Costus Root (Muxiang), Frankincense (Ruxiang, de-oiled), Elm Bark Flour (Yupi Mian, equal parts).
Preparation and Usage: Simmer with vinegar until thick, then apply.
Zhenjun Miaotie San (True Lord's Wonderful Paste) (from *Waike Zhengzong*): Treats carbuncles, sores, and malignant ulcers of abnormal shapes that are stubborn, hard, and fail to suppurate. Use this medication; it induces pain in painless areas and stops severe pain.
Ingredients: Buckwheat Flour (Qiaomai Mian), Wheat Flour (Baimian, each 2.5kg), Pure Sulfur (Mingjing Liu Huang, 5kg, finely ground).
Preparation and Usage: Mix lightly with clear water to achieve appropriate moisture, dry into sheets in a wooden box, wrap in single paper, and air-dry in the wind. Before use, grind extremely fine and mix with fresh drawn water for application. For broken skin with bleeding, wet rot, and pain, mix with sesame oil. For blistering diseases, lung wind, acne, and dye stains, mix with indigo dye liquid; all are effective.
Huiyang Yulong Gao (Restore Yang Jade Dragon Ointment) (from *Waike Zhengzong*): Treats yin-type back carbuncles that are not elevated, painless, non-febrile, and non-suppurative; cold-dampness flowing accumulation; chronic damage from wind; cold pain and bi-syndrome; damp tinea pedis; numbness in hands and feet; bone and joint pain; and all cases with unchanged skin color, diffuse swelling without a head, or crane-leg deformity, provided there is no red skin or hot muscle. It is an excellent external method for rescuing yin-abscesses.
Ingredients: Aconite Root (Caowu, stir-fried), Ginger (Junjiang, baked, each 90g), Red Peony Root (Chishao, stir-fried), Pinellia Tuber (Tiannanxing, baked), Angelica Dahurica (Baizhi, each 30g), Cinnamon Twig (Rougui, 15g).
Preparation and Usage: Process and grind into fine powder. Mix with hot wine for application. Junjiang and Rougui are hot and generate blood; however, their heat alone may not disperse pathology. Caowu and Tiannanxing break evil qi, dispel wind-toxicity, revive dead tissue, relieve bone pain, dissolve masses, and restore yang qi. Chishao and Baizhi disperse stagnant blood and relieve pain. Wine promotes circulation. Even in severe cold syndromes, it is effective, likened to flames in cold ashes or spring in dead wood. Cold diseases cause muscle decay and insensitivity; pain often indicates deep bone involvement. Without removing the cold root, ordinary medicines fail.
Chonghe Gao (Harmony Ointment) (from *Waike Zhengzong*): Treats back carbuncles due to yin-yang disharmony and unclear cold-heat patterns.
Ingredients: Bauhinia Bark (Zijing Pi, i.e., Hongneixiao, stir-fried, 150g), Angelica Pubescens Root (Duhuo, stir-fried, 90g), Red Peony Root (Chishaoyao, stir-fried, 60g), Angelica Dahurica (Baizhi, 30g), Acorus Rhizome (Shichangpu, i.e., Wangjianxiao, also known as Yangchunxue or Mulawax, 45g).
Preparation and Usage: Grind into fine powder. Mix with scallion decoction or hot wine for application. Zijing Pi is the essence of Wood, breaking qi and blood stasis to reduce swelling. Duhuo is the essence of Earth, moving stagnant blood, relieving cold bone pain, and removing dampness. Shichangpu is the essence of Water, breaking hardness, generating blood, relieving pain, and reducing wind-swelling. Baizhi is the essence of Metal, dispelling wind, generating flesh, and stopping pain. Chishaoyao is the essence of Fire, generating and activating blood, resolving stasis, and relieving pain. Blood generation prevents tissue death; blood movement ensures meridian flow, preventing ulceration and congestion. This is a fine medicine for dispersing wind, moving qi, activating blood, reducing swelling, and softening hardness. The five elements are balanced, ensuring efficacy, especially for cold syndromes like flowing bone abscesses. Thus: "Wind disperses, blood scatters; Qi flows, hardness removes." Adding Frankincense and Myrrh treats flowing accumulation with restricted tendon extension.
Shenying Dan (Divine Response Pill) (from *Yizong Shuoyue*): Treats sores caused by qi-blood disharmony and obstruction, presenting with high swelling, redness, and pain, accompanied by phlegm, stagnation, cold, and dampness.
