The whole body feels hot and scorching to the touch. The patient prefers sleeping on the right side and cannot lie on the left; lying on the left causes rapid breathing and restlessness. The expectorated sputum resembles purulent blood with a foul, fishy odor. When tested with water, it sinks rather than floats. The pulse is rapid and forceful. This condition is named Fei Yong (Lung Abscess). It arises from labor-induced injury to Qi and Blood, causing the Cou Li (skin and pores) to open and become susceptible to pathogenic factors. Cold pathogens contend with Blood, accumulating to form an abscess. Accumulated heat fails to dissipate, corrupting Blood into pus, and coughing damages the lung tissue, resulting in pain upon palpation of the right hypochondrium. If the abscess has ruptured and wind pathogens have invaded, it is incurable. If the expectorated pus resembles congee (rice porridge), it is difficult to treat. However, Fei Wei (Lung Flaccidity) is characterized by deficiency of Righteous Qi, whereas Fei Yong is characterized by excess of Pathogenic Factors. The treatment principle for Fei Wei lies in nourishing Blood and Qi, calming the Spirit, and clearing Metal (clearing Lung heat/dryness); while the treatment for Fei Yong lies in tonifying Qi and Blood combined with draining pus. Nevertheless, Fei Wei can also present with cold syndromes, manifesting as dizziness, expectoration of saliva and foam without coughing, absence of thirst, and urinary incontinence. This indicates cold in the Lung and damp-fluid retention in the Heart region. Ancient practitioners used Zhi Gan Cao (Honey-fried Licorice Root) and Gan Jiang (Dried Ginger) to treat this, aiming to tonify deficiency and exhaustion.
