Chen Shigong stated: Chong She (Double Tongue) is caused by the reckless movement of Heart Fire, manifesting as an additional tongue-like structure beneath the tongue. The affected area should be pricked with a thread needle to release foul blood. Internally, Huanglian Xiexin Tang (Coptis Decoction to Drain the Heart) should be administered; externally, Bingpeng San (Borneol and Borax Powder) should be applied. (*Waike Zhengzong*)
Feng Luzhan stated: Chong She results from Heat in the Heart and Spleen. Since the Heart corresponds to the tongue and governs Blood, and the Spleen's meridians connect to the underside of the tongue, with Heart (Fire) and Spleen (Earth) sharing a mother-son relationship, the presence of Heat causes both Qi and Blood to become excessive. The condition attaches to the underside of the tongue, near the root, resembling a tongue but slightly shorter, with a different color. Pricking with needles is also suitable. (*Yizong Jinjian - Jinnang Milu*)
Dou Hanqing stated: Chong She involves the growth of a small tongue beneath the main tongue, which appears bright red. External signs include swelling under the chin and hard nodules. This indicates Heat in the Heart Meridian, with toxic vapors emerging from under the tongue. First, apply Zixue San (Purple Snow Powder); after hot saliva flows out, urgently administer Ganjie Tang (Licorice and Platycodon Decoction) decocted with Shengjiang (Fresh Ginger) and Dengxincao (Juncus Effusus). (*Chuangyang Jingyan Quanshu*)
He further stated: Lianhua Chong She (Lotus Double Tongue) arises when accumulated Heat combines with Wind pathogen, reflecting the principle of Heart Fire flaring upward.
He further stated: Lianhua She (Lotus Tongue) involves the growth of three small tongues beneath the main tongue, shaped like a lotus flower. Since the tongue is the sprout of the Heart, Heart Fire flaming upward, excessive rumination, injury by Fire pathogen, or cooling off while exposed to wind after drinking alcohol can cause Wind and Phlegm to struggle against each other, forming this condition. It is urgent to take Qingliang Jiedu Tang (Cooling and Detoxifying Decoction) with modifications, followed by insufflation of powder medicines. If the swelling does not subside, it is essential to prick out purple-black blood with a small knife.
He further stated: The growth of a small tongue beneath the main tongue is named Chong She. Because the tongue is the sprout of the Heart, Heart Fire flaming upward causes its formation. Use a small knife to prick the purple-black areas, insufflate Bingpian San (Borneol Powder), and take Ganjie Tang (Licorice and Platycodon Decoction) added with Zhizi (Gardenia) and Lianqiao (Forsythia).
He further stated: Swelling in the throat accompanied by Chong She indicates disease in both the Heart and Stomach meridians, with pathogens present in both. Heart pathogen manifests under the tongue, while Stomach pathogen emerges in the throat. When the pathogen in the throat triggers the pathogen in the Heart meridian, both diseases erupt simultaneously. External symptoms include headache, stiff neck, and tidal fever. The method for inducing expectoration is the same as before; subsequently treat with Zixue San (Purple Snow Powder) and Bingpian San (Borneol Powder).
Wang Shengzhi stated: Chong She occurs in men, women, adults, and children alike. It is due to the reckless movement of Heart Fire; the affected area should be pricked with a thread needle. (*Yangyi Lili*)
Luo Qian'an stated: The Spleen's meridians tie to the sides of the tongue, the Liver's meridians tie to the root of the tongue, and the Heart's meridians tie to the base of the tongue. When the three meridians are attacked by fetal toxin moving upward, a second tongue-like appearance forms under the tongue, named Chong She. Yanshou Dan (Longevity Pill) is appropriate. (*Tuina Mishu*)
Gu Shicheng stated: Chong She and Lianhua Chong She often occur in men and women who are emotionally depressed and have suffered excessive worry and rumination. If neglected over time, it leads to ulceration and putrefaction. Once the tongue tip ulcerates, although the outer shell remains, the interior resembles rotting fish intestines. One must absolutely avoid using refined/mineral drugs for insufflation or application; doing so will cause blood to gush like a spring, leading to perforation of the cheeks and rotting of the gums, with almost no survivors. I have seen several cases where patients died due to the reckless treatment by incompetent practitioners. Caution is imperative.
