Infant "Mantis Child" (Tanglangzi) arises from the fetus inhaling and swallowing in the womb, leading to fetal fire and blood heat. It manifests three to five days after birth, or half a month to over a month later. Initially, the outer cheeks are hard while the inner parts are swollen, making breastfeeding difficult. At this stage, take one bean of Crotonis Fructus (Badou, 巴豆) and seven li of Moschus (Shexiang, 麝香), crush and grind them evenly into two pills. Place these on a plaster and apply to bilateral Taiyang (Ex-HN5, 太阳) and Fengmen (BL12, 风门) acupoints. Blisters will appear the next day; remove the plaster, promptly puncture the blisters with a silver needle, and drain the toxic fluid for recovery. Care must be taken that this fluid does not enter the eyes, as it may cause visual impairment. Another method involves taking one liang of fresh Rehmanniae Radix (Shengdihuang, 鲜生地) and one liang of Rheum Palmatum (Dahuang, 大黄), crushing them into a paste, adding two fen of Moschus (Shexiang, 麝香), dividing into two portions, and binding them to both Yongquan (KI1, 涌泉) acupoints on the soles of the feet. If breastfeeding remains difficult after three days, incise with a knife, excise the necrotic tissue, and apply ink made from genuine Beijing inkstone (Jingmo, 京墨) for self-healing; some cases may develop external cheek induration and suppuration due to surgical injury.
Snow Mouth Canker (Xuekou Gan) results from fetal heat accumulating in the Heart and Spleen, steaming upwards to the mouth. White patches cover the tongue like goose beak debris, hence also called Goose Mouth (E Kou, 鹅口). In severe cases, the pharynx swells in layers, hindering feeding and causing excessive crying during nursing. Immediately use one qian of Natrii Sulfas (Yueshi, 月石), dissolve in half a cup of boiling water. Wrap cotton around a chopstick, dip in the solution, and vigorously wipe away the white coating until bleeding occurs without harm. Then insufflate Bingpeng San (Ice and Borax Powder). If white spots resembling plum blossoms appear on the tongue and inside the mouth, it is named Plum Blossom Canker (Meihua Gan, 梅花疳), treated similarly. For internal decoction, use Coptidis Rhizoma (Chuanlian, 川连), Rheum Palmatum (Dahuang, 大黄), Lonicerae Japonicae Flos (Yinhua, 银花), Uncariae Ramulus cum Uncis (Gouteng, 钩藤), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), Moutan Cortex (Danpi, 丹皮), Gardeniae Fructus (Shanzhi, 山栀), Dendrobii Caulis (Shihu, 石斛), etc., or add Rhinocerotis Cornu juice (Xijiao, 犀角).
Regarding Jaw Lock with Cheek Swelling (Yayao Tuosai), etiologies include: one arising from Wind-Cold-Heat-Dampness, with Yangming Damp-Heat steaming together; the other from Yin deficiency and empty collaterals, with Shaoyang Gallbladder Fire counterflowing along the meridians. Cases from Wind-Cold-Heat-Dampness begin with aversion to cold and fever, facial edema and cheek swelling, inability to open and close the jaw, gingival swelling extending to the throat, where liquids seem difficult to swallow but actually can pass. At this stage, employ dispersing and releasing exterior pathogens, using Akebiae Caulis (Niubangzi, 牛蒡 - note: usually Arctii Fructus here implies Arctii Fructus/Niubangzi), Menthae Haplocalycis Herba (Bohe, 薄荷), Gentianae Macrophyllae Radix (Qinjiao, 秦艽), Bombyx Batryticatus (Jiangcan, 僵蚕), Prunellae Spica (Xiakucao, 夏枯草), Schizonepetae Spica (Jingjie, 荆芥), Dendrobii Caulis (Shihu, 石斛), etc. Do not immediately use fresh Rehmanniae Radix (Shengdi, 鲜地) or Antelope Horn (Lingyangjiao, 羚羊) for clearing fire just because of throat distension and heat. Suppression and stagnation will make swelling harder and jaw lock tighter. After three to four days, if cold and fever persist and cannot dissipate, pus forming at the end of the molars is called Jaw Lock (Yayao); pus at the outer cheek is called Cheek Swelling (Tuosai); pus at the tooth root is called Dental Abscess (Yayong, 牙痈). Clear fire and eliminate heat using Antelope Horn (Lingyangjiao, 羚羊), Rhinoceros Horn (Xijiao, 犀角), Dendrobii Caulis (Shihu, 石斛), Phragmitis Rhizome (Lugen, 芦根), Menthae Haplocalycis Herba (Bohe, 薄荷), Schizonepetae Spica (Jingjie, 荆芥). If the tongue coating is white, greasy, and slippery with nausea and vomiting, use Scutellariae Radix (Huangqin, 黄芩), Coptidis Rhizoma (Huanglian, 黄连), Gardeniae Fructus (Shanzhi, 山栀) or Wendan Decoction method. If stool is dry and solid, indicating Lung and Stomach excess heat, use fresh Rehmanniae Radix (Shengdi, 鲜地), Trichosanthis Fructus (Gualou, 栝蒌), Aurantii Fructus Immaturus (Zhishi, 枳实), Armeniacae Semen Amarum (Xingren, 杏仁). After rupture, pathogens exit with pus, body heat subsides, Spleen and Stomach recover, jaw opens naturally, and after a few days of recuperation, pus resolves and the wound closes.
Cases arising from Yin deficiency and empty collaterals often involve individuals who habitually drink alcohol and lack sexual restraint, experiencing chronic depression. Occasionally exposed to minor pathogens, triggering Liver Yang and Gallbladder Fire to ascend into collaterals. It starts with rigid jaw lock, cheek swelling, gingival distension, aversion to cold, body heat, fine and rapid pulse, gradually forming pus. Ten days to half a month later, pus drains from gums or beside the tongue, spreading from left to right or vice versa, named Along-Tooth Toxicity (Yanya Du, 沿牙毒); prolonged cases protrude from the cheek side, named Through-Tooth Toxicity (Chuanya Du, 穿牙毒); penetrating externally and internally, named Bone Trough Wind (Guchao Feng, 骨槽风). Delay leads to necrotic bone, requiring multiple bone fragments to shed before healing. Though not fatal, recovery is slow. Treatment focuses on soothing Liver, dispersing depression, and nourishing Yin, assisted by detoxification and clearing heat. In infants, lacking alcohol indulgence or Liver Qi stagnation, this arises from incomplete tooth eruption, unfull marrow, and insufficient Ying and Wei Qi. Minor Wind-Cold or pathogenic heat invades the marrow; righteous Qi fails to expel pathogens. Diffuse cheek swelling occurs; although pus forms internally, it inevitably ruptures externally, failing to heal and forming necrotic bone. Treatment generally harmonizes Qi and Blood and supports Stomach Qi. Only when righteous Qi is sufficient can necrotic bone shed and the wound close.
