Ruyi Jinhuang San (如意金黄散, Ruyi Golden Yellow Powder) is suitable for Yang-heat conditions but detrimental to Yin-cold conditions. Whenever the pathology resides in the sinews and bones, presenting first with soreness and pain followed by hardness and diffuse swelling, Jinhuang San must not be used. The Zhu family of Huangqiang has studied this over several generations and deeply understands its drawbacks. My teacher, Mr. Langxian, therefore formulated this prescription to treat early-stage symptoms of Gujue (附骨疽, Bone Abscess) and Huantiao Ju (环跳疽, Abscess at the Huantiao Acupoint), characterized initially by vague pain that gradually progresses to a lump-like wooden swelling; its efficacy is most rapid. It is also effective for treating pulling pain in the sinews and bones caused by falls and injuries.
Chao Xiangfu (Stir-baked Cyperi Rhizoma), Xi Qianghuo (Western Notopterygii Rhizoma et Radix), Chuan Duhuo (Sichuan Angelicae Pubescentis Radix), Shang Angui (Upper Grade Cinnamomi Cortex, removing dead bark), Sheng Nanxing (Raw Arisaematis Rhizoma), Bei Xixin (Northern Asari Radix et Folium) (each 4 liang); Fen Gancao (Powdered Glycyrrhizae Radix et Rhizoma) (4 liang); Chuan Wutou (Sichuan Aconiti Radix), Cao Wutou (Processed Aconiti Radix), Gaoliangjiang (Galangal Rhizome) (each 2 liang); Gong Dingxiang (Caryophylli Flos) (1 liang); Jixingzi (Prunus armeniaca seed) (5 liang).
Grind each into an extremely fine, pure powder and mix evenly. When using, boil Wu Hui Jiu (grain wine without ash impurities) with 3-5 stalks of green onion including roots, then use this liquid to mix the medicine. Apply as a hot compress to the affected area, wrap with silk gauze, and change twice daily. If the cold pathogen is severe, combine with Siwen Dan (Four Warm Pellets) in equal proportions.
This is a modified application of Ruyi Jinhuang San. Jinhuang San has a cold-cooling nature and is only suitable for cases with significant redness, swelling, and heat. If there is muscle swelling and pain at the onset without redness, although it is considered a Yang syndrome, it involves Qi stagnation and Blood coagulation; the medication should focus on unblocking and dispersing to achieve resolution. Suddenly using strongly cold medicines to suppress the stagnation will cause Qi and Blood to become hard and congealed, making them impossible to disperse; this effectively forces the pathogens to consolidate, nurtures pus formation, and inadvertently aids ulceration. Whose fault is this? In treating such cases, even if warm medicines cannot be used to exacerbate the condition, the sole secret must be to unblock collaterals and drain/disperse. Mr. Langxian Zhu personally compiled this formula, which is cooling but not biased towards Yin-cold; it disperses swelling, softens hardness, and drains stagnant heat. It is effective for Yang-Fa (阳发, Yang-type swellings) with redness, swelling, and heat, or for Ru Yong (乳痈, Breast Carbuncle) and Ding Du (疔毒, Carbuncle toxins) that have not yet become highly swollen or red, as well as for diffuse, hard swelling; it yields immediate results in all these cases. Its merit actually surpasses that of Jinhuang San.
Qianghuo (Notopterygii Rhizoma et Radix), Danggui (Angelicae Sinensis Radix), Gancao (Glycyrrhizae Radix et Rhizoma) (each 3 liang); Chenpi (Citri Reticulatae Pericarpium), Huangbai (Phellodendri Cortex), Dahuang (Rhei Radix et Rhizoma), Jixingzi (Prunus armeniaca seed) (each 2 liang); Nanxing (Arisaematis Rhizoma), Baizhi (Angelicae Dahuricae Radix), Chishao (Paeoniae Radix Rubra) (each 1.5 liang); Mayaxiao (Salpetre), Yinzhu (Cinnabaris) (each 1 liang); Lvdou Fen (Vigna mungo powder) (4 liang).
Grind the above into fine powder, mix evenly, and store tightly sealed. For redness, swelling, and heat, mix with natural juice extracted from pounded Hedysengi Caulis (Lonicerae Japonicae Caulis). Natural juices from Dayeqingye (Isatidis Folium), Furongye (Hibisci Sabdariffae Folium), Machi (Portulacae Herba), etc., can also be used. For Shi Du (时毒, Seasonal Toxins) and Fa Yi (发颐, Parotitis), decoct Fangfeng (Saposhnikoviae Radix) 3 qian and Bohe Ye (Menthae Haplocalycis Folium) 2 qian in water to mix the powder, or add about ten drops of Bohe You (Menthae Haplocalycis Oil), mixing with hot decoction of Gancao.
