Miasma (miasmatic toxin) originates from the Zang organs, spreading from the Yin aspect to the Yang aspect. The fire-toxin blazes and scorches, consuming and injuring Ying blood (nutritive blood). Therefore, after the eruption of measles, there are often complications of lingering toxins. One must not use Qi-tonifying herbs to assist the fire; rather, one should only employ methods to nourish Yin to subdue Yang. This is the general principle of treatment.
The disease manifested as scrofula (tuberculous lymphadenitis) on the neck following the measles, persisting from spring to winter with unceasing progression. I have devised three therapeutic methods to discuss its management. Generally, if scrofula does not resolve, it is because the toxin has not been cleared. However, toxin is essentially fire; to remove the toxin, one must remove the fire. It must be understood that fire can be excess or deficiency, and diseases can be new or chronic. Before the measles erupt, the fire is in an excess state, so medications should be cooling. After the measles recede, the fire is in a deficiency state, so medications should be nourishing. By prescribing Liuwei Dihuang Tang (Six-Ingredient Rehmannia Decoction) as a decoction to swallow Xiaoluo Wan (Scrofula-Resolving Pill), it is hoped that the scrofula will dissipate without damaging the Yuan qi (original qi). Furthermore, humans rely on Stomach Qi (digestive function) as their foundation. For long-term illness requiring medication, the Spleen and Stomach must be thoroughly investigated. This approach simultaneously nourishes Yin and softens hardness to eliminate scrofula, while tonifying the Spleen and Stomach to support the Yuan qi; these two paths run parallel without contradiction.
An Bo's commentary: This brilliant discussion is delivered with clarity, leaving the reader with a refreshing sensation.
The exogenous syndrome has persisted for over twenty days, yet the hypochondriac pain remains unresolved. Analyzing hypochondriac pain caused by external pathogens, the pathology lies in the Shaoyang channel; hypochondriac pain caused by internal injury lies in the Jueyin channel. Now that the external pathogen has been resolved for many days, why does the hypochondriac pain not heal? Since there is no emotional depression, it must be an injury caused by physical strain or sudden movement, triggered by the residual external pathogen. Chronic pain typically resides in the collaterals, which govern blood. To prevent the accumulation of blood stasis leading to hematemesis or epistaxis, we follow the example of "Liver Fixation" (Gan Zhuo) from *Jin Gui Yao Lue* (Essential Prescriptions of the Golden Cabinet), using Xuanfu Dahong Huaxia Tang (Inula and Bighead-Kikery Flower Decoction).
An Bo's commentary: When a disease lingers and enters the collaterals, this reasoning is indeed appropriate. If phlegm-searching and fluid-draining herbs were added to the prescription, no hidden pathogens would remain.
The illness has lasted over four years, beginning with vomiting and gradually progressing to amenorrhea (cessation of menstruation). Warming, clearing, and purging methods were all attempted without effect. Physicians then diagnosed it as Laozai (consumptive disease, such as tuberculosis) and abandoned treatment. The pulse was not rapid, and there were no accompanying symptoms of wind invasion, shortness of breath, alternating chills and fever, or cough, suggesting a distinction from consumptive disease. If it were truly a case of advanced deficiency and weakness, with the disease entrenched in the Gaohuang acupoints (deep-seated vital areas), how could it prolong life for so long? The *Huangdi Neijing* (Yellow Emperor's Inner Canon) states: "Treating disease requires seeking its root." It also states: "The primary disease is the root; the secondary disease is the branch." This illness originated from vomiting, so vomiting must be considered the root of the disease. If vomiting can be stopped, food intake will be preserved, providing sustenance for life. With the free circulation of Qi and blood, other symptoms will resolve spontaneously without specific treatment. Reviewing medical texts, the discussion of vomiting syndromes covers various pathways. This patient suffers neither from true cold nor excess fire, which explains why warming and clearing methods were ineffective. As for purgative methods, they are indicated for acute exogenous febrile diseases; applying them to chronic internal injuries is fundamentally erroneous. Inquiry into dietary habits revealed that food stagnates in the diaphragm after swallowing, refusing to descend, and is immediately vomited up. There is rebellious upward movement and frequent belching. The reason for this is that the Liver is the source of the pathology, and the Stomach is the site of transmission. Stomach Qi should descend to maintain harmony; however, rebellious Liver Qi obstructs the descent of Stomach Qi, causing disharmony and pathology. The Spleen belongs to damp earth, while the Stomach belongs to dry earth. Liujunzi Tang (Six Gentlemen Decoction) and Yigong San (Extraordinary Efficacy Powder) can only strengthen Spleen Yang. Here, the pathology is in the Stomach, not the Spleen; since dampness and dryness differ significantly, they cannot be treated indiscriminately. Furthermore, the Liver is the "General," possessing a rigid nature and impatience. Wood energy prefers smooth flow and detests depression. Ancient physicians treated Liver diseases using pungent dispersants, sour astringents, and sweet relaxants, as well as methods to nourish Water to generate Wood, or fortify Earth to control Wood. There are diverse methods, not merely attacking or purging. Treatment should focus on calming the Stomach and restraining the Liver, harmonizing both the Jueyin and Yangming channels. The royal way (gentle regulation) yields no immediate results; therefore, advising the patient to avoid anger and relax the mind is essential to assist the medicinal efficacy.
An Bo's commentary: Zaoshintu (Fried Longan Aril / Dragon Brain Earth, specifically Fulonggan) warms the middle and stops vomiting while pacifying the Liver; this substance is indispensable for this condition.
