[按]此案一方面经淋沥色紫，为内有瘀阻；另一方面病历廿余日，体质已虚。症属虚中夹实，不宜一味通瘀。方用红花、川芎、苏木以通为止；地黄、龟板、冬青子、旱莲草、黑大豆养血滋阴；牛角腮含有蛋白胶质，功能止血。合为通涩并用之剂。
22．朱女。花甲以外之人，天癸已绝，月内漏红者再，腰酸，少腹沉坠。此病理性出血，更恐子宫实质病变生瘤、生癌。此方增加血液凝固，预防大冲。生熟地各15克，墨旱莲12克，乌贼骨30克(煅先煎)，冬青子12克，杜仲9克，生阿胶24克，(烊化，分2次冲)，川续断9克，黑大豆30克(煎汤代水)，震灵丹9克(分3次吞)。[按]老年妇女经断后漏红，甚至带下恶臭，必须作妇科检查以确定是否有子宫肌瘤或癌症，这在临床切不可疏忽大意。23．姚女。经淋沥3周之久，曾三度量多如冲，其色鲜红。此为子宫出血，非一般月经可比。阿胶30克(烊冲)，干地黄18克，小蓟炭12克，山萸肉9克，炒黑蒲黄12克，牛角鳃炭9克，仙鹤草12克，川断9克，金毛脊9克，陈棕炭9克，苎麻根12克。二诊：崩漏之量较前大减，而总不能根除，脉不整调，其人平素有心悸之疾，是血少使然。阿胶24克(烊冲)，熟地24克，杜仲'9克，金毛脊9克，桑寄生9克，炒黑蒲黄9克，山萸肉9克，五味子4．5克，金樱子9克，震灵丹9克(分2次吞)。
24．李女。正常月经是生理性出血，崩漏是病理性出血。病者面色咣白，爪甲无华，舌淡脉细，古籍以为脾不统血，当气血两补。黄芪18克，党参9克，熟地18克，旱莲草9克，升麻3克，炮附块9克，炮姜炭2．40克，杜仲9克，牛角（角思）9克，煅乌贼骨24克，苎麻根24克，阿胶30克(烊冲)，仙鹤草15克。
25．吴女。暴崩当止，久漏当通。经行淋沥，匝月不净，少腹既见胀满，究以通之为宜。所谓通药，实际上皆子宫收缩剂。生蒲黄12克，小蓟12克，苏木6克，当归9克，炙乳没各6克，藏红花3克，川芎4．5克，炮姜3克，肉桂末1．5克，震灵丹9克(分2次吞)。二诊：通涩并用，胶红饮其选也。阿胶24克(烊分三次冲)，全当归12克，熟地15克，小蓟炭9克，仙鹤草15克，炒蒲黄9克，杜红花3克，冬瓜子15克，震灵丹9克(分2次吞)。
26．王女。年近六旬，动则头眩，肝虚也。经绝已久，忽漏红，色鲜，五心烦热，前曾白带淋漓。不仅是病理性出血，子宫生瘤亦属可能。熟地30克，阿胶24克(烊冲)，龟板24克(先煎)，旱莲草12克，生地榆9克，牛角鳃炭12克，乌贼骨30克(煅先煎)，黑大豆30克，震灵丹12克(2次吞)。
[按]以上四案均为崩漏。姚案为阴虚血热妄行，首用养阴、凉血止血，继则养肝肾、调冲任；李案为脾虚不能统血，用归脾汤意，两补气血，所谓温经止血；吴案久漏，内有瘀血留着，用以通为止之法；王案高年经绝月久，又见漏红，且白带淋漓，有子宫瘤之嫌，用药旨在调补冲任、养阴止血。并嘱去医院进一步检查，以排除恶变。
带下
1．徐女。据其面色，可用补剂去其带。绵杜仲9克，玉蜀黍须15克，杭白芍9克，云茯苓9克，淮山药9克，山萸肉9克，金樱子9克，芡实9克，金毛脊9克。二诊：用补药除带，天然恢复体力之衰弱，间接使其带减少而已。熟地黄18克，杜仲9克，云茯苓9克，冬青子9克，山药9克，金毛脊9克，金樱子9克，龙眼肉9克。[按]体虚之人，面色少华，故用补虚束带的方法。先生认为这种方法只能间接使其带减少，可见治带应以清利湿热、消除炎症为主要措施。
2．吴女。其主要证候，可划分两部分：①子宫内膜炎症引起经之先期与带有腥味；②四肢麻，腱反射降低。上者古人属于湿热下注，下者属于血虚。苦参片9克，香白芷9克，象贝母9克，淮牛膝12克，宣木瓜9克，北细辛2．4克，稀莶草9克，杜赤豆30克。[按]经之先期与带有腥味，先生径责之为子宫内膜炎症，治用苦参、白芷、象贝、赤豆，以清热、利湿、消炎。其余四味均为肢麻而设。
3．孙女。经将行之旬日，下赤白带，质粘而腥，腰酸而腹胀痛，其痛偏向少腹之左方。其为炎症无疑也。马鞭草12克，旱莲草12克，生侧柏叶24克，荜澄茄9克，桑白皮12克，川黄柏9克，小茴香4．5克，小蓟12克，瞿麦穗12克，甘草梢4克。另：刘氏猪肚丸240克，日2服，每服4．5克[按]此案药分三路：一以黄柏、瞿麦、桑皮、甘草梢等味，清利湿热消炎，治带下粘而腥；一用旱莲、侧柏、小蓟滋阴凉血，以止赤带；并佐用小茴香理气行滞，以治其少腹胀痛。
4．张女。经后4个月，腹痛而有热，洒淅恶寒，其痛得按更甚，带下频。寒热之来源，痛使然也。桑白皮12克，生侧柏叶30克，台乌药6克，杭白芍9克，象贝母9克，苦参片9克，飞滑石12克(包)，粉萆薜9克，炙乳没各4．5克，桃仁泥9克。二诊：药后，带下减而腹痛之势杀，可见带下与痛有联系。生苍术9克，川黄柏9克，马鞭草9克，苦参片9克，生侧柏叶30克(煎汤带水)，荜澄茄9克，炙乳没各4．5克，小茴香4．5克。[按]该病人初诊时，体温38．6℃，脉搏105次／分。复诊时，体温降至37．2℃，脉搏84次／分。
5．沈女。主症在腹痛，因痛而带下频，手不可近，自觉有寒热。小茴香6克，延胡索9克，白芍12克，苦参片9克，当归9克，象贝母12克，柴胡9克，桃仁15克，白薇9克。[按]此案主症在腹痛。带下频、寒热均为腹痛所引起。腹痛手不可触，为内有瘀阻。方中用苦参、象贝、桃仁，消炎、祛瘀；小茴、玄胡、归、芍活血、镇痛；柴胡、白薇解郁、清热。全方以清热、解除腹痛为目标。
6．邵女。据其所述之情态，腹痛之原因仍是在带下，中脘之痛乃放散性。杭白芍12克，樗白皮9克，萆薢9克，生侧柏叶18克，泽泻9克，瞿麦9克，象贝母9克，云苓18克，甘草梢4．5克。[按]带下导致腹痛，其痛必在少腹。从药测症，病由湿热蕴结下焦，症情颇似盆腔炎之类。方中大队清利湿热之品，均有消炎作用。白芍和营缓痛，贝母"功专散结除热"，可治一切痈疡肿毒；二药相伍，对炎症性腹痛有较好的效果。
7．邓女。黄带多属湿热下注，其质虽粘，却无腥臭。粉萆薢9克，泽泻9克，云苓12克，冬葵子9克，瞿麦9克，白薇9克，三妙丸12克，小生地12克，剪芡实9克，篇蓄草9克。另：海金沙9克，飞滑石12克，二味同泡代茶。
[按]古人论带下，以色黄腥臭属湿热，以色淡清稀为体虚。本案黄带无腥臭，系湿热下注，损伤阴液，故治以清利下焦湿热为主，辅以生地、芡实养阴固带。
8．王女。经先期始则量多如冲，继则淋沥不绝，淡红中夹有白带，自觉有腥臭。既是腥臭，则非生理之出血；既有寒热，便有炎症。小溲频而短，炎症在泌尿系。樗皮6克，瞿麦9克，杭芍9克，生侧柏叶18克，旱莲草12克，白薇9克，苦参4．5克，象贝母9克，猪苓9克，飞滑石12克，白茅根1扎
二诊：凡子宫附属器炎症，内服剂本难发生直接作用。药后带与腥臭皆差减，吾人当据事实而确定药理。樗皮12克，黄柏3克，白薇9克，苦参片4．5克，白芍12克，柏子仁9克(打)，炙乳没各2．4克，荜澄茄9克，制香附9克，老苏梗6克，生侧柏叶30克(煎汤代水)。
三诊：带下腥臭者固涩法虽能暂止，但非根治，故古人以捕化湿热为主。生地榆9克，粉萆薢9克，生苍术6克，瞿麦9克，萹蓄草9克，黄柏2．4克，香白芷6克，赤苓9克，荜澄茄9克樗皮12克，生侧柏叶30克(煎汤代水)。
[按]月经先期，先量多如冲，后淋沥不绝，继则淡红中央有趣臭的白带，是为胞宫郁热，湿浊内阻，亦即盆腔炎症之类。方中苦寒：坚阴，清热化浊之品，如苦参、象贝、黄柏、地榆等，均有良好的消炎作用。
9．许女。带下如浊涕，微感腰部作胀，此不能纯用固涩。白芍9克，生侧柏叶15克，女贞子9克，怀牛膝9克，飞滑石12克(研包)，象贝母9克，泽泻9克，芡实9克，干地黄12克，怀山药9克。
[按]带下，总由带脉失于约束而为病，其病因有脾虚、湿热、肝郁、肾亏之异。其精微下泄者为虚，痰湿下注者多实。本案所病带下，状若浊涕，当是黄白相杂而质地稠粘，系痰湿下注而致；其腰部微胀，乃邪结任带，导致肾虚之征。调补三阴，固涩任带，固属治本之图，但虚处恋邪，亦当兼顾。先生以地黄、白芍、女贞、山药、芡实，补肾、养肝、实脾，益三阴以束任带；侧柏、象贝、泽泻，滑石、牛膝，清任、渗湿、化痰，驱邪浊以达下。十味之中，半事补涩，半用疏化，崇本廓邪，各擅其长，周匝允当，具见功力。
10．邵女。以带下为主症，少腹痛，痛在两侧，此卵巢部分，大致是子宫附属器发生炎症。生侧柏叶12克，象贝母12克，杭白芍9克，樗白皮9克；苦参片6克，泽泻9克，生山栀9克，黄柏3克，小蓟12克，粉草4．5克，马鞭草9克，凤尾草9克
[按]凤尾草有清热利湿、凉血消炎之功，为民间治带下的有效单方。11．李女。下赤白带，总是炎症。日来其量加多。改予古人清化湿热之法。黄柏9克，苦参6克，萆薢9克，白芍9克，泽泻9克，瞿麦9克，象贝9克，旱莲草9克，滑石12克，干地黄12克。
12．薛女。9年来，经虽以时下，而时有淋沥；其色或淡或紫，与带下混合，少腹痛。凡痛与带下总是炎症。樗皮9克，侧柏叶9克，象贝9克，苦参片9克，杭白芍9克，瞿麦9克，象牙屑4．5克(分3次吞)，威喜丸9克(分吞)。[按]李、薛二案均为炎症，带下的特点是赤白相混，治疗亦均以清化湿热，即消炎为主。
13．杨女。半月以来，白带与经相间，连续不断；最近数日，驯至经带不分，有腥味，腰为之酸楚。生侧柏叶15克，樗根白皮12克，怀牛膝9克，泽泻9克，川黄柏9克，生苍术9克，小茴香2．4克，荜澄茄9克。二诊：宗樗皮侧柏叶汤加减，二服后带下腥臭霍然若失。小蓟12克，川黄柏6克，杭白芍9克，粉萆薢9克，生苡仁15克，紫花地丁9克，苦参片9克，制香附9克，小茴香2．4克，象贝母9克。
14．陈女。现在之虚弱，基于往日之积劳；产后2个月，带下赤白，腰痛如折。杜仲9克，怀山药9克，小蓟炭12克，扁豆衣9克，续断9克，芡实9克，仙鹤草15克，怀牛膝9克，白芍12克，五味子4．5克。二诊：头晕眩，带下如注。生芪9克，怀山药15克，甘杞子9克，杜仲9克，煅牡蛎30克，党参9克，升麻4．5克，白芍9克，云苓9克。
15．徐女。带下色淡如水，且无臭气，其脉弱，补之可愈。杜仲9克，金毛脊9克，山药9克，怀牛膝9克，金樱子9克，五味子4．5克，芡实9克，鹿角霜15克，震灵丹9克(分3次吞)。

二诊：凡子宫附属器炎症，内服剂本难发生直接作用。药后带与腥臭皆差减，吾人当据事实而确定药理。樗皮12克，黄柏3克，白薇9克，苦参片4.5克，白芍12克，柏子仁9克（打），炙乳没各2.4克，荜澄茄9克，制香附9克，老苏梗6克，生侧柏叶30克（煎汤代水）。
三诊：带下腥臭者固涩法虽能暂止，但非根治，故古人以捕化湿热为主。生地榆9克，粉萆薢9克，生苍术6克，瞿麦9克，萹蓄草9克，黄柏2.4克，香白芷6克，赤苓9克，荜澄茄9克樗皮12克，生侧柏叶30克（煎汤代水）。
[按语]月经先期，先量多如冲，后淋沥不绝，继则淡红中央有趣臭的白带，是为胞宫郁热，湿浊内阻，亦即盆腔炎症之类。方中苦寒：坚阴，清热化浊之品，如苦参、象贝、黄柏、地榆等，均有良好的消炎作用。
9．许女。带下如浊涕，微感腰部作胀，此不能纯用固涩。白芍9克，生侧柏叶15克，女贞子9克，怀牛膝9克，飞滑石12克（研包），象贝母9克，泽泻9克，芡实9克，干地黄12克，怀山药9克。
[按语]带下，总由带脉失于约束而为病，其病因有脾虚、湿热、肝郁、肾亏之异。其精微下泄者为虚，痰湿下注者多实。本案所病带下，状若浊涕，当是黄白相杂而质地稠粘，系痰湿下注而致；其腰部微胀，乃邪结任带，导致肾虚之征。调补三阴，固涩任带，固属治本之图，但虚处恋邪，亦当兼顾。先生以地黄、白芍、女贞、山药、芡实，补肾、养肝、实脾，益三阴以束任带；侧柏、象贝、泽泻，滑石、牛膝，清任、渗湿、化痰，驱邪浊以达下。十味之中，半事补涩，半用疏化，崇本廓邪，各擅其长，周匝允当，具见功力。
10．邵女。以带下为主症，少腹痛，痛在两侧，此卵巢部分，大致是子宫附属器发生炎症。生侧柏叶12克，象贝母12克，杭白芍9克，樗白皮9克；苦参片6克，泽泻9克，生山栀9克，黄柏3克，小蓟12克，粉草4.5克，马鞭草9克，凤尾草9克
[按语]凤尾草有清热利湿、凉血消炎之功，为民间治带下的有效单方。11．李女。下赤白带，总是炎症。日来其量加多。改予古人清化湿热之法。黄柏9克，苦参6克，萆薢9克，白芍9克，泽泻9克，瞿麦9克，象贝9克，旱莲草9克，滑石12克，干地黄12克。
12．薛女。9年来，经虽以时下，而时有淋沥；其色或淡或紫，与带下混合，少腹痛。凡痛与带下总是炎症。樗皮9克，侧柏叶9克，象贝9克，苦参片9克，杭白芍9克，瞿麦9克，象牙屑4.5克（分3次吞），威喜丸9克（分吞）。[按语]李、薛二案均为炎症，带下的特点是赤白相混，治疗亦均以清化湿热，即消炎为主。
13．杨女。半月以来，白带与经相间，连续不断；最近数日，驯至经带不分，有腥味，腰为之酸楚。生侧柏叶15克，樗根白皮12克，怀牛膝9克，泽泻9克，川黄柏9克，生苍术9克，小茴香2.4克，荜澄茄9克。二诊：宗樗皮侧柏叶汤加减，二服后带下腥臭霍然若失。小蓟12克，川黄柏6克，杭白芍9克，粉萆薢9克，生苡仁15克，紫花地丁9克，苦参片9克，制香附9克，小茴香2.4克，象贝母9克。
14．陈女。现在之虚弱，基于往日之积劳；产后2个月，带下赤白，腰痛如折。杜仲9克，怀山药9克，小蓟炭12克，扁豆衣9克，续断9克，芡实9克，仙鹤草15克，怀牛膝9克，白芍12克，五味子4.5克。二诊：头晕眩，带下如注。生芪9克，怀山药15克，甘杞子9克，杜仲9克，煅牡蛎30克，党参9克，升麻4.5克，白芍9克，云苓9克。
15．徐女。带下色淡如水，且无臭气，其脉弱，补之可愈。杜仲9克，金毛脊9克，山药9克，怀牛膝9克，金樱子9克，五味子4.5克，芡实9克，鹿角霜15克，震灵丹9克（分3次吞）。
19. Female Patient Di. Her facial complexion and tongue presentation both indicate severe anemia (Anemia). Ginseng (Ginseng Radix et Rhizoma) and Aconite (Aconiti Lateralis Radix Praeparata) restore Yang and rescue from collapse; clinically proven to have anti-shock effects, they are established as the main herbs. Astragalus, Rehmannia, Donkey-hide Gelatin, Ginger Charcoal, and Agrimony function to tonify blood, secure essence, stop bleeding, and control hemorrhage. [Commentary] Whole body pale yellow like wax is a sign of acute anemia; palpitations with bilateral foot edema indicate insufficient heart power. The cause is excessive blood loss from massive uterine bleeding. Ancient texts state "when blood is lost, Qi is also lost." Such anemia is essentially a deficiency of both Qi and Blood, requiring large-dose warming and tonification to be effective. [Commentary] On one hand, menstrual dripping with purple color indicates internal blood stasis; on the other hand, the medical history is over twenty days, and the constitution is already deficient. The syndrome belongs to deficiency with intermingled excess, not suitable for purely unblocking stasis. The formula uses Safflower, Ligusticum, and Sappan Wood to stop bleeding by unblocking; Rehmannia, Tortoise Shell, Ilex Seeds, Eclipta, and Black Soybean nourish blood and yin; Buffalo Horn scale contains protein colloid, functioning to stop bleeding. [Commentary] The above four cases are all metrorrhagia and metrostaxis. Case Yao is Yin deficiency with blood heat causing reckless flow, initially using nourishing Yin, cooling blood, and stopping bleeding, then nourishing Liver and Kidney, regulating Chong and Ren vessels; Case Li is Spleen deficiency failing to control blood, using the intent of Guipi Tang, dual tonification of Qi and Blood, so-called warming meridians and stopping bleeding; Case Wu is prolonged dripping with internal blood stasis remaining, using unblocking to achieve cessation; Case Wang is an elderly woman long past menopause, now leaking red blood with profuse leukorrhea, suspecting uterine tumor, medication aims to regulate and tonify Chong and Ren, nourish Yin and stop bleeding. Advised to go to hospital for further examination to rule out malignancy.
Leukorrhea Disease
1. Female Patient Xu. Based on her complexion, a tonic can be used to remove leukorrhea. Eucommia Bark 9g, Corn Silk 15g, Hangzhou White Peony Root 9g, Poria 9g, Huai Yam 9g, Cornelian Cherry Fruit 9g, Chinese Bayberry Fruit 9g, Chinese Quinoa Seed 9g, Polypodium Vulgare Rhizome 9g. Second visit: Using tonic medicine to remove leukorrhea naturally restores physical weakness, indirectly reducing leukorrhea. Prepared Rehmannia Root 18g, Eucommia Bark 9g, Poria 9g, Ilex Seeds 9g, Yam 9g, Polypodium Vulgare Rhizome 9g, Chinese Bayberry Fruit 9g, Longan Aril 9g. 2. Female Patient Wu. Her main symptoms can be divided into two parts: ① Endometritis causes early menstruation and fishy-smelling discharge; ④ Numbness in limbs, reduced tendon reflexes. The former belongs to Damp-Heat pouring downward according to ancients, the latter to Blood deficiency. Sophora Root Tablets 9g, Angelica Dahurica 9g, Fritillaria Bulb 9g, Huai Achyranthes Root 12g, Exocarpium Citri Grandis 9g, Northern Asarum 2.4g, Siegesbeckia Herb 9g, Red Adzuki Bean 30g. [Commentary] Early menstruation and fishy-smelling discharge are directly attributed by Mr. Zhang to endometritis, treated with Sophora, Angelica Dahurica, Fritillaria, and Red Bean to clear heat, drain dampness, and reduce inflammation. The remaining four herbs are set for limb numbness.
3. Female Patient Sun. Ten days before expected menstruation, discharge of red and white fluid, sticky and fishy, lower back soreness and abdominal distension pain, pain biased to the left side of the lower abdomen. Undoubtedly inflammation. Verbena Herb 12g, Eclipta Herb 12g, Raw Biota Tops 24g, Litsea Cubeba Fruit 9g, Mulberry Root Bark 12g, Phellodendron Bark 9g, Fennel 4.5g, Small Thistle 12g, Dianthus Flower Spike 12g, Licorice Tip 4g. Additionally: Liu's Pig Stomach Pill 240g, taken twice daily, 4.5g per dose. [Commentary] This case's herbs are divided into three routes: One route uses Phellodendron, Dianthus, Mulberry Root Bark, Licorice Tip, etc., to clear damp-heat and reduce inflammation, treating sticky and fishy leukorrhea; another uses Eclipta, Biota, Small Thistle to nourish Yin and cool blood to stop red discharge; assisted by Fennel to regulate Qi and move stagnation to treat lower abdominal distension and pain.
4. Female Patient Zhang. Four months after menstruation, abdominal pain with heat, shivering chills, pain worsens with pressure, frequent leukorrhea. The source of cold and heat is caused by pain. 5. Female Patient Shen. Main symptom is abdominal pain, causing frequent leukorrhea due to pain, touch unbearable, feeling cold and heat internally. Fennel 6g, Corydalis Rhizome 9g, White Peony Root 12g, Sophora Root Tablets 9g, Angelica Sinensis Root 9g, Fritillaria Bulb 12g, Bupleurum Root 9g, Peach Kernel 15g, Anemarrhena Rhizome 9g. [Commentary] The main symptom of this case is abdominal pain. Frequent leukorrhea and cold/heat feelings are all caused by abdominal pain. Abdominal pain unbearable to touch indicates internal blood stasis. In the formula, Sophora, Fritillaria, and Peach Kernel are used for anti-inflammatory and removing stasis; Fennel, Corydalis, Angelica, and White Peony for activating blood and analgesia; Bupleurum and Anemarrhena for resolving depression and clearing heat. The entire formula aims at clearing heat and relieving abdominal pain.
6. Female Patient Shao. Based on her description, the cause of abdominal pain is still leukorrhea, epigastric pain is radiating. Hangzhou White Peony Root 12g, Periploca Bark 9g, Dioscorea Hypoglauca Rhizome 9g, Raw Biota Tops 18g, Alisma Rhizome 9g, Dianthus Flower 9g, Fritillaria Bulb 9g, Poria 18g, Licorice Tip 4.5g. [Commentary] Leukorrhea causing abdominal pain, the pain must be in the lower abdomen. Inferring symptoms from herbs, the disease is due to Damp-Heat accumulating in the Lower Jiao, symptoms resembling pelvic inflammatory disease. The large group of herbs clearing damp-heat all have anti-inflammatory effects. White Peony harmonizes Ying and relieves pain gently, Fritillaria "specializes in dissipating nodules and removing heat," treating all kinds of carbuncles and sores; the combination of these two herbs has good effects on inflammatory abdominal pain.
7. Female Patient Deng. Yellow leukorrhea mostly belongs to Damp-Heat pouring downward, although sticky, no fishy odor. Fenugreek Root 9g, Alisma Rhizome 9g, Poria 12g, Okra Seeds 9g, Dianthus Flower 9g, Anemarrhena Rhizome 9g, Sanmiao Pill 12g, Little Raw Rehmannia 12g, Cut Quinoa Seed 9g, Polygonum Aviculare Herb 9g. Additionally: Jinsha 9g, Fly Talc 12g, these two soaked together to replace tea.
[Commentary] Ancients discussed leukorrhea, considering yellow and fishy-smelling as Damp-Heat, pale and thin as Body Deficiency. This case's yellow leukorrhea has no fishy odor, caused by Damp-Heat pouring downward damaging Yin fluids, thus treatment focuses mainly on clearing and draining Lower Jiao Damp-Heat, assisted by Raw Rehmannia and Quinoa Seed to nourish Yin and bind leukorrhea.
8. Female Patient Wang. Menstruation early, initially volume like a flood, then continuous dripping, pale red mixed with white discharge, feeling fishy odor. Since fishy, it is not physiological bleeding; since cold and heat exist, there is inflammation. Frequent and short urination, inflammation in the urinary system. Second visit: For inflammation of uterine appendages, oral preparations inherently struggle to have direct effects. After medication, discharge and fishy odor both improved, we must determine pharmacology based on facts. [Commentary] Early menstruation, initially volume like a flood, then continuous dripping, followed by pale red central fishy-smelling white discharge, this is Uterine Heat Depression, Damp-Turbidity internal obstruction, i.e., pelvic inflammatory disease-like. Bitter-cold herbs in the formula, such as Sophora, Fritillaria, Phellodendron, and Sanguisorba, which solidify Yin, clear heat, and transform turbidity, all have good anti-inflammatory effects.
9. Female Patient Xu. Leukorrhea like turbid mucus, slight waist distension, this cannot be purely treated with astringents. White Peony Root 9g, Raw Biota Tops 15g, Ligustrum Fruit 9g, Huai Achyranthes Root 9g, Fly Talc 12g (ground and wrapped), Fritillaria Bulb 9g, Alisma Rhizome 9g, Chinese Quinoa Seed 9g, Dry Rehmannia Root 12g, Huai Yam 9g.
[Commentary] Leukorrhea generally results from Dai Mai losing its constraint, with etiologies including Spleen Deficiency, Damp-Heat, Liver Stagnation, and Kidney Deficiency. Leakage of essence and nutrients is deficiency, while Phlegm-Damp pouring downward is mostly excess. The leukorrhea in this case resembles turbid mucus, likely yellow-white mixed and thick/sticky, caused by Phlegm-Damp pouring downward; slight waist distension indicates Evil knotting Ren and Dai meridians, leading to Kidney Deficiency signs. Regulating and tonifying the Three Yin, astringing Ren and Dai is indeed a strategy for treating the root, but when deficiency harbors Evil, it must also be considered. Mr. Zhang uses Rehmannia, White Peony, Ligustrum, Yam, and Quinoa Seed to tonify Kidney, nourish Liver, strengthen Spleen, benefit Three Yin to bind Ren and Dai; Biota, Fritillaria, Alisma, Talc, and Achyranthes to clear Ren, drain dampness, resolve Phlegm, drive out turbid Evil downwards. Among the ten herbs, half deal with tonifying and astringing, half with dispersing and transforming, honoring the root and clearing Evil, each excelling in their own way, comprehensive and appropriate, demonstrating skill.
