Postpartum aphasia (loss of voice) is caused by retained blood (stagnant blood) ascending to interfere with the Heart and Lungs. The orifices of these two organs are invaded by blood, and combined with contraction of Wind-Cold, it leads to a stiff tongue and inability to speak. Zhu Xue Bu Xin Tang (Blood-Expelling Heart-Nourishing Decoction) is indicated. Alternatively, if phlegm and Qi are obstructed, causing closed eyes and silence, use one qian of raw Bai Fan (Alumen, 白矾), ground into powder, mixed with hot water to induce vomiting. Or, if malignant blood attacks the Heart, leading to near-death silence, use three qian of Yu Jin (Curcuma Radix, 郁金), calcined to retain its properties, mixed with vinegar for oral administration. If the body was originally deficient, and labor injuries during delivery cause chaotic Qi and Blood to ascend and block the Heart orifice, preventing fluids from reaching the tongue, resulting in a stiff tongue and aphasia, Xiong Gui Tang (Ligusticum and Angelica Decoction) plus Shi Chang Pu (Acorus Rhizome, 石菖蒲) is indicated.
Zhu Xue Bu Xin Tang: Dang Gui (Angelicae Sinensis Radix, 当归, 1.5 qian), Sheng Di Huang (Rehmanniae Radix, 生地黄), Jie Geng (Platycodonis Radix, 桔梗), Zi Su Ye (Perillae Folium, 紫苏叶), Qian Hu (Peucedani Radix, 前胡), Fu Ling (Poria, 茯苓), Fang Feng (Saposhnikoviae Radix, 防风), Huang Lian (Coptidis Rhizoma, 黄连), Dan Xing (Armeniacae Amarum Semen Preparatum, 胆星), Hong Hua (Carthami Flos, 红花), Ge Gen (Puerariae Lobatae Radix, 葛根) (each 1 qian); Ren Shen (Ginseng Radix et Rhizoma, 人参), Bo He (Menthae Haplocalycis Herba, 薄荷), Sheng Ma (Cimicifugae Rhizoma, 升麻) (each 0.7 qian); Ban Xia (Pinelliae Rhizoma Praeparatum cum Zingibere et Alumine, 半夏, prepared with ginger, 1.2 qian); Gan Cao (Glycyrrhizae Radix et Rhizoma, 甘草, 0.5 qian); Sheng Jiang (Zingiberis Rhizoma Recens, 姜, 3 slices). Decoct in water and take orally.
Xiong Gui Tang: Dang Gui (Angelicae Sinensis Radix, 当归), Chuan Xiong (Chuanxiong Rhizoma, 川芎) (each 5 qian). Decoct in water and take orally.
Postpartum sudden visions of ghosts and spirits result from extreme Blood deficiency, where retained blood rushes upward, pathogenic factors invade the Heart, causing rambling speech as if seeing ghosts and spirits; this is not due to Wind pathogen. Miao Xiang San (Miraculous Fragrance Powder) and Tiao Jing Tang (Regulating Menses Decoction) are indicated. If postpartum madness and rambling speech occur five to six days after delivery, accompanied by holding a knife intending to kill, it is due to sudden violent collapse of Yin Blood and deficient flaming of Liver Fire. Ze Lan Tang (Dewberry Leaf Decoction) is indicated.
Miao Xiang San: Shan Yao (Dioscoreae Rhizoma, 山药, stir-fried with ginger juice), Fu Ling (Poria, 茯苓), Fu Shen (Poria with Hostwood, 茯神), Huang Qi (Astragali Radix, 黄芪), Yuan Zhi (Polygalae Radix, 远志, processed) (each 1 liang); Ren Shen (Ginseng Radix et Rhizoma, 人参), Gan Cao (Glycyrrhizae Radix et Rhizoma, 甘草), Jie Geng (Platycodonis Radix, 桔梗) (each 5 qian); Zhu Sha (Cinnabar, 朱砂, water-ground, 3 qian); Mu Xiang (Aucklandiae Radix, 木香, 2.5 qian); She Xiang (Moschus, 麝香, 1 qian, separately ground).
