(Spermatorrhea)
The *Neijing* states: Essence is the root of the body, the source of vital energy, and the transformation of marrow. It also states: When Yin is peaceful and Yang is secure, spirit and mind are regulated; when Yin and Yang separate and rupture, spirit and mind will be exhausted. Generally, the storage and control of essence reside in the Kidney, while the command over essence lies in the Heart. The Heart belongs to Ministerial Fire (Jun Huo), and the Kidney to Official Fire (Xiang Huo). Official fire obeys the orders of Ministerial fire; therefore, although the pathology is located in the Kidney, the responsibility lies with the Heart. The Heart stores the Shen (Spirit); when Shen moves, essence leaks. There are five pathogenic mechanisms: 1. Dream intercourse, due to blazing fire and deficient Shen, should be treated with herbs that nourish the Heart and astringe; 2. Slippage and loss, due to Kidney deficiency failing to grasp, should be treated by nourishing and tonifying Kidney Qi; 3. Fullness and overflow, due to young widowers or celibate men, should be managed by quietude allowing essence to transform naturally (essence can transform into Qi); 4. Emotional desire, due to delusional speech and visual/auditory stimulation, requires controlling thoughts and subduing the Heart; 5. Damp-heat, due to heat damaging free flow, requires draining dampness and clearing fire. Broadly speaking, restraining Official Fire, strengthening Kidney Water, calming the Heart Spirit, and securing essence and marrow are unchanging principles. A surging and rapid pulse at the Chi position indicates loss of essence and turbid urine; a short and small pulse at the Cun position indicates dream emission.
