(Note: Heel Pain)
The *Classic of Internal Medicine* states: Excessive dampness leads to swelling. It also states: Cooling off in the wind and engaging in sexual intercourse after intoxication often lead to Hong Kong Foot (a disease characterized by swelling, pain, weakness, and numbness of the lower legs and feet). The feet are the external manifestation of the Kidney (*Shen*) and govern the lower body. The Kidney belongs to Water and is primarily associated with Cold transformation. Sudden invasion by Cold pathogenic factors damages the Kidney first; as Water-Dampness accumulates in the Lower Jiao, the feet are affected first. After food enters the Stomach, Spleen Dampness flows downward, pouring into the feet and failing to dissipate. Wind, Cold, Summerheat, and Dampness can all cause this disease, but Damp-Heat is most common. Initially, there is aversion to cold and fever, resembling Taiyang Disease (*Taiyang Bing*, a syndrome caused by pathogenic factors invading the Taiyang channel). However, the shins and calves appear red, shiny, swollen, and painful, with soreness and pain in the legs and knees, scanty and difficult urination, aversion to the smell of food, and even upward rushing of pathogens causing heart pain and vomiting of green fluid. Do not mistakenly treat it as a swelling or toxin. If swollen, it is named Wet Hong Kong Foot, suitable for treatment with methods that promote diuresis to drain dampness and clear heat. If painful but not swollen, it is named Dry Hong Kong Foot, suitable for nourishing blood and moistening dryness. In the early stage, it is appropriate to use Baidu San (Decoction for Defeating Toxins) to induce sweating and expel pathogens (the main formula for Plague diseases); in winter, use Wuji San (Five Accumulations Powder) (see Middle Cold section), followed by the main formula of this school; subsequently, take Danggui Nian Tong Tang (Angelica Decoction for Relieving Pain) (see Body Pain section). A floating pulse indicates Wind, a tight pulse indicates Cold, a soggy pulse indicates Dampness, and a rapid pulse indicates Heat. If the pulse fluctuates suddenly in size, accompanied by delirious speech and inability to eat, the condition is critical.
Main Formula: Cangzhu (Atractylodis Rhizoma), Qianghuo (Notopterygii Rhizoma et Radix), Danggui (Angelicae Sinensis Radix) (each 1.5 qian), Fangfeng (Saposhnikoviae Radix), Fangji (Stephaniae Tetrandrae Radix), Zexie (Alismatis Rhizoma), Jiuhuangqin (Scutellariae Radix fried with wine), Yiren (Coicis Semen), Niuxi (Achyranthis Bidentatae Radix) (each 1 qian).
Add Shengjiang (Zingiberis Rhizoma Recens) and Zao (Jujubae Fructus), decoct in water and take orally.
For muscle spasms, add Mugua (Chaenomelis Fructus) and Lingxian (Syneilesis Herba); for constipation, add Zhishi (Aurantii Fructus Immaturus) and Dahuang (Rhei Radix et Rhizoma); for dysuria, add Zhuling (Polyporus), Yinchen (Artemisiae Scopariae Herba), or Dafupi (Arecae Pericarpium), Binglang (Arecae Semen), Jingjie (Schizonepetae Herba), Wuyao (Linderae Radix), Chenpi (Citri Reticulatae Pericarpium), Zisu (Perillae Folium), Zhike (Aurantii Fructus), Sangbai (Mori Cortex), Mugua (Chaenomelis Fructus), decoct with Shengjiang (Zingiberis Rhizoma Recens).
Binglang Tang (Betel Nut Decoction): Treats all types of Hong Kong Foot, unblocking stagnation and benefiting the chest and diaphragm.
Xiangfu (Cyperi Rhizoma), Chenpi (Citri Reticulatae Pericarpium), Zisu (Perillae Folium), Binglang (Arecae Semen), Wujia Pi (Acanthopanacis Cortex), Mugua (Chaenomelis Fructus), Zhigancao (Glycyrrhizae Radix et Rhizoma preparata) (each 7 fen). Add one slice of Shengjiang (Zingiberis Rhizoma Recens), decoct in water and take orally.
*Danliao Fang*: Treats Hong Kong Foot where pathogens enter the abdomen and rush to the heart, causing severe pain, abdominal swelling and fullness, and obstruction of both bowel and bladder movements.
Suye (Perillae Folium), Suzi (Perillae Fructus), Mugua (Chaenomelis Fructus) (each 7 fen), Qianghuo (Notopterygii Rhizoma et Radix), Baishao Yao (Paeoniae Radix Alba), Binglang (Arecae Semen), Zhike (Aurantii Fructus) (each 5 fen), Qingpi (Citri Reticulatae Pericarpium), Gancao (Glycyrrhizae Radix et Rhizoma) (each 3 fen). Decoct with Shengjiang (Zingiberis Rhizoma Recens), and mix with ground Chenxiang (Aquilariae Lignum Resinatum) and Muxiang (Vladimiriae Radix) (each 5 fen) before taking.
Jiming San (Crowing Cock Powder): The most effective first-line remedy for men and women suffering from Wind-Dampness Hong Kong Foot, where pathogens flow into the meridians, causing extreme swelling and pain.
Binglang (Arecae Semen) (7 pieces), Chenpi (Citri Reticulatae Pericarpium), Mugua (Chaenomelis Fructus) (each 1 liang), Wuzhuyu (Evodiae Fructus) (2 qian), Shengjiang (Zingiberis Rhizoma Recens), Jiegeng (Platycodonis Radix) (each 5 qian), Zisu (Perillae Folium) (3 qian).
Grind the above ingredients into a coarse powder, divide into four doses. Soak overnight in three zhong of water, decoct until half a zhong remains; for the dregs, add two zhong of water and decoct until one zhong remains. Place the medicine at the bedside, and take in divided doses while cold during the fifth watch (around 3-5 AM). In winter, take slightly warm, followed by eating some biscuit-like food to settle the stomach; if black stool is passed at dawn, the patient recovers.
Jiawei Ermiao Wan (Modified Two Marvels Pills): For damp-Heat affecting both feet, causing burning pain and swelling that gradually spreads to the groin and abdomen, or for recurrent Hong Kong Foot.
Cangzhu (Atractylodis Rhizoma) (4 liang), Huangbai (Phellodendri Cortex fried with wine) (2 liang), Niuxi (Achyranthis Bidentatae Radix), Fangji (Stephaniae Tetrandrae Radix), Danggui (Angelicae Sinensis Radix) (all soaked in wine), Bibai (Dioscoreae Hypoglaucae Rhizoma) (from Sichuan), Guiban (Testudinis Carapax et Plastrum roasted with酥 and fried with wine), Shudi (Rehmanniae Radix Preparata pounded) (each 1 liang).
Grind into powder, make pills with wine paste the size of tung seeds. Take 100 pills on an empty stomach with salted wine.
Danggui Nian Tong Tang (Angelica Decoction for Relieving Pain): Most effective for Damp-Heat Hong Kong Foot, as well as joint swelling and pain, and severe pain throughout the body (see Body Pain section).
Simple Formula: Definitely effective for Hong Kong Foot.
Cangzhu (Atractylodis Rhizoma processed), Huangbai (Phellodendri Cortex processed) (each 4 qian). Decoct in water and take on an empty stomach. Externally, apply hot-warmed salt compresses, or wash/soak with warm urine. Eat more boiled white radish to regulate qi.
Hong Kong Foot Rushing to the Heart: Baijian (Alumen) 3 liang, use Jiangshui (fermented rice water) 1.5 dou, boil several times, and soak the feet. This is highly effective.
