Yellow Emperor asked: There are cases of disease characterized by anger and mania. How does this disease arise? (Anger and mania refer to frequent anger and mental derangement, specifically cursing without distinction between relatives and strangers.) Qibo replied: It arises from Yang. The Yellow Emperor asked: How can Yang cause a person to become manic? Qibo said: When the Qi of Yang is suddenly frustrated and difficult to resolve, it leads to irritability; this disease is named Yang Jue (Yang reversal). (The Qi of Yang should be smooth and unobstructed. If suddenly frustrated, the will cannot be expressed; or if matters are difficult to decide, Yang Qi is suppressed and rebels upward, leading to anger and mania, hence the name Yang Jue.) The Yellow Emperor asked: How do we know this? Qibo said: The Maiyang channel (Stomach Meridian) usually has pulsation, whereas the Juyang (Taiyang) and Shaoyang channels do not. If the latter pulse rapidly where they normally do not, this is the sign. (By "Maiyang usually pulses," it refers to acupoints such as Xiaguan, Dicang, Daying, Renying, Qichong, and Chongyang, where arterial pulses are constantly felt. By "Juyang and Shaoyang do not pulse," it refers to Weizhong and Kunlun for Juyang, and Tinghui and Xuanzhong for Shaoyang; their pulses are slight and not prominent. If these areas show vigorous pulsation, the pulsation in the normally active Maiyang channel would be even more intense, which is the sign of Yang Jue with anger and mania.) The Yellow Emperor asked: How should it be treated? Qibo said: Depriving them of food will cure it. Food enters the Yin and augments Yang Qi; therefore, depriving them of food cures it. (The five flavors enter and are transformed by the Spleen, representing food entering Yin. Stored in the Stomach to nourish the Qi of the Zang-Fu organs, it augments Yang. Reducing food intake reduces Qi; thus, restricting food prevents Stomach Fire from assisting Yang pathogenic factors, allowing Yang Jue and mania to subside.) Administer a decoction made of Shengtie Luo (Iron Filings). Shengtie Luo has the effect of rapidly descending Qi. (Shengtie Luo refers to iron filings hammered off during smelting, which can be soaked in water and drunk. It belongs to Metal, is cold and heavy in nature, and is most effective at suppressing heat, opening stagnation, and pacifying Wood and Fire pathogenic factors. Thus, it descends rebellious Qi and eliminates mania. Medicines using Tie Jing, Tie Hua Fen, Zhen Sha, or Iron Rust Water share this intent. Detailed meanings of Dian Kuang are found in Section 37 of the Acupuncture category and should be cross-referenced here.)
