Zhang, a middle-aged man, suffered from frequent recurrences of epilepsy. Before an attack, he would inevitably become angry due to external events, losing control of his emotions uncontrollably. During an attack, he would suddenly fall and lose consciousness, bite his tongue causing ulceration, experience fright and convulsions, have loud phlegm rales in the throat, and suffer from urinary and fecal incontinence. This condition stems from a constitutional deficiency of Kidney Yin, leading to hyperactivity of Liver Yang. Phlegm rises with fire, obstructing the Pericardium (Pericardium), hence he wakes up quickly but regains consciousness slowly, and attacks are frequent. Treating the acute stage by addressing the branch symptoms, use Qianhu (Peucedani Radix), Qingpi (Aurantii Fructus Immaturus Pericarpium), Chuanbei (Fritillariae Cirrhosae Bulbus), Lianqiao (Forsythiae Fructus), Gouteng (Uncariae Ramulus cum Uncis), Zhuli (Bambusae Sap), Shichangpu (Acori Tatarinowii Rhizoma), and Shanzhi (Gardeniae Fructus). Decoct with Mingfan (Alum) water. After two doses, all symptoms subsided, and mental clarity was restored. Subsequently, administer Wan (Pill) formula for tonifying the Kidney and calming the Liver. When attacks became infrequent, continue long-term administration of the following pill formula: Fushen (Poria with Hostwood), Lingyangjiao (Saigae Tataricae Cornu), Danning (Arisaematis Cum Bile), Tianzhuhuang (Calami Sibirici Rhizoma), Yujin (Curcumae Longae Radix), Chuanbei (Fritillariae Cirrhosae Bulbus), Lianzixin (Nelumbinis Plumula), Xiniuhuang (Bovis Calculus), Zhixin (Gardeniae Cor), drip pills with Mingfan (Alum) water, coat with Zhusha (Cinnabaris). The patient recovered after taking this.
Zhao, since childhood, suffered repeatedly from Yin Epilepsy with falls. Usually after the first watch of the night, he would shout loudly with excessive salivation, and only wake up after prolonged convulsions. This indicates that Wind, Fire, and Phlegm are mutually stirring; clearly, the Foot Shaoyang Gallbladder Channel and Hand Jueyin Pericardium Channel are affected. The primary treatment is to extinguish Wind and Fire, assisted by resolving Phlegm. Use Lingyangjiao (Saigae Tataricae Cornu), Xian Shichangpu (Fresh Acori Tatarinowii Rhizoma), Shanzhi (Gardeniae Fructus), Gouteng (Uncariae Ramulus cum Uncis), Danning (Arisaematis Cum Bile), Juhong (Citri Reticulatae Pericarpium), Fangfeng (Saposhnikoviae Radix), Qianhu (Peucedani Radix), Zhuli (Bambusae Sap). After several doses, attacks will surely decrease. However, for a chronic illness lasting years, it is necessary to tonify the root. Use Luzhen (Codonopsis Radix), Mian Huangqi (Astragali Radix), Fushen (Poria with Hostwood), Zhi Cao (Glycyrrhizae Radix et Rhizoma Preparata), Shanyao (Dioscoreae Rhizoma), Beimu (Fritillariae Bulbus), Shuditan (Rehmanniae Radix Preparata Carbonized), Danggui (Angelicae Sinensis Radix), Baishao (Paeoniae Radix Alba). Grind into powder and take, mixed with Jingmi xie (Rice bran/powder), to ensure the Spleen Yuan is robust and vigorous. Administer Baolong Wan (Pill) intermittently to avoid the burden of stubborn disease.
Zhetong (Palpitations with Anxiety) refers to the heart beating uneasily, feeling frightened without external stimuli, with involuntary palpitations in the chest. Jing (Fright) refers to the loss of Shen (Spirit) stability, caused by shock from sensory experiences, occurring suddenly upon being startled. Kong (Fear) refers to gallbladder deficiency with trembling thighs, as if being pursued, where fear persists for a long time and is hard to release. Zhetong damages Heart Shen, Jing damages Gallbladder fluid, and Kong damages Kidney Essence; these three conditions involve pathologies of the Heart, Gallbladder, Liver, and Kidney. Fear is more severe than Fright; prolonged Fright transforms into Zhetong. Deficiency of the Heart and Gallbladder invariably stems from deficiency of Kidney Essence. Ancients stated that Yang is governed by Yin, and the Heart originates from the Kidney. Instability above and below arises from issues below (Kidney); Heart Qi deficiency is due to Essence deficiency. This reflects the principle of mutual rooting of Qi and Essence, and mutual assistance between Jun (Monarch) and Xiang (Minister) Fires, which is indeed correct. However, Zhetong and Jing/Kong, along with Sadness, Worry, and Anger, are all diseases of emotional states. If patients do not moderate labor and desires, nourish the Shen, and strengthen the Gallbladder, the root of the disease is difficult to eradicate. Physicians must carefully examine the condition and regulate it accordingly. If Heart and Spleen Qi and Blood are constitutionally deficient, causing Zhetong and Jing/Kong, or if sudden great fright causes confusion of Shen and Zhi, use Qifu Yin (Seven Blessings Decoction); for severe cases, use Dabu Yuan Jian (Great Tonification of the Origin Decoction). If Kidney Water is deficient, with insufficient True Yin causing Zhetong, use Zuogui Yin (Left Restoring Decoction). If Mingmen Fire is declining, with insufficient True Yang causing Zhetong, use Yougui Yin (Right Restoring Decoction). If Essence and Blood of the Three Yin channels are depleted, with insufficient Yang within Yin causing Zhetong and Jing/Kong, use Daying Jian (Great Nourishing Decoction) or Liyin Jian (Regulating Yin Decoction). If Water deficiency leads to Fire hyperactivity, causing irritability, heat, thirst, and Zhetong/Jingji (Palpitations with Startle), use Er Yin Jian (Two Yin Decoction) or Jiayi Yiyin Jian (Modified One Yin Decoction). If Overthinking and depression damage Ying (Nutrient) Qi of the Heart, causing Zhetong and Jingji, use Xiaoyao San (Free and Easy Wanderer Powder) or Yiying Jian (Nourishing Ying Decoction). If Phlegm-Fire is abundant, causing Zhetong in the epigastrium, use Wendan Tang (Warm the Gallbladder Decoction) plus Chao Huanglian (Stir-fried Coptidis Rhizoma), Shanzhi (Gardeniae Fructus), Danggui (Angelicae Sinensis Radix), Beimu (Fritillariae Bulbus). If Cold-Phlegm stagnates in the epigastrium causing Zhetong, use Jiangshu Tang (Ginger and Atractylodes Decoction). If Phlegm confuses the Heart orifices causing Jingji, use Wendan Tang (Warm the Gallbladder Decoction); for severe cases, use Zhusha Xiaotan Yin (Cinnabar Resolving Phlegm Decoction). These are the treatment methods of Zhang Jingyue.
