Cough is caused by the accumulation of phlegm and saliva, where external wind-cold pathogens and heat-toxin are constrained and cannot be dispersed, leading to the obstruction of the lung orifices. Initially, Hejie San (Harmonizing Powder) should be used; if there is excessive phlegm after the exterior syndrome resolves, Chuanbei (Fritillariae Bulbus), Dansing (Arisaematis Cum Bile), Maidong (Ophiopogonis Radix), or Qingfei Yin (Lung-Clearing Decoction) may be added. Cough results from the upward rush of pathogenic factors and stagnant phlegm that fails to circulate. The treatment of cough should prioritize resolving phlegm, while the root of treating phlegm lies in regulating Qi. Using Zhike (Aurantii Fructus) and Juhong (Citri Reticulatae Pericarpium Rubrum) to promote Qi circulation will naturally eliminate phagmatic Qi and dissipate fluid retention. By clearing Lung Metal and resolving heat-toxin, the pathogenic fire will subside. Body fluids nourish essence and blood; when scorched by fire-toxin, they fail to circulate normally and transform into sticky phlegm obstructing the interior. This blocks righteous Qi in the chest and diaphragm, causing wheezing or cough, tongue stiffness with inability to speak, sounds in the throat, difficulty in milk secretion, or immediate coughing and vomiting upon ingestion of milk. This indicates the stagnation of toxic vapors in the Lung and Stomach. Treatment should involve raising and dispersing while resolving toxicity, using Qianhu (Peucedani Radix), Jiegeng (Platycodonis Radix), Fuhua (Inulae Flos), Zhike, Juhong, and Dalili (Magnoliae Flos or specific local herb, here likely referring to a tonic herb like Millettia speciosa or similar contextually) to mobilize phlegm. If phlegm already exists in the neck, do not treat it recklessly; simply using raw Dalili can help move it.
Hiccups refer to the sensation of Qi obstruction. It arises from the withering of Stomach Yin, caused by toxic fire from the Lower Jiao郁结 (constrained) in the Stomach without dispersion, rather than being due to Yin fire ascending along the Stomach channel. Treatment should employ Shigao (Gypsum Fibrosum), Fuling (Poria), Maidong, Juhong, Xianhu (Dendrobii Caulis), Shanzhi (Gardeniae Fructus), Zhuru (Bambusae Caulis in Taenia), Jiutoulan, Moli Hua (Jasminis Flos), and Shushang Gan Tao.
Toxic pathogens lie dormant in the Kidney and transmit to the waist. The waist serves as the joint hub of the body. Scorching by toxic fire leads to the withering of Kidney Water, resulting in a lack of interaction between Water and Fire, thus causing lower back pain. In the initial stage, Baidu San (Detoxifying Powder) [containing Chuanqiang (Notopterygii Rhizoma et Radix), Duhuo (Angelicae Pubescentis Radix), Qianhu, Chaihu (Bupleuri Radix), Chuanxiong (Chuanxiong Rhizoma), Jiegeng, Shengma (Cimicifugae Rhizoma), Gange (Puerariae Radix), Gancao (Glycyrrhizae Radix et Rhizoma), Qingpi (Tangerinae Pericarpium Viride)] should be used. If accompanied by external wind-cold and headache, this same formula is applicable. Pain in both hypochondria indicates residual toxins remain within, preventing the normal ascent and descent of Yin and Yang Qi. Treatment should utilize Chaihu, Zhike, Jiegeng, and Chi Fuling (Poria cum Rubro) to facilitate the flow of Qi through urination and defecation, thereby stopping the pain.
