Zi Hezi treated a woman who, on the sixth day postpartum, suffered from Blood Confusion (coma caused by blood stasis obstructing the mind). He administered Liangge San (Cooling the Chest Powder) 2 liang, Siwu Tang (Four Substances Decoction) 3 liang, and Puxiao (Mirabilite) 1 liang, mixed as a single dose. The patient passed large amounts of dark purple-black bloody fluid, and remains robust and healthy to this day. (Inferring from the end note, it is known that she was naturally robust, hence such potent medicine could be used.)
Chai Yuqing treated the daughter-in-law of Official Li Fukian, who had suffered a miscarriage (partial abortion), with profuse sweating, dizziness, and coma (this is Blood Confusion). Other physicians' prescriptions were deemed too risky to take. Li walked to invite Chai for consultation. First, he instructed someone to pound one jin of leek leaves into a paste, stir-fry them with good vinegar, and use the hot fumes to fumigate the nose. After slight recovery, Qinghun San (Clear the Soul Powder) was given, taken with child's urine and yellow wine. She recovered after ten days of conditioning.
Xue Lizhai treated a woman who, due to drinking alcohol postpartum, experienced excessive lochia and suffered from Blood Fainting. Her breath smelled of alcohol; this indicated that the heat from alcohol caused the blood to move recklessly, leading to faintness due to deficiency. He prescribed Foshou San (Buddha's Hand Powder) with two qian of baked Kudzu Root (Gegen), and she recovered after two doses. Alcohol has a fierce nature; it should not be consumed during pregnancy or postpartum, lest it cause such symptoms. A crowded, hot, and noisy delivery room can also induce this condition. (Wu Xiong notes: This condition requires clearing blood and guiding out stasis; Chuanxiong and Danggui should be used cautiously. Although Ge Gen resolves alcohol toxicity, its ascending and dispersing nature is questionable. Claiming recovery in one dose is hard to fully believe.)
Master Mei treated residual blood remaining postpartum that rushed upward, causing chest oppression, abdominal pain. He gave her two sheng of lotus root juice, which cured her.
Xue Lizhai treated a parturient suffering from lochia attacking upward, resulting in stupor, locked jaw, incessant cold sweat, and cold extremities. He used Liujunzi Tang (Six Gentlemen Decoction) with one qian of Prepared Aconite Root (Fuzi) to restore Yang. She revived instantly after two doses. Then, Shiquan Dabu Tang (Ten Significant Tonifying Decoction) was used to nourish Qi and Blood, ensuring stability.
Another parturient with similar symptoms dared not touch her abdomen. One dose of Shixiao San (Pain-Dispersing Powder) expelled blood stasis, curing her. The next day, she had abdominal pain and diarrhea, which settled after treatment with Shiquan Dabu Tang.
Yet another parturient took Rhubarb (Dahuang) and other drugs. Although blood was discharged, she subsequently developed headache, fever, and aversion to cold. The next day, she fell into stupor, guarding her abdomen tightly with both hands, making pulse diagnosis impossible. Her complexion was greenish-white, indicating Spleen deficiency-cold causing pain. Liujunzi Tang with Ginger (Shengjiang) and Cinnamon Twig (Guizhi) stopped the pain. Bazen San (Eight Treasures Powder) with Ginger and Cinnamon Twig then conditioned her to stability.
Wang Zhizhong stated: Postpartum Blood Fainting, alternating chills and fever, or Blood Snatching Heart (blood rushing to the heart), are malignant diseases. I read *Shipin Bencao* (Compendium of Food Materials) and saw cases where deer horn powder mixed with wine, taken several times daily, proved effective. When a woman in my household suffered this, I had her take it with miraculous effect. I taught others to do the same, all verified. However, since alcohol is unsuitable postpartum, I advised using child's urine instead. Avoid purgative medicines. (From *Zisheng Jing*.)
Shen Yaofeng said: For postpartum excessive blood loss causing dizziness and fainting, heavy doses of Donkey-hide Gelatin (Ejiao) dissolved in water with a little child's urine are suitable. If blood loss is not significant, Duoming San (Life-Restoring Powder) is appropriate: De-oiled Myrrh (Moyao) 2 qian, Dragon's Blood (Xuexie) 1 qian, ground together, divided into two doses, taken with sugar-water and wine. A certain surnamed woman fainted postpartum and remained unconscious for two days. Lochia was scanty and ceased during the fainting spell. Her husband obtained Hupo San (Amber Powder) from neighbors, about 2 qian, mixed with wine and forced down, waking her immediately. The drug's color and aroma resembled Myrrh, likely being this formula.
A surnamed Lü woman suffered Blood Fainting the day after delivery. She woke briefly, then closed her eyes and tilted her head, relapsing dozens of times daily. Lochia was abundant at birth and continued. Her pulse was large, with a wiry and stiff left Guan position. One liang of Ejiao melted in wine, mixed with child's urine, was administered. That night, fainting decreased slightly, but head sweating occurred, and abdominal pain with a palpable mass appeared, followed by chills like malaria. After shaking, fever intensified. Duoming San with Myrrh and Dragon's Blood, 2 qian, mixed with wine, was given. Chills, fever, abdominal pain, and fainting vanished instantly. Only generalized sweating remained, indicating Qi and Blood circulation restored but revealing deficiency. Huangqi (Astragalus Root) 5 qian, fried Danggui Shen (Angelica Sinensis Root Heart) 2 qian, Gancao (Licorice Root) 1 qian, fried Suanzaoren (Ziziphus Seed) 3 qian, fried Xiaomai (Wheat Grain) 5 qian, and three Dazao (Jujube) were decocted and taken. Sweating stopped, and she stabilized.
Wu Xiong notes: Even if lochia is scanty, if there is no distress in the chest or abdomen, one must not indiscriminately administer blood-breaking herbs. This autumn, Mrs. Zhou Heting, newly delivered, suffered dizziness, spontaneous sweating, reluctance to speak, and inability to open her eyes. Her pulse was deficient, wiry, floating, and large. Inquiry revealed no lochia, yet no abdominal discomfort. Prescribing Ouli (Oyster Shell), Yingshi (Quartz), Biejia (Turtle Shell), Hupo (Amber), Danshen (Salvia Root), Gancao, Hongzao (Red Jujube), and Xiaomai reduced symptoms after one cup, and she recovered in days. This was likely inherent Blood deficiency; delivery drained Ying-Yin, leaving Yang unanchored. Adhering rigidly to the common rule of postpartum blood stasis rushing upward, without careful pulse-symptom analysis, would have been fatal.
