Whenever pregnancy occurs, the menstrual blood is originally retained to nourish the fetus and stored to transform into milk. If the Qi of the Chong (Penetrating Vessel) and Ren (Conception Vessel) is deficient and cannot constrain the blood, causing menstruation to occur monthly, this is named "Bao Lou" (Uterine Leakage). If the blood is exhausted, the fetus will die.
However, there are pregnant women with abundant blood whose menstruation continues normally while the fetus remains stable; this is colloquially called "Gou'er Tai" (Dog Fetus). Treating this as uterine leakage will inevitably cause miscarriage. If not treated as such, the fetus may not miscarry. There are also cases where the pulse is slippery and rapid, without wind-heat symptoms, and the menstrual flow is normal but slightly less than before; this is due to a small fetus and excess maternal blood-Qi. By the third or fourth month, when the fetus grows larger and requires more nourishment, the menstruation will naturally stop. It is common now to see women giving birth at seven or eight months; people consider the cessation of menstruation as the standard for full term, calling it premature, but conception actually occurred before menstruation stopped, making it full term by then, though people do not realize this. There are also robust pregnant women who have only spotting monthly; if there are no symptoms of lower back soreness or fetal movement, medication is unnecessary. This is because their blood-Qi is strong, and menstruation will naturally cease after the fourth or fifth month. If a pregnant woman is weak and emaciated, with frequent lower back soreness and fetal movement, accompanied by monthly spotting or continuous bleeding, this is not excess blood but "Tai Lou" (Fetal Leakage), and "Jiawei Buzhong Antai Yin" (Modified Decoction to Tonify the Middle and Calm the Fetus) should be taken. It is said that fetal leakage is mostly caused by blood-heat, but there are also cases of Qi deficiency and blood insufficiency where taking cooling herbs worsens the bleeding and leads to poor appetite and fatigue; this is Spleen Qi deficiency failing to control the blood, and "Guipi Tang" (Restore the Spleen Decoction) or similar formulas should be modified and used, determined by pulse diagnosis.
Miscarriage diseases often occur in the third, fifth, and seventh months. If a previous miscarriage occurred at three months, the next one will likely happen at the same time due to deficiency, so "Zunsheng Antai Yin" (Decoction for Preserving Life and Calming the Fetus) should be taken in advance, or "Taiyuan Yin" (Fetal Origin Decoction) modified, or formulas like "Taishan Panshi San" (Mount Tai Porphyry Powder) and "Antai Wanquan Shenyingsan" (Perfect and Divine Efficacy Powder for Calming the Fetus) chosen according to symptoms for prevention. They must be taken regularly; after the seventh month, there will be no worries. One must also abstain from sexual intercourse, anger, overwork, and fried/roasted foods. Some miscarry within one month of conception; people are unaware, thinking it is just a period, not realizing a silent miscarriage has occurred. After menstruation ends and conception happens through initial intercourse, rest and abstinence are most important. Subsequent intercourse disturbs the uterus, leading to miscarriage at one, three, or five months. Consider that donkeys and horses protect their fetuses; the male kicks away intruders, hence no miscarriage. Should humans not be cautious? Labor, anger, lifting heavy objects, and washing the lower body (opening the orifices) can also cause miscarriage. Many childless people suffer from one-month miscarriages, not because they cannot conceive. For trauma causing fetal disturbance, "Jiao'ai Antai San" (Gelatin and Mugwort Powder for Calming the Fetus) is suitable. Before three or four months, when the fetus is unformed, it is called "Duotai" (Fallen Fetus). After five or six months, when formed, it is called "Banchan" (Half-birth). Colloquially all are called "Xiaochan" (Minor Birth), generally belonging to maternal Qi and Blood deficiency and unstable fetal origin. Qi deficiency fails to hold, Blood weakness fails to irrigate, often leading to minor birth. The Kidney connects to the uterus, and the waist is the residence of the Kidney; lower back pain indicates risk of miscarriage, which must be prevented. To treat fetal leakage, one should pre-nourish the root using "Qianjin Baoyun Wan" (Thousand Gold Pregnancy Preservation Pill). "Zhilou Jueshen Dan" (Divine Medicine to Stop Leakage and Cut Off [Bleeding]) is also very effective. If a pregnant woman suddenly flows large amounts of water, the fetus will definitely miscarry. If the flow is gradual, Qi-tonifying and fetal-calming medicines can be used. Yellow discharge like bean juice with fetal movement and abdominal pain indicates Liver and Spleen Damp-Heat; "Shengyang Chushi Tang" (Raise Yang and Remove Dampness Decoction) is used. *Daquan* treats this with one tael of Huangqin (Scutellariae Radix), one qian of Chuanxiong, and one he of glutinous rice decocted. Note that minor birth differs from major birth: major birth is like a ripe fruit falling naturally; minor birth is like picking it raw, breaking the shell and cutting the root. Negligence causes many diseases and deaths. This condition starts with condensing blood to form the fetus, continues with Jing-Blood nourishing it, and ends with Jing-Blood insufficiency causing withering and miscarriage, so stasis is minimal. If there is abdominal pain forming hard masses, it belongs to Blood deficiency and Qi rebellion; first use "Shenghua Tang" (Generate the New Decoction) to remove stasis and generate new blood, followed by strong warming tonification. Knowing only to resolve stasis and break stagnation makes rebellious Qi rise further, often leading to death. Beware! After three months of pregnancy, if the Chi (Cun-Cord) pulse is choppy or weak/faint, the fetus is unstable; only a surging pulse ensures the fetus does not miscarry. Thus, *Maijue* says: "Pulse string-taut, firm, slippery, and smooth indicates safety; deep, fine, and faint leads to the spring of death." This is exactly what is meant. Although half-births are due to Qi and Blood insufficiency, constitutions vary (Yin/Yang bias, Cold/Heat), so treatment must rely on pulse differentiation. For Yin deficiency with internal heat, using warming agents like Aijie (Moxa Leaf), Fuzi (Aconite Root), Baizhu (Atractylodis Macrocephalae Rhizoma), and Sharen (Amomi Fructus) consumes Yin further, like plants lacking rain, naturally withering. For Yang deficiency with internal cold, using cooling agents like Huangqin (Scutellariae Radix) and Shaoyao (Paeoniae Radix) damages Spleen-Stomach Yang, weakening Qi and Blood, like fruits failing to set in autumn/winter. In general, before three months, nourish Spleen and Stomach; after four months, strengthen Waist and Kidneys, tonify Blood, smooth Qi, and assist with clearing Heat. This is the general principle.
**Jiawei Buzhong Antai Yin**: For pregnant women with robust Yuan Qi who still have slight bleeding after conception due to abundant blood-Qi. One and a half zhong of water, decoct to seven parts, take far from meals. Once pregnancy is suspected, take one dose every three-five days; after four months, safety is assured. If injury to five-seven month fetus, take after passing seven months for best effect. Xu Donggao said: Miscarriage in two-three months is called Xiaochan. Caused by weak constitution, dual Qi/Blood deficiency, excess organ fire, and Heat in Blood branch. Doctors say calming fetus requires warming agents like Aijie, Fuzi, Sharen, which increases disaster and accelerates miscarriage. Unknowingly, clear and harmonious Qi/Blood without Fire frying ensures fetal stability. Qi deficiency cannot hold, Blood Heat causes reckless overflow; how can miscarriage be avoided? Xiangfu moves Qi and opens depression, but overuse damages Righteous Qi; Sharen moves Spleen Qi, but overuse consumes Righteous Qi. Moreover, aromatic-dry substances damage Qi/Blood; seeking to calm fetus, they instead damage it and cause miscarriage. Only Taishan Panshi San and Qianjin Baoyun Wan possess the wonder of creation, 100% effective, foolproof, thus presented for benevolent gentlemen to know.
