(Referring to constipation)
Postpartum tidal fever, sweating, and constipation should not be exclusively diagnosed as Yangming disease (Yangming syndrome). Dry mouth and dry throat should not be exclusively diagnosed as Shaoyin disease (Shaoyin syndrome). Abdominal fullness, dry mouth, and severe constipation should not be exclusively diagnosed as Taiyin disease (Taiyin syndrome). Furthermore, excessive sweating and delirium should not be exclusively attributed to dry feces in the intestines and stomach. These symptoms are mostly caused by fatigue and overwork damaging the Spleen Qi, leading to slightly delayed transformation and transportation functions, and depletion of Qi and Blood. The treatment should favor Yangzheng Tongyou Decoction (Nourish Righteousness and Unblock the Obstruction Decoction). Do not adhere to heterodox methods or recklessly use Chengqi Decoctions (Gut-Clearing Formulas), lest it leads to irreversible damage with dry lips.
Commentary: This discussion deserves careful study; it reflects the principle stated in the book that postpartum conditions should primarily be treated by greatly tonifying Qi and Blood, while other symptoms are treated secondarily. It also embodies the principle that "sweet-warm flavors can eliminate high fever."
Yangzheng Tongyou Decoction (also known as Zhuxue Runchang Tang / Blood-Assisting and Intestine-Moistening Decoction)
Chuanxiong Rhizoma (2.5 qian) Zhi Gancao (Prepared Licorice Root, 0.5 qian) Tao Ren (Peach Seed, 5 grains) Rongzhuang (Cistanche Stem, 5 qian) Chenpi (Aged Tangerine Peel, 0.4 qian) Huma Ren (Hemp Seed, 3 qian)
Decoct in water and take warm. Adjustments for other symptoms are as described previously. In cases of excessive sweating, delirium, and solid stool, this indicates a deficiency of both Qi and Blood, decline of Shen (Spirit), and loss of control by the Heart Master. Urgent treatment should focus on nourishing Ying (Nutritive) Qi and calming the Spirit. Add Fuling (Poria), Zaoren (Sour Jujube Seed), Baizi Ren (Biota Seed), Renshen (Ginseng), [missing character], and Baizhu (Atractylodes Macrocephala Rhizome), each 1 qian. Decoct in water and take. If constipation persists for more than ten days with severe dryness and obstruction, dry feces must be present at the anus. Use honey suppositories, or introduce powder of Gengjiao (Gleditsia Sinensis Fruit) or pig bile along with dried salt as a laxative enema. Be cautious not to mistakenly use purgatives, which may cause incessant diarrhea or lead to deficiency-type closure, resulting in abdominal distension, causing significant harm. Immediately after delivery, one must first take several doses of Huasheng Tang (Life-Restoring Decoction) to ensure that symptoms of blood dryness and intestinal withering do not occur.
Within seven days postpartum, fever, headache, and aversion to cold should not be exclusively diagnosed as Taiyang disease (Taiyang syndrome). Fever, headache, and hypochondriac pain should not be exclusively diagnosed as Shaoyang disease (Shaoyang syndrome). Both conditions arise from deficiency of both Qi and Blood and disharmony of Yin and Yang, mimicking exterior pathogens. One must absolutely avoid using Mahuang Decoction (Ephedra Decoction) to treat Taiyang-like syndromes, or Chaihu Decoction (Bupleurum Decoction) to treat Shaoyang-like syndromes.
Because sweating after blood loss further depletes Yang, it leads to exhaustion of both Yin and Yang, with disastrous consequences. Even if there is a genuine contraction of Cold Damage (Shanghan), the Chuanxiong and Jiangtan (Charred Ginger) in the Jiawei Huasheng Tang can disperse cold, and once Ying and Wei (Defensive) Qi are harmonized, various diseases will recede naturally.
Jiawei Huasheng Tang (Modified Life-Restoring Decoction)
Chuanxiong Rhizoma (2 qian) Danggui (Angelica Sinensis Root, 2 qian) Jiangtan (Charred Ginger, 0.5 qian) Zhi Gancao (Prepared Licorice Root, 0.5 qian) Tao Ren (Peach Seed, 10 grains) Chenpi (Aged Tangerine Peel, 0.3 qian) Dazao (Jujube Fruit, 2 pieces)
Decoct in water and take warm. Administer two doses consecutively. If it is known to be a wind-pathogen invasion and symptoms persist after two doses, add Qianghuo (Notopterygium Root, 0.4 qian) and Congbai (Scallion White, 2 cun).
