Chills: If caused by damp-phlegm stagnating in the middle jiao, with a deep and slow pulse, yang qi is suppressed and cannot diffuse outward, resulting in chills. It is appropriate to induce vomiting using Jiang tea (River Tea) mixed with sesame oil and ginger juice to expel phlegm, followed by Tongjing San (Unblocking Meridian Powder) minus Ephedrae Herba (Mahuang), Mirabilite (Mangxiao), and Rhubarb (Dahuang), plus Angelicae Sinensis Radix (Danggui) and Rehmanniae Radix (Dihuang). Hidden Pulse: If heat is extreme and blood is deficient, chills may also occur. With a deep and choppy pulse, it is appropriate to use Siwu Tang (Four Substances Decoction) with doubled Rehmanniae Radix, plus Atractylodis Macrocephalae Rhizoma (Baizhu), Cinnamomi Ramulus (Rougui), Phellodendri Cortex (Huangbai), Ginseng Radix et Rhizoma (Renshen), and Licorice Root (Gancao).
Trembling with Fever: When one yang pathogen strikes, it manifests as shortness of breath, frequent coughing, and diarrhea; its transmission leads to Xinche (cardiac pulling sensation). "Che" means movement. Due to mother-son transmission, diarrhea occurs, treated primarily with Lizhong Tang (Regulate the Middle Decoction).
Labor Wind: The disease location is typically below the lung, causing neck stiffness and blurred vision. Labor generates heat, leading to expectoration resembling mucus; contracting wind causes aversion to wind and trembling chills. The Lung governs skin and hair; it is appropriate to use Tongjing San plus Pinelliae Tuber (Banxia) and Angelicae Sinensis Radix.
Bi Qi (Obstruction Qi): This results from excessive yin qi and blood failing to nourish, causing the body to feel cold as if in water, all belonging to deficiency-cold patterns. Dried Ginger (Shengjiang) and Prepared Aconite Root (Fuzi) are appropriate.
Five Excesses and Five Deficiencies: Full pulse and thin pulse correspond to the Heart; hot skin and cold skin correspond to the Lung; abdominal distension and inability to eat correspond to the Spleen; chest oppression, dizziness, and shortness of breath correspond to the Liver; blockage of both bowels or incontinence of both bowels corresponds to the Kidney.
Yin Stagnation in Yang: Caused by overexertion leading to cold, or drunkenness from alcohol, the next day brings epigastric pain resembling hunger. Overeating then leads to left hypochondriac pain with a mass, with a fine, choppy, deep, and rapid pulse. Consumption of leek juice, peach kernel, and child's urine can alleviate this. Similarly, for left mammary pain with a nodule, Shigao (Gypsum Fibrosum), Angelicae Dahuricae Radix (Baizhi), Puerariae Lobatae Radix (Ge Gen), Trichosanthis Radix (Gualou), and Vespa Nidus (Fengfang) are used.
Yang Stagnation in Yin: Dissatisfaction leading to profuse epistaxis, with a floating and rapid pulse, large and hollow upon heavy pressure, is treated with Siwu Tang plus Daylily Root (Xuan Cao) and ginger juice. Bleeding from the anus due to dietary irregularities during flight, with a deep, choppy, seemingly rapid pulse, is treated with Curcuma Longa Rhizome (Yujin), Ligusticum Chuanxiong Rhizome (Chuanxiong), Angelicae Dahuricae Radix, Atractylodis Rhizoma (Cangzhu), Paeoniae Radix Rubra (Shaoyao), Puerariae Lobatae Radix, and Cyperi Rhizoma (Xiangfu). The Right Kidney belongs to Fire, supplemented with Morinda Officinalis Root (Bajitian) and Eucommia Bark (Duzhong); the Left Kidney belongs to Water, supplemented with Rehmanniae Radix, Cornus Fruit (Shanzhuyu), and Phellodendri Cortex.
Liver: Epigastric pain centered behind the heart, extending upward to support both hypochondria, belongs to the Liver meridian. Obstruction of the diaphragm and throat with inability to swallow food indicates Earth deficiency. Severe cases involve tinnitus, vertigo, inability to see people, frequent rigidity and collapse, tenesmus, hypochondriac pain, vomiting, and diarrhea, leading to irritability. Deficiency manifests as blindness, deafness, fearfulness, feeling as if being hunted.
Heart: Heat in the chest, dry throat, chest fullness, skin pain, alternating chills and fever, cough, asthma, palpitations, mania, all blood disorders, chest pain, hypochondriac fullness, and pain between the sternum, back, and scapula. Deficiency manifests as abdominal distension, pain radiating from the hypochondrium to the lower back.
Spleen: Edema of the dorsum pedis and bone pain, Yin Bi (obstruction) with lumbar, spinal, head, and neck pain, difficult defecation, fluid retention with chest obstruction, cholera with vomiting and diarrhea, undigested stool with borborygmus, spleen heat governing deficiency.
Lung: Internal changes in joints, right hypochondriac pain, cold invading internally, loose stools, chest and hypochondriac fullness radiating to the lower abdomen, inability to turn sideways, dry throat, dusty complexion, male hernia, female lower abdominal pain. Excess manifests as cough, counterflow qi, shoulder and back pain; deficiency manifests as shortness of breath, inability to take deep breaths, deafness, and dry throat.
Kidney: Lumbar and leg pain, immobility of large joints, difficulty bending, abdominal distension and hardness, night sweats. Excess manifests as abdominal and calf swelling, heaviness; deficiency manifests as chest fullness, lower abdominal pain, and cold extremities.
