In all cases of jaundice, if the pulse at Cun Kou (Cun position) is imperceptible near the palm crease, and the mouth and nose are cold, it is incurable. If the pulse is deep, there is thirst with a desire to drink water, but urination is difficult; these conditions all lead to yellowing of the body. Abdominal fullness, a flaccid tongue with yellowish discoloration, restlessness, and inability to sleep indicate belonging to the category of jaundice.
The Master said: In cases of jaundice accompanied by fever, irritability, panting, chest fullness, and dry mouth, this results from the application of fire therapy to force sweating during the onset of the disease, causing external heat and internal heat to combine. However, jaundice generally arises from dampness. When the entire body is hot and yellow, with abdominal heat, the heat is located in the interior, and purgation should be employed.
The Master said: The course of jaundice is expected to last eighteen days. If treated for more than ten days, recovery may occur; if the condition worsens instead, it becomes difficult to treat.
It is also said: Jaundice accompanied by thirst is difficult to treat; jaundice without thirst is treatable. If the disease originates in the Yin aspect, vomiting will necessarily occur; if it originates in the Yang aspect, chills followed by fever will occur.
The Master said: For all patients with jaundice, promoting urination is sufficient. If the pulse is floating, release via sweating is indicated, suitable for Guizhi Jia Huangqi Tang (Cinnamon Twig Decoction plus Astragalus). Furthermore, for men presenting with yellow complexion but normal, free urination, Xiao Jianzhong Tang (Minor Construct the Middle Decoction) should be administered.
In jaundice with abdominal fullness, difficult and red urination, and spontaneous sweating, this indicates that the exterior is harmonized while the interior is excess and solid. Purgation is indicated, suitable for Dahuang Huangbai Zhizi Mangxiao Tang (Rhubarb, Phellodendron, Gardenia, and Mirabilite Decoction).
In jaundice where urine color remains unchanged, there is a desire for diarrhea, abdominal fullness, and wheezing, clearing heat is contraindicated; forcing heat clearance will inevitably cause hiccups. For hiccups, Xiaobanxia Tang (Minor Ginger and Pinellia Decoction) is the primary treatment.
Patients with Jiudan (Alcoholic Jaundice) invariably have difficult urination. Their signs include heat in the heart region and heat under the feet; this constitutes the syndrome.
Oppression and heat in the heart region, inability to eat, and occasional desire to vomit constitute the name Jiudan (Alcoholic Jaundice).
In Jiudan, some present without heat, remaining quiet and clear-minded, with abdominal fullness, desire to vomit, and dry nose. If the pulse is floating, induce vomiting first; if the pulse is deep and wiry, induce purgation first.
For Jiudan with heat in the heart region and desire to vomit, vomiting leads to immediate relief.
Jiudan presents with yellow complexion, heat knotting in the epigastrium causing irritability.
If Jiudan is treated with purgation, it may eventually transform into Heidan (Black Jaundice), characterized by blue-black eyes, black face, a sensation in the center like eating minced garlic, completely black stool, and skin numbness where scratching does not elicit pain. With a floating and weak pulse, although the complexion is black, a faint yellow hue reveals the diagnosis.
At Cun Kou, if the pulse is micro and weak: micro indicates aversion to cold; weak indicates fever. If sweating should have occurred but did not, joint pain results; if irritability should have occurred but did not, profuse sweating occurs. At Zu Yang, if the pulse is slow and late, stomach qi is excessively strong. At Shao Yin, if the pulse is micro, essence is damaged; yin qi becomes cold, Shao Yin is deficient, and grain qi becomes excessive. After eating, there is irritability and fullness; after fullness, fever arises. Exogenous heat digests grain, leading to hunger again after fever subsides. Heat causes abdominal fullness; micro pulse damages essence. Excessive grain qi leads to emaciation, named Gure Re (Grain Cold-Heat). In Yangming disease, a slow pulse means one cannot eat until full; eating until full causes irritability. Dizziness implies difficult urination, indicating impending Gu Dan (Grain Jaundice). Even if purged, abdominal fullness persists because the pulse is slow.
The Master said: At Cun Kou, if the pulse is floating and moderate: floating indicates wind; moderate indicates bi obstruction. This bi is not wind-stroke; limbs suffer from irritability. The spleen's color must appear yellow, and stagnant heat circulates. At Zu Yang, if the pulse is tight and rapid: rapid indicates heat, which digests grain; tight indicates cold, causing fullness immediately after eating. Floating Chi pulse indicates kidney injury; tight Zu Yang pulse indicates spleen injury. Wind and cold contend; eating causes dizziness, grain qi fails to digest, stomach suffers from turbidity, turbid qi flows downward, and urination stops. Yin is affected by cold, heat flows into the bladder, the whole body turns yellow, named Gu Dan (Grain Jaundice). Black forehead, slight sweating, heat in hands and feet, evening onset, bladder urgency, and free urination constitute Nv Lao Dan (Female Labor Jaundice). Abdominal distension resembling water accumulation is incurable. Jaundice patients who feel fever in the late afternoon but paradoxically feel aversion to cold have contracted Nv Lao Dan. Bladder urgency, lower abdominal fullness, whole-body yellowing, black forehead, and heat under feet result in Heidan (Black Jaundice). Abdominal distension like water, black stool, and occasional loose stools characterize Nv Lao disease, not water retention. Abdominal fullness is difficult to treat. Xiaoshi Fanshi San (Niter and Alum Powder) is the primary treatment.
Malaria pulses are naturally wiry. Wiry and rapid pulses indicate much heat; wiry and slow pulses indicate much cold. Wiry, small, and tight pulses can be purged; wiry and slow pulses respond to warming herbs. Tight and rapid pulses respond to sweating or acupuncture. Floating and large pulses respond to vomiting. Wiry and rapid pulses indicate wind-fire; dietary management stops them. Malaria forming masses is called Nuomu (Malaria Mother), treated with Biejia Jian Wan (Turtle Shell Decoction). Malaria presenting only with heat is Wen Malaria (Warm Malaria). The pulse is normal, the body has no chills only heat, joints are painful and irritable, occasional vomiting, resolving in the morning if starting in the evening, or vice versa, is named Wen Malaria, treated with Baihu Jia Guizhi Tang (White Tiger plus Cinnamon Twig Decoction). Malaria with predominant chills is Mu Malaria, treated with Shuqi San (Shuqi Powder).
