In general, if the eyes are bright and vision is clear, the condition is treatable. If the eyes are dim and unable to recognize people, or if the eyeballs roll upward, or stare fixedly, or remain rigid and round, or exhibit "dai yan" (upward deviation of the eyes) accompanied by opisthotonos, or if the eyelids are sunken, these are all untreatable conditions.
Those who keep their eyes open and wish to see others present a Yang syndrome; those who keep their eyes closed and do not wish to see others present a Yin syndrome.
A sensation of blurriness in the eyes and lack of coordination in gaze indicates excessive heat within the interior.
Eye pain belongs to Heat in the Yangming channel. Redness of the eyes also indicates severe heat.
Photophobia (aversion to light) will inevitably lead to epistaxis (nosebleed).
Yellowing of the sclera indicates that jaundice (body yellowing) is about to develop.
When a disease is about to resolve, one may observe yellowing of the canthi, brightness of the nasal tip, and luminosity at the shangen (root of the nose).
A bluish color of the nasal tip indicates abdominal pain with cold syndrome, which is a fatal prognosis. A slightly blackish color indicates water qi pathology. A yellowish color indicates difficulty in urination. A white color indicates Qi deficiency. A reddish color indicates Lung Heat. A bright and lustrous color indicates retained fluid retention (liu yin).
Dry nostrils belong to Heat in the Yangming channel and will inevitably lead to epistaxis. Dry nostrils with a black color resembling soot indicate severe deep-seated heat from Yang toxicity. Cold, slippery, and black nostrils indicate extreme cold from Yin toxicity.
Snoring during sleep indicates Wind-Warmth. Nasal congestion with turbid discharge indicates Wind-Heat. Flaring of the nostrils indicates Lung Wind or exhaustion of Lung Qi, which is untreatable.
Chapped and dry lips indicate Spleen Heat. Chapped lips that are red and lustrous are auspicious; chapped lips that are black are ominous. Swelling and redness of both lips and mouth indicate severe heat. Cyanosis and blackening of both lips and mouth indicate extreme cold. Bitter taste in the mouth indicates Gallbladder Heat. Sweet taste in the mouth indicates Spleen Heat. Dry mouth and throat indicate Kidney Heat. Dry tongue and mouth with desire to drink water indicate Heat in the Yangming channel. Trismus (lockjaw) and difficulty speaking indicate Jing-Feng (spasm wind).
Sores on the upper lip indicate Hu-Chong (roundworms) damaging the internal organs; sores on the lower lip indicate Huo-Chong damaging the anus. Conditions such as cyanotic lips, curled tongue, perioral cyanosis, blackening around the mouth, open mouth with straight airflow, fish-mouth appearance, continuous trembling of the lips, and non-returning breath are all untreatable.
Red and lustrous earlobes indicate life; yellow, white, black, or cyanotic and dry earlobes indicate death. Thin and white or thin and black earlobes both indicate failure of the Kidneys. Deafness and ear pain belong to Heat in the Shaoyang channel and are still treatable. However, deafness combined with a curled tongue and cyanotic lips belongs to the Jueyin channel and is difficult to treat.
A fresh red tongue is auspicious. A blue tongue indicates cold; blue and purple indicates Yin and cold. Red and purple indicates Yang and heat. Black indicates extreme hyperactivity and is difficult to treat. A white and slippery tongue coating indicates exterior cold. It is also said that this represents Heat in the Dantian and Cold in the chest. A yellow and dry tongue coating with thirst indicates abundant heat. A black and dry tongue coating with thirst indicates extreme and hyperactive heat. If there is no thirst, but the tongue has a black and slippery coating, it indicates Cold and Yin. A curled, scorched-black, and dry tongue indicates extreme heat from Yang toxicity. A blue tongue with a slippery coating indicates extreme cold from Yin toxicity. Tongue swelling, dry and cracked surface, and prickles on the tongue all indicate severe heat. Stiff, rigid, or shortened tongue, along with mental confusion and unclear speech, indicates death. In the case of Yin-Yang Yi disease, protrusion of the tongue for several cun indicates death. The tongue is the orifice of the Heart and belongs to Fire; a red color is auspicious. Only black indicates Water overcoming Fire, hence it is difficult to treat.
