[Da] Postpartum obstruction of both urination and defecation occurs because the intestines and stomach originally harbor heat pathogens. Due to the loss of blood and body fluids during childbirth, the body fluids become depleted and dry, causing qi stagnation and blockage in the intestines and stomach. Heat qi becomes knotted within the intestines and stomach, thus leading to the obstruction of both urination and defecation.
[Xue] I once treated a postpartum woman suffering from obstruction of both urination and defecation. Various medications were ineffective, and her condition was critical. I instructed her to drink cow's milk. After one day, her symptoms slightly improved, and she recovered completely after three days. Human milk is even more effective.
Taohua San (Peach Blossom Powder): Treats constipation of both urination and defecation caused by qi stagnation and blood stasis in the bladder.
Ingredients: Tao Ren (Persicae Semen), Kuizi (Abelmoschi Fructus), Huashi (Talcum), Binglang (Arecae Semen) (equal parts).
Usage: Grind into fine powder. Take 2 qian per dose, mixed with decoction of scallion white on an empty stomach.
Formula for treating postpartum difficulty in urination and defecation with bleeding:
Ingredients: Cheqianzi (Plantaginis Semen), Huangqin (Scutellariae Radix), Puhuang (Typhae Pollen), Muli (Concha Ostreae), Sheng Dihuang (Rehmanniae Radix), Shaoyao (Paeoniae Radix Alba) (each 1 liang 5 qian).
Usage: Grind into fine powder. Take one fangcun bi (a measure equivalent to a square inch spoonful) with rice water on an empty stomach. Avoid wheat products and garlic.
Tongqi San (Ventilation Qi Powder): Treats postpartum obstruction of both urination and defecation. The navel-sealing method can also be used. (See previous section on miscellaneous diseases: obstruction of defecation.)
[Xin] To treat a postpartum woman who suddenly presents with lower abdominal distension resembling a gu-poison swelling, and obstruction of both urination and defecation, the primary acupoints are Qihai (CV6), Zusanli (ST36), Guanyuan (CV4), Sanyinjiao (SP6), and YinGu (KI10).
[Xue] For postpartum fecal incontinence due to spleen and kidney deficiency, use Huanshao Dan (Revitalize Young Pill), primarily supported by Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). If accompanied by deficiency-cold, add Roudoukou (Myristicae Semen) and Buguzhi (Psoraleae Fructus), or use Sishen Wan (Four Spirits Pill). If there is spleen-kidney deficiency-cold, use Bawei Wan (Eight-Ingredient Pill) combined with Sishen Wan, supplemented by the aforementioned two formulas.
Note: Postpartum fecal incontinence is caused by insecurity of kidney qi; it should be treated with Wuweizi Wan (Schisandra Berry Pill). (Refer to Miscellaneous Diseases: Diarrhea.)
One postpartum patient had loose stools, poor appetite, and occasional morning fecal incontinence. This indicated deficiency-cold of middle qi and insufficiency of spleen and kidney. She recovered after taking Buzhong Yiqi Tang administered with Sishen Wan.
Another postpartum patient excreted feces through urine, a condition named "Jiaochang" (Intestinal Crossing). This resulted from dual deficiency of qi and blood, causing organs to lose their normal functional pathways. She recovered after taking Liujunzi Tang (Six Gentlemen Decoction) for two doses, followed by Wuling San (Five-Ingredient Powder with Poria) for two doses. This approach is also applicable to ordinary cases of intestinal crossing.
Guangji Formula for treating postpartum fecal incontinence:
Take grass from old swallow nests, burn it to ash, mix with wine, and take half a zhan (measure). This formula is also suitable for men.
Jiyan Fang (Collected Effective Prescriptions) for treating postpartum fecal incontinence. Also suitable for men.
Ingredients: Fanshi (Alumen, calcined), Muli (Concha Ostreae, calcined) (equal parts).
Usage: Grind into powder. Take one fangcun bi with wine, three times daily.
Buyi (Supplementary Additions) Formula for treating postpartum fecal incontinence without awareness of the urge:
Ingredients: Bailian (Ampelopsis Radix), Shaoyao (Paeoniae Radix Alba) (equal parts, ground into powder).
Usage: Take one fangcun bi with wine.
Buyi (Supplementary Additions) treats postpartum fecal incontinence without sensation; Guchang Wan (Intestine-Stabilizing Pill) may be taken. (Formula see previous section on miscellaneous diseases: diarrhea.)
Hua Tuo stated: If a patient lying in bed suffers from fecal incontinence without awareness, it indicates death.
