[Daquan] In some cases, after the fetus is delivered, the placenta (Baoyi) does not detach, a condition known in the world as "Xi Bao" (Stagnant Placenta). This occurs because of excessive exertion during labor; by the time the fetus is born, the mother's body is exhausted and unable to exert further force. During the interval before the placenta is expelled, external cold pathogens invade, causing blood vessels to become sluggish, thus preventing the placenta from being discharged. Immediate treatment with herbal medicine is required. This does not harm the infant, but the concern is that the placenta remains tightly connected to the umbilical cord. If the placenta is not expelled, the umbilical cord cannot be cut and the infant bathed in a timely manner; cold pathogens may injure the infant, leading to disease. The old method states that if the placenta does not descend, fearing injury to the infant, one should simply cut the umbilical cord according to protocol. The place of delivery must conform to the directions of the four seasons and avoid taboos related to the Five Elements; violations often lead to difficult labor.
Guojizhong discussed: What is the cause of retained placenta? The answer is: After the mother gives birth, blood flows into the placenta, distending it, which prevents it from descending. If treatment is delayed, abdominal distension and pain will occur, progressively ascending to strike the chest and heart, causing pain, panting, and urgency. One should administer Duoming Dan (Life-Restoring Pill) to expel the stagnant blood within the placenta. Once the blood disperses and distension subsides, the placenta will naturally descend. Niuxi Tang (Achyranthes Decoction) is also effective.
Xue Ji discussed: Some cases are due to lochia entering the placenta, causing distension and preventing expulsion; others are due to deficiency of Yuan Qi (Primal Qi), which fails to push the placenta out. In cases where lochia flows into the placenta, there is abdominal distension and pain; use Duoming Dan or Shixiao San (Pain-Relieving Powder) to resolve blood stasis. If treatment is delayed, rescue is impossible. In cases where Yuan Qi is insufficient to push, there is no abdominal distension or pain; use Baosheng Wuyou San (Preserve Life and Ensure Safety Powder) to tonify and secure Yuan Qi. Alternatively, apply one liang of castor bean paste to the sole of the mother's right foot; wash it off immediately after the placenta is expelled. If delayed, the rectum may prolapse. If the rectum does not retract, apply this paste to the vertex of the head, and the rectum will naturally retract. Yimu Wan (Motherwort Pill) is also effective. A relative's wife had retained placenta with chest and abdominal distension and pain, intolerable to touch; this was caused by blood stasis. Administer one dose of Shixiao San with hot wine, and both lochia and placenta were expelled. Another patient had retained placenta without abdominal distension or pain, but tenderness upon pressure; this was Qi deficiency failing to push. Use Wuyou San to expel the placenta. Regarding previous cases, I asked midwives who suggested taking Motherwort Pills or inducing vomiting by placing the mother's hair in her mouth, which would cause the placenta to descend spontaneously; failure to do so would result in death. The aforementioned methods are very effective. A postpartum woman presented with a red face, five-palm heat,敗血 (stagnant blood) entering the uterus, retained placenta, and cold sweats. Thinking that removing the stagnant blood would allow the placenta to descend naturally, I used two he of black beans, thoroughly fried, then quenched a red-hot iron scale weight in wine with the beans. The resulting bean-dripping wine was used to dissolve two pills of Yimu Dan (Motherwort Pill); the placenta was expelled with the blood, and other symptoms resolved completely.
Duoming Dan (Life-Restoring Pill)
Fuzi (Prepared Aconite Root, half liang, soaked) Mudanpi (Tree Peony Bark, one liang) Ganxi (Dry Resin, two qian and a half, crushed, stir-fried until smoke ceases)
Grind all into fine powder. Boil strong vinegar (one sheng) with Rhubarb powder (one liang) until a paste forms. Mix with the powder to form pills the size of sophora seeds. Swallow five to seven pills with warm wine at any time.
Huarui Shi San (Flower Manganese Stone Powder) Treats postpartum faintness of breath due to incomplete expulsion of stagnant blood, blood confusion and vertigo, evil blood rushing to the heart, fetal death in utero, retained placenta leading to death. As long as the heart area remains warm, urgently mix one qian with child's urine and administer. It will expel foul matter resembling pig liver, preventing blood wind and blood qi diseases for life. Blood above the diaphragm transforms into yellow fluid, which is vomited or excreted in urine. If the placenta is expelled first, ordinary medicines cannot reach the pathology; if blood stagnation is treated first, common medicines lack this efficacy. Only this medicine achieves both goals.
Huarui Shi (Flower Manganese Stone, one jin) Shangse Liuhuang (Yellow Sulfur, four liang, each ground finely)
Mix the two ingredients evenly. Seal a clay jar with paper and mud, ensuring it can hold the medicine. Once dry, place the medicine inside, seal tightly with mud, and bake thoroughly in a kiln. Place on a square brick inscribed with Bagua and Five Elements characters. Surround with one cheng of charcoal, layering carefully. Start fire from below at Si-Wu hours, allowing heat to penetrate gradually upwards. Catch falling embers on upper fires. Continue overnight until charcoal is consumed and fire cools. Leave for another night. Remove when cool, grind finely, sieve through silk to extreme fineness, and store in porcelain jars. Use according to method. (This medicine is also Huarui Shi San for treating metal wounds; ordinary people should keep it for emergencies.)
A relative's wife suffered from retained placenta postpartum, with blood distension causing coma and unconsciousness. She was told she was dead. I said: This is blood distension; it can be saved with Huarui Shi San. One qian was mixed with child's urine and administered; she revived immediately, and the placenta and foul water were expelled without incident.
Xiong San (Ligusticum Powder) Treats retained placenta.
Xiong (Ligusticum Chuanxiong Rhizome) Danggui (Angelica Sinensis Root, dried, half liang each) Yubai Pi (Elm Bark, one liang, chopped)
Grind into fine powder. Take two qian per dose. Before meals, mix with fresh Rehmannia juice and warm wine.
Bixiao Fang Niuxi Tang (Must-Efficacy Achyranthes Decoction) Treats retained placenta with hard, distended, and painful umbilical and abdominal regions, which can be fatal. Taking this medicine causes the placenta to rot and descend; dead fetuses are also expelled.
Niuxi (Achyranthes Root) Qumai (Dianthus Herb, four liang each) Danggui (Angelica Sinensis Root, three liang) Tongcao (Tetrapanax Pith, six liang) Huashi (Talcum, eight liang) Kuizi (Abutilon Seed, five liang)
Finely chop. Boil in nine sheng of water until three sheng remain. Divide into three doses. If the placenta does not descend and abdominal distension occurs, it is fatal; take this medicine to cause the placenta to rot and exit.
Niuxi San (Achyranthes Powder) Treats retained placenta after miscarriage at five or six months of pregnancy.
Niuxi (Achyranthes Root) Chuanxiong (Ligusticum Rhizome) Piaoxiao (Mirabilite) Puhuang (Lygodium Spore, seven qian and a half each) Guixin (Cinnamon Twig, half liang) Danggui (Angelica Sinensis Root, one liang and a half)
Make into coarse powder. Take four qian per dose. Boil with one sheng of water, three slices of ginger, and one fen of fresh Rehmannia until six fen remains. Strain and drink warm frequently.
Qianjin Beiji Dan (Thousand Golden Emergency Pill) Treats postpartum evil blood rushing to the heart, retained placenta, and abdominal blood masses.
Use Jinwen Dahuang (Jinwen Rhubarb, one liang) ground into fine powder. Boil with half sheng of strong vinegar until a paste forms. Make pills the size of sesame seeds. Swallow five or seven pills with warm vinegar soup. Evil blood will be expelled shortly, leading to recovery.
"Bao Ming" Treats retained placenta, fetal death in utero, blood rushing upward causing faintness, sudden heavy bleeding, or dry placenta preventing delivery. Banxia Tang (Pinellia Decoction) is the primary formula. (See earlier section on promoting delivery.)
Xitai Wan (Fetus-Expelling Pill)
Banxia (Pinellia Tuber) Baisan (White Powder, half liang each)
Grind into fine powder and make pills the size of sesame seeds. Take thirty pills with Banxia Tang after meals, gradually increasing to fifty pills. If ineffective, increase dosage using Yubai Pi San (Elm Bark Powder). If still ineffective, use Dasheng San (Great Sage Powder). For those with chronic heat, Ren Shen Jingjie San (Ginseng and Schizonepeta Powder) is suitable.
Treating retained placenta; abdominal distension can be fatal.
Heidou (Black Soybean, one he, fried until cooked)
Add one sheng of vinegar, boil for three to five boils, remove beans, and divide into three doses. Can also be boiled in wine.
"Bixiao Fang" Treats retained placenta. Take Puhuang (Lygodium Spore) the size of a jujube. ("Ji Yan", "Qian Jin", "Cui Shi Fang" agree.)
Another prescription: Shengdihuang Zhi (Fresh Rehmannia Juice, one sheng) Ku Jiu (Vinegar, three he). Warm and drink.
Another prescription: Niuxi (Achyranthes Root, one liang) Kuizi (Abutilon Seed, one he). Crush, boil in one and a half sheng of water until one sheng remains. Strain and divide into two doses.
"Guang Ji" Prescription for Retained Placenta.
Take three finger-pinches of soil from the chimney flue, swallow with water. ("Ji Yan", "Qian Jin", "Be Ji" agree.)
Another prescription: Fulonggan (Dragon's Liver/Calcined Earth, large piece, ground) Mix well with good vinegar, apply to the umbilicus. Subsequently decoct and drink Sheng Gancao Tang (Fresh Licorice Decoction, three to four he).
Another prescription: Drink vinegar soup to expel it.
Another prescription: Ji Zi (Chicken Egg, one) Ku Jiu (Vinegar, one he). Mix and drink; it will be expelled immediately.
"Yan Nian Fang" Treats retained placenta; abdominal distension can be fatal.
Swallow three chicken egg yolks. Simultaneously untie hair and prick the throat to induce vomiting; it will be expelled. If extremely weak, boil one mole rat in water for twenty boils, and administer the liquid orally; it will be expelled. ("Cui Shi" agrees.)
"Jiu Ji Fang" Chi Xiaodou (Red Adzuki Bean, seven grains for males, fourteen for females). Swallow with east-flowing water.
Treatment for Retained Placenta.
Gualou Shi (Trichosanthes Fruit Seed, one fruit, seeds ground finely)
Mix half a cup of wine and half a cup of child's urine. Boil until reduced to seventy percent. Strain and drink warm. (If fruit is unavailable, root may be used.)
Another prescription: Honghua (Safflower, one liang). Boil in wine, consume concentrated juice.
Another prescription: Lu Jiao Bang Xie (Deer Horn Shavings), grind finely to three fen. Mix with scallion white decoction and take.
Whenever preparing for delivery, first remove commonly worn clothes and cover the stove with them; the placenta will descend naturally, facilitating delivery.
"Guang Ji Fang" For retained placenta, take the husband's single-layer garment and cover the well with it; it will be expelled immediately.
Another prescription: Take the mother's shoe sole, heat it over fire, and moxa/iron the lower abdomen up and down fourteen times.
Another prescription: Take the front part of a broken straw sandal found by the road, including the toe loop, burn to ash, mix with child's urine and wine, and take.
Another prescription: Burn Gaojiao Ci (Gleditsia Spine) to ash. Take one qian per dose, mix with warm wine.
"Bao Qing Fang" states: Among women's diseases, none are more severe than childbirth. In obstetrics, nothing is more critical than promoting delivery during labor; after delivery, nothing is more urgent than retained placenta. Only Huarui Shi San (Flower Manganese Stone Powder) is most crucial. Although formulas like Heishen San (Black Divine Powder) and Hupo San (Amber Powder) have efficacy, rural residents far from pharmacies may find themselves without options in emergencies. Here is a wonderful method: If the placenta does not descend after delivery and waits too long, not only will the mother be exhausted, but blood will flow into the uterus, distending it, striking the chest and heart, causing panting and pain, leading to critical danger. If such symptoms occur, urgently cut the umbilical cord and tie a small object to hang it down. (When tying, be careful: tie first, then cut; otherwise, the placenta may rise and crush the heart, causing death.) This prevents blood from flooding into the placenta, causing it to shrink and descend naturally. Even if delayed for several days, it will not harm the person. As long as the mother remains calm, it will eventually descend. Tested repeatedly with success. Do not blindly trust midwives to use manual manipulation; many have died due to this. Exercise utmost caution.
[Acupuncture]
Retained Placenta: Sanyinjiao (SP6), Zhongji (CV3). Apply reducing method.
[Biao You Jing] Another Method: Zhaohai (KI6), Neiguan (PC6).
[Jia Yi Jing] For women with difficult labor, if the placenta does not descend, Kunlun (BL60) is indicated.
