[Da] Dysentery occurring postpartum is caused by labor injury during childbirth, leading to insufficiency of the Zang-Fu organs. If the confinement period has not yet passed and the body's deficiency has not recovered, early exertion or mistaken consumption of raw and cold foods can occur. Rising and moving too early exposes one to external wind-cold pathogens, which enter the intestines and stomach taking advantage of the deficiency. Mistakenly eating hard-to-digest raw and cold items damages the Spleen and Stomach. All these conditions lead to watery diarrhea, which in severe cases transforms into dysentery. If blood seeps into the Large Intestine, it becomes bloody dysentery (Xue Li), which is difficult to treat and is commonly known as "childbirth dysentery" (Chan Zi Li). Contracting cold results in white dysentery, resembling fish brains; contracting heat results in red-yellow dysentery or sudden passage of fresh blood. If cold and heat pathogens contend with each other, the discharge is mixed red and white, or pus and blood are intermingled. If the discharge is green, it indicates extreme cold. If there is no desire for food, irregular bowel movements, and no limit to frequency day or night, this is critical. The postpartum woman is originally deficient; adding prolonged dysentery leads to weakness, emaciation, and fatigue, becoming more debilitated, termed "deficient and emaciated dysentery" (Xu Lei Xia Li). Another type is where Qi mechanism is obstructed postpartum leading to red and white dysentery, termed "Qi dysentery" (Qi Li). Treatment methods: For heat patterns, use cooling methods; for cold patterns, use warming methods; for contending cold and heat, use harmonizing methods; for sliding/incontinence, use astringent methods; for deficiency and emaciation, use tonifying methods; for inability to distinguish water and grain, promote urination. If the parturient has a stubborn temperament and cannot relax, one must smooth her Qi, and she will surely be at peace.
[Xue] In the aforementioned symptoms, white dysentery belongs to the Qi level, while red dysentery belongs to the Blood level. Detailed discussions are found under "Diarrhea and Dysentery." A parturient ate eggs and experienced abdominal pain with a blue-yellow complexion. After taking Pingwei San and Erchen Tang, she developed dysentery and abdominal distension. Switching to Liuqi Yinzi caused a mass in the lower abdomen to attack upwards intermittently, and food intake decreased further. This was due to Spleen-Stomach deficiency-cold and Liver Wood overacting on Earth. Using Buzhong Yiqi Tang added with Muxiang and Wuzhuyu led to gradual recovery. Subsequently, using Bazhen Tang for major tonification, along with regulating herbs, led to full recovery. Another parturient, before the month was complete postpartum, ate cold food and drank alcohol. Cold and heat attacked the blood, causing pure bloody dysentery with severe tenesmus and pain. Her pulse was large but forceless, and she had dry mouth. Taking Huangqin Shaoyao Tang for three doses brought relief.
[Xiong] To treat postpartum red and white dysentery, use Sijunzi Tang added with Huangqi and Yumishe (Poppy Shell); the effect is miraculous.
[Yun] Jiuji San treats postpartum red and white dysentery with colicky abdominal pain.
Ingredients: Baishao (White Peony Root), Ejiao (Donkey-Hide Gelatin), Aiye (Mugwort Leaf), Shudihuang (Prepared Rehmannia Root) (each 1 liang); Gancao (Licorice Root), Danggui (Angelica Sinensis) (each 3 liang).
Preparation: Coarsely crush, decoct in water, divide into two doses, drink on an empty stomach.
[Da] Danggui Shaoyao San treats colicky abdominal pain during pregnancy, urgent pain below the heart, and also treats postpartum blood syncope and chronic dysentery due to metrorrhagia. (Formula see "Metrorrhagia".)
To treat bloody dysentery during pregnancy: Use 2 liang of Ejiao, boil in 1.5 sheng of wine until reduced to 1 sheng, take in one dose.
For various postpartum dysenteries: Boil Xiebai (Allium macrostemon Bulb) and eat. Another method: Stir-fry Xiebai with Sheep Kidney Fat, eat on an empty stomach; very effective.
[Jing] Treat dysentery postpartum and antepartum: One Biejiarjia (Tortoise Plastron), roast with rice vinegar, grind to powder, take with vinegar-water mixture. (Danxi said: Tortoise plastron greatly tonifies Yin, treating fatigue from overwork.)
[Dan] A woman, after spontaneous abortion, suffered from chest fullness, poor appetite, and persistent dysentery. Her pulse was weak, especially on the left hand.
Ingredients: Huashi (Talcum), Baishaoyao (Fried White Peony Root), Cangzhu (Atractylodes Rhizome) (each 5 qian); Baizhu (Atractylopsis Rhizome) (2.5 qian); Hezi (Chebula Fruit, roasted) (2 qian); Ganjiang (Dried Ginger) (4 qian); Fuling (Poria) (1 qian).
Preparation: Grind to fine powder, take with 40-50 pills of Baohe Wan.
[Yun] Postpartum bloody dysentery with umbilical and abdominal pain: Siwu Tang added with Huaihua (Sophora Flower), Huanglian (Coptis Rhizome), Yumike (Poppy Shell), etc.
Huanglian Wan treats postpartum red and white dysentery with unbearable abdominal twisting pain.
Ingredients: Huanglian (4 liang); Ejiao, Puhuang (Pollens Typhae), Zhizi Ren (Gardenia Seed) (each 1 liang); Danggui (2.5 liang); Huangqin (Scutellaria Root), Huangbo (Phellodendron Bark) (each 2 liang).
Preparation: Grind to powder, make pills with refined honey the size of paulownia seeds. Take 60-70 pills per dose, swallow with rice water, three times daily and once at night.
[Zhong] For postpartum dysentery with extreme deficiency, Baitouweng Jia Gancao Ejiao Tang is indicated. (Maijing writes: Severe heat dysentery with heavy discharge, in newly delivered women with extreme deficiency.)
Ingredients: Baitouweng (Pulsatilla Root), Ejiao (each 2 liang); Huanglian, Huangbai, Qinpi (each 3 liang); Gancao (2 liang).
Preparation: Add 7 sheng of water, decoct to obtain 2.5 sheng, add Ejiao and stir until dissolved completely. Take warm, divided into three doses.
For women with dysentery at the time of delivery: ShanZhi (Gardenia Fruit) in any amount, burn to ash, grind to fine powder. On an empty stomach, mix with warm cooked water and take 1 qian. For severe cases, do not exceed five doses.
Postpartum bloody dysentery with urinary retention and umbilical/abdominal pain: Crush fresh Machixian (Portulaca Oleracea) to extract juice (2 da he), boil for one boil, add 1 he of honey, mix, and take in one dose.
[Mei] Treat postpartum uncontrollable bloody discharge, pain from residual blood, and masses.
Guixin (Cinnamon Twig) and Ganjiang (Dried Ginger) in equal parts, grind to powder. Mix with wine and take on an empty stomach, one fangcun bi (measure).
[Da] To treat postpartum dysentery with fifty stools per day: Collect excrement of wood-boring insects from inside the wood, stir-fry until yellow. Immediately pour water over it to achieve appropriate thickness. Take this mixture, and the condition will heal immediately.
[Sheng] Formula to treat various postpartum dysenteries: Crush Cang'er Ye (Xanthium Leaf) to extract juice, half zhan. Take warm, three to four times daily.
[Ke] When postpartum dysentery causes thirst, it is because the essence of food and drink fails to transform into Blood, Qi, and Body Fluids to nourish the Zang-Fu organs. The Zang-Fu become deficient and dry, causing dysentery and thirst. If forced drinking occurs, it is hard to stop, and instead, water qi overflows. Since the Spleen and Stomach are deficient and cannot control water, water flows freely, soaking the skin, causing edema. One should only stop the thirst, and the dysentery will heal itself.
[Xue] If the aforementioned symptom involves thirst without preference for cold drinks, it belongs to Stomach Qi deficiency failing to generate fluids; use Qiwei Baizhu San. Nighttime fever and thirst belong to Kidney Water deficiency failing to moisten; use Liuwei Wan, assisted by Yiqi Tang, to nourish the source of transformation. A parturient had diarrhea and dysentery, fever, thirst, expectoration, emaciation, poor appetite, chest/epigastric stuffiness, or lower abdominal bearing-down sensation, lasting over a year. I considered this Spleen-Kidney diarrhea. Morning:ersh Shen Wan; Evening: Liujunzi Tang. Recovered after three months.
Another parturient with the above symptoms was exhausted, could not eat, and was near death. This was Spleen and Stomach dual deficiency. Treated with Sishen Wan and Shiquan Dabu Tang, and she recovered.
"Bixiao" treats postpartum dysentery with unmeasured thirst and drinking: Maidong (Ophiopogon Tuber, 3 liang), Wumei (Chinese Plum, 20 pieces). Finely cut, boil in 1 sheng of water to obtain 7 he. Sip slowly.
"Jingxiao" treats chronic postpartum dysentery with dried-up fluids and unceasing thirst: Longgu (Fossilized Bones, 12 fen), Houpo (Magnolia Bark), Fuling, Huangqi, Maidong, Renshen (each 8 fen); Shengjiang (Fresh Ginger, 6 fen); Dazao (Jujube, 14 pieces, finely cut). Add 1 da sheng of water, decoct to obtain 7 he. Divide into two doses, take on an empty stomach.
Treat postpartum dysentery where long duration leads to fluid exhaustion, edema of limbs, dry mouth, and dry tongue. "Gujin Luyan": One Donggua (Winter Melon). Paste thickly with yellow mud (5 cun thick), bake until soft and cooked. Peel, squeeze out juice, and take to cure.
"Buyi" Renshen Baizhu San treats postpartum dysentery with exhausted fluids and unceasing thirst. (Formula see "Antepartum Cholera-like Disease".)
