壮热者，一向不止，由血气壅实，五脏生热，蒸熨于内则眠卧不安，精神恍忽，
熏发于外则表里俱热，烦躁喘粗，甚则发惊痫也。轻剂，火府丹、地黄煎。重剂、
双解饮、七宝散、大黄朴硝汤。
地黄煎 治小儿壮热烦心，眠卧不安。
生地黄（汁一升） 白沙蜜（三合） 酥（三合） 生门冬（汁三合）
上，重汤煮至成膏，每服数匙。
火府丹 治小儿壮热。
生地黄 木通 甘草 黄芩
上，水一钟，煎服。
金莲饮子 治小儿蕴积壮热，赤眼口疮，心烦躁闷，咽干多渴，潮热不止。
防风 甘草（炙） 连翘 柴胡（去芦） 山栀子（各半两）
上为末。每服二钱，用水六分，煎至三分，食后服。
黄龙汤 治发热不退，或寒热往来。
柴胡（五钱） 黄芩（炒） 甘草（炙。各二钱） 赤芍药（三钱）
上，每服一钱，姜枣水煎。
牛黄膏 治壮热，咽喉涎响，或不省人事，或左右手偏搐，或唇口眼鼻颤动，此热
涎内蓄，风邪外感也，宜急服之。
蝎尾（四十九枚） 巴豆肉（去油膜，一钱半） 梅花脑（半匙） 辰砂（研，二钱） 郁金（三钱，
皂角水煮） 牛黄（少许） 麝香（一匙）
上为末。每服一匙，蜜水调下。量儿虚实用之。
栀子仁汤 治阳毒壮热，百节疼痛，下后热不退者。
栀子仁（酒炒） 赤芍药 大青 知母（各一两） 升麻 黄芩（酒炒） 石膏（各二两）
柴胡（一两半） 甘草（五钱） 杏仁（二两，浸，去皮，麸炒微黄）
上，每服三钱，生姜三片，水煎服。
六物黄芩汤 治壮热腹大短气，往来寒热，饮食不化。
黄芩（酒炒） 大青 甘草（炙） 麦门冬（去心） 石膏（各半两） 桂（一钱）
上，每服一二钱，水煎服。
五物人参饮 治壮热咳嗽，心腹胀满。
人参 甘草（各半两） 麦门冬（去心） 生地黄（各一两半） 茅根（半握）
上，每服二三钱，水煎服。
壮热，是指发热持续不退。这是由于血气壅滞充实，五脏产生内热，热气在体内蒸腾熏灼，导致睡眠和卧姿不安稳，精神恍惚；热气熏蒸发越于体表，则出现表里俱热、烦躁、气喘且呼吸粗重等症状，严重时甚至引发惊风痫证。治疗轻症者，可用火府丹、地黄煎；治疗重症者，可用双解饮、七宝散、大黄朴硝汤。
地黄煎：治疗小儿壮热烦闷、心中不适，睡眠和卧姿不安稳。
处方组成：生地黄（取汁一升）、白沙蜜（三合）、酥（三合）、生门冬（取汁三合）。
制法与用法：以上药物，隔水炖煮至成为膏状，每次服用数匙。
火府丹：治疗小儿壮热。
处方组成：生地黄、木通、甘草、黄芩。
制法与用法：以上药物，用水一钟，煎煮后服用。
金莲饮子：治疗小儿因蕴积而致的壮热，伴有红眼、口疮，心中烦躁闷乱，咽干多渴，潮热持续不退。
处方组成：防风、甘草（炙）、连翘、柴胡（去芦头）、山栀子（各半两）。
制法与用法：以上药物研为细末。每次服用二钱，用水六分，煎煮至三分，饭后服用。
黄龙汤：治疗发热不退，或寒热往来交替发作。
处方组成：柴胡（五钱）、黄芩（炒）、甘草（炙，各二钱）、赤芍药（三钱）。
制法与用法：以上药物，每次服用一钱，用生姜、大枣的水煎服。
牛黄膏：治疗壮热，咽喉间有痰涎响声，或神志不清不省人事，或左右手偏侧抽搐，或嘴唇、口、眼、鼻颤动。这是热痰内蓄，兼感风邪所致，应当立即服用此药。
处方组成：蝎尾（四十九枚）、巴豆肉（去油膜，一钱半）、梅花脑（半匙）、辰砂（研细，二钱）、郁金（三钱，用皂角水煮过）、牛黄（少许）、麝香（一匙）。
制法与用法：以上药物研为细末。每次服用一匙，用蜂蜜水调服。根据患儿的虚实情况酌情使用。
栀子仁汤：治疗阳毒所致的壮热，全身关节疼痛，经攻下治疗后热势仍不退者。
处方组成：栀子仁（酒炒）、赤芍药、大青、知母（各一两）、升麻、黄芩（酒炒）、石膏（各二两）、柴胡（一两半）、甘草（五钱）、杏仁（二两，浸泡，去皮，麸皮炒至微黄）。
制法与用法：以上药物，每次服用三钱，加生姜三片，用水煎服。
六物黄芩汤：治疗壮热伴腹部胀大、呼吸短促，寒热往来，饮食不能消化。
处方组成：黄芩（酒炒）、大青、甘草（炙）、麦门冬（去心）、石膏（各半两）、桂（一钱）。
制法与用法：以上药物，每次服用一二钱，用水煎服。
五物人参饮：治疗壮热伴咳嗽，心胸腹胀满。
处方组成：人参、甘草（各半两）、麦门冬（去心）、生地黄（各一两半）、茅根（半握量）。
制法与用法：以上药物，每次服用二三钱，用水煎服。
Hyperpyrexia refers to fever that persists continuously without abating. This condition arises from the stagnation and excess of blood and qi, generating internal heat within the five zang organs. When this heat steams and scorches internally, it leads to restlessness in sleep and posture, as well as mental confusion; when it薰 (pervades) outwardly, it results in heat affecting both the interior and exterior, accompanied by irritability, panting, and coarse breathing. In severe cases, it may even trigger convulsions and epilepsy. For mild cases, Huofu Dan (Fire Mansion Pill) or Dihuang Jian (Rehmannia Decoction) may be used. For severe cases, Shuangjie Yin (Double-Resolution Drink), Qibao San (Seven-Treasure Powder), or Dahuang Puxiao Tang (Rhubarb and Mirabilite Decoction) are indicated.
Dihuang Jian (Rehmannia Decoction): Treats hyperpyrexia, vexation, and restlessness in sleep and posture in children.
Ingredients: Fresh Rehmanniae Radix (juice, 1 sheng), white sand honey (3 he), butter/ghee (3 he), fresh Ophiopogonis Radix (juice, 3 he).
Preparation and Administration: The above ingredients are cooked over a double boiler until they form a paste. Take several teaspoons per dose.
Huofu Dan (Fire Mansion Pill): Treats hyperpyrexia in children.
Ingredients: Fresh Rehmanniae Radix, Akebiae Caulis (Mutong), Glycyrrhizae Radix et Rhizoma (Gancao), Scutellariae Radix (Huangqin).
Preparation and Administration: The above ingredients are decocted in one zhong of water and taken orally.
Jinlian Yinzi (Golden Lotus Drink): Treats hyperpyrexia due to accumulation and retention in children, accompanied by red eyes, oral ulcers, mental vexation and oppression, dry throat with excessive thirst, and persistent tidal fever.
Ingredients: Saposhnikoviae Radix (Fangfeng), Glycyrrhizae Radix et Rhizoma (Zhi Gancao, roasted), Forsythiae Fructus (Lianqiao), Bupleuri Radix (Chaihu, radix removed), Gardeniae Fructus (Shanzhizi, each 0.5 liang).
Preparation and Administration: The above ingredients are ground into fine powder. Take 2 qian per dose, decocted in 6 fen of water until reduced to 3 fen, taken after meals.
Huanglong Tang (Yellow Dragon Decoction): Treats persistent fever or alternating chills and fever.
Ingredients: Bupleuri Radix (Chaihu, 5 qian), Scutellariae Radix (Huangqin, stir-fried), Glycyrrhizae Radix et Rhizoma (Zhi Gancao, roasted, each 2 qian), Paeoniae Radix Rubra (Chishao, 3 qian).
Preparation and Administration: The above ingredients are decocted with ginger and jujube water. Take 1 qian per dose.
Niuhuang Gao (Calculus Bovis Paste): Treats hyperpyrexia, rattling phlegm in the throat, unconsciousness, hemiconvulsions of either hand, or tremors of the lips, mouth, eyes, and nose. This is caused by the internal accumulation of hot phlegm and external contraction of wind pathogenic factors; immediate administration is required.
Ingredients: Scorpio (Xiewei, 49 pieces), Crotonis Semen (Badou Rou, oil and membrane removed, 1.5 qian), Plum Blossom Brain (Meihua Nao, half spoon), Cinnabar (Chensha, powdered, 2 qian), Curcumae Radix (Yujin, 3 qian, boiled in Gleditsiae Sinensis Fruit juice), Calculus Bovis (Niuhuang, a small amount), Moschus (Shexiang, one spoon).
Preparation and Administration: The above ingredients are ground into fine powder. Take one spoon per dose, mixed with honey water. Adjust dosage according to the child’s deficiency or excess condition.
Zhiren Tang (Gardenia Kernel Decoction): Treats hyperpyrexia due to Yang toxin, pain in all joints, and persistent heat even after purgative treatment.
Ingredients: Gardeniae Fructus Kernel (Zhiren, wine-fried), Paeoniae Radix Rubra (Chishao), Isatidis Folium (Daqing), Anemarrhenae Rhizoma (Zhimu, each 1 liang), Cimicifugae Rhizoma (Shengma), Scutellariae Radix (Huangqin, wine-fried), Gypsum Fibrosum (Shigao, each 2 liang), Bupleuri Radix (Chaihu, 1.5 liang), Glycyrrhizae Radix et Rhizoma (Gancao, 5 qian), Armeniacae Semen Amarum (Xingren, 2 liang, soaked, peeled, bran-fried until slightly yellow).
Preparation and Administration: The above ingredients are decocted with three slices of fresh ginger in water. Take 3 qian per dose.
Liuyu Huangqin Tang (Six-Ingredient Scutellaria Decoction): Treats hyperpyrexia with abdominal distension and shortness of breath, alternating chills and fever, and indigestion.
Ingredients: Scutellariae Radix (Huangqin, wine-fried), Isatidis Folium (Daqing), Glycyrrhizae Radix et Rhizoma (Zhi Gancao, roasted), Ophiopogonis Radix (Maidong, heart removed), Gypsum Fibrosum (Shigao, each 0.5 liang), Cinnamomi Cortex (Gui, 1 qian).
Preparation and Administration: The above ingredients are decocted in water. Take 1–2 qian per dose.
Wuwu Renshen Yin (Five-Ingredient Ginseng Drink): Treats hyperpyrexia with cough, and chest/abdominal fullness.
Ingredients: Ginseng Radix et Rhizoma (Renshen), Glycyrrhizae Radix et Rhizoma (Gancao, each 0.5 liang), Ophiopogonis Radix (Maidong, heart removed), Fresh Rehmanniae Radix (Shengdihuang, each 1.5 liang), Imperatae Rhizoma (Maogen, half handful).
Preparation and Administration: The above ingredients are decocted in water. Take 2–3 qian per dose.
