[Ulceration] When pustules of smallpox mature and ulcerate, it is a common occurrence. However, if they ulcerate before pus has formed, this is termed "Lan" (Ulceration). The cause of "Lan" is when the pathogenic factor should be dispersed via sweating but fails to do so, leading to blockage by toxic qi, resulting in panting, shortness of breath, chest oppression, and restlessness. Conversely, if dispersing methods are mistakenly used when inappropriate, heat toxins erupt violently with the Yang qi, causing widespread ulceration of skin and flesh; this is the fault of improper use of exterior-releasing treatments. Treatment should focus on regulating the Spleen and Stomach, promoting appetite, ensuring proper bowel movements, nourishing Ying (Nutritive) and Wei (Defensive) Qi, generating muscle tissue, and resolving toxicity. The resolution should not be too cold, and nourishment should not be too hot; this constitutes a good method. It is advisable to administer Shiquan Dabu Tang (Ten Complete Tonifying Decoction), removing Guizhi (Cinnamon Twig) and adding Fangfeng (Saposhnikovia Root) and Jingjie Sui (Schizonepeta Spike), taken in generous doses for best results. If constipation occurs, use Dan Dao Fa (Gallbladder Guiding Method) to moisten the intestines. If pus and water do not dry up, apply Baicao San (Decayed Grass Powder) as a dressing. If ulceration and suppuration are accompanied by severe pain, apply Tianshui San (Heavenly Water Powder, i.e., Liuyi San seen in the initial heat stage) mixed with Baihua Gao (Hundred Flowers Ointment, used for scabbing) topically. In cases where exterior-releasing was excessive, leading to external ulceration and internal deficiency, with Yang Qi failing to hold fast and spontaneous diarrhea due to Zang-Fu dysfunction, urgent treatment to rescue the interior while releasing the exterior is required, primarily using Chen's Muxiang San (Aucklandia Powder) as prescribed by Master Chen. For extremity coldness (Jue Ni), Yigong San (Yi Gong Powder) is indicated for poor eruption.
[Board-like Yellow] [Yuan] When smallpox pus clarifies, the toxin is exhausted and the condition is perfect; how could there be "Board-like Yellow"? One must understand that the value of jade lies not in its thickness but in the purity of its substance, and the value of pearls lies not in their size but in their luster. Similarly, the quality of smallpox paste lies not in its fullness but in its vibrant yellow color. "Yellow" is the correct color of Central Earth; the paste draws from the Spleen, so its yellow should naturally be moist and upright. In favorable cases, Qi receives protection without rebellion, Blood nourishes Ying without injury, the follicles are loose, and the clarified pus flows within, active without stickiness or blockage, bright and moist without a dead appearance. Where is there such a thing as "Board-like Yellow"? However, if Yin and Yang deviate from righteous Qi, and fierce toxins act cruelly, the roots and follicles become thin and inferior, the sacs collapse flat, and the clarified pus becomes board-like and sticky. If dry, it is blocked and inactive; if like dry wax, it lacks bright yellow luster. Upon finger pressure, it condenses and remains fixed. This indicates excessive heat in the Five Zang organs and obstruction of the Two Qi, which is called "Board-like Yellow." If the forehead (Fangguang Tian Ting) is board-yellow while the rest is moist and active, carbuncles will likely form at the brain and nape. If the cheeks and face are board-yellow while the four limbs are moist and beautiful, smallpox carbuncles will emerge on the shoulders. If the abdomen is board-yellow, carbuncles arise at Quchi (LI11) and Zusanli (ST36). If the back and spine are board-yellow, carbuncles knot at the lateral sacral bones. Generalized board-yellow across the body is fatal. Board-yellow at the front and back of the neck is fatal. Board-yellow on the head surface is fatal. Board-yellow around the eye sockets and above the lips is fatal. Board-yellow at the flanks and testicles is fatal. Observing these signs, one sees that while smallpox follicle pus should indeed be full and yellow, it is even more crucial that it be moist and active.
Chengquan San treats Board-like Yellow in smallpox.
