Tang Shi stated: In pediatric heat diarrhea, the stool is yellow and reddish, possibly with froth, caused by accumulation within the zang-fu organs. This may result from the nursing mother's preference for alcohol or hot-natured foods, or from the infant contracting dampness at birth which transforms into heat. If physicians do not understand this, but merely administer cardamom (Doukou) and chebula (Hezi), it is like pouring water on stone—ineffective. Since they fail to recognize the syndrome and thus cannot differentiate between cold and heat, and their medication methods are incorrect, how can they expect efficacy? For this syndrome, one should verify it by observing scanty dark urine, dry mouth, and irritability. The treatment method should employ Qian Yi's Baizhu San (Atractylodes Macrocephala Decoction), removing Xiang (Musk/Aucklandia depending on context, here likely referring to removing aromatic herbs if not specified, but text says "qu muxiang" remove Mu Xiang), or Wuling San (Five-Ingredient Powder with Poria). If there is fright, Guizhi (Cinnamon Twig) may be removed. If the heat is severe, Sini San (Frigid Extremities Powder) or Daxiehu Tang (Major Bupleurum Decoction) [both from Shanghan Lun], with Dahuang (Rhubarb) removed, should be used, showing remarkable efficacy. Additionally, Huanglian Wan (Coptis Pill) [for Gan malnutrition] and other formulas are also good. Tiaozhong Tang (Regulate Middle Decoction) [for dysentery], with Dahuang removed, plus Huanglian (Coptis) and Zhike (Aurantii Fructus). If diarrhea is accompanied by heat, belonging to the combined disease of Taiyang and Shaoyang presenting with spontaneous diarrhea, Huangqin Tang (Scutellaria Decoction) should be administered; add Banxia (Pinellia) if vomiting occurs. For heat-dysentery accompanied by difficult, dark red urination, Jiajian Sishun Qingliang Yin (Modified Smooth and Cooling Drink for Interior Heat) should be used to stop it.
Xue Shi stated: Redness on the right cheek with a desire for cold drinks indicates excess heat in the Stomach meridian, treated with Xiehuang San (Drain the Yellow Powder) [for Spleen heat]. Aversion to cold and preference for heat indicate deficiency heat in the Stomach meridian, treated with Baizhu San (Atractylodes Macrocephala Decoction) [for thirst]. Redness on both the right cheek and forehead indicates heat in the Heart and Spleen, treated with Xiehuang San (Drain the Yellow Powder) added with charred Huanglian (Coptis). If both the left cheek and right cheek are red, it indicates Liver fire invading Spleen earth, treated with Sijunzi Tang (Four Gentlemen Decoction) [for vomiting and diarrhea] added with Chaihu (Bupleurum). If the infant suddenly suffers from milk and food stagnation, Baohe Wan (Preserve Harmony Pill) [for retained food] should be used. The nursing mother must particularly avoid rich flavors and regulate her diet. If the mother suffers from food stagnation causing the infant's vomiting and diarrhea, the mother should be treated. Generally, initial fever indicates an excess syndrome due to pathogenic qi predominance; eventual transformation into cold indicates a deficiency syndrome due to depletion of true qi; prolonged illness with fever indicates true interior cold with false exterior heat. For prolonged diarrhea leading to deficiency-cold of Yuan Qi, refer to previous syndromes for treatment.
