Chao Yuanfang stated that when pathogenic factors contend with healthy qi, tinnitus occurs; if prolonged, the pathogenic factors stagnate, leading to deafness.
Changwu San (Calamus and Aconite Powder): Treats pediatric ear ringing that persists day and night.
Shichangpu (Rhizoma Acori Tatarinowii), Wutou (Radix Aconiti Lateralis Preparata, stir-fried). Each 4 fen.
Grind the above into powder. Wrap in cotton and insert into the ear, twice daily.
Qianjin Yi Fang (Supplement to the Essential Prescriptions Worth a Thousand Gold) formula for treating ear pain:
Fuzi (Radix Aconiti Lateralis Preparata, processed, peeled), Shichangpu (Rhizoma Acori Tatarinowii).
Take equal parts of these two herbs, wrap them, and insert into the ear.
For syndrome differentiation and treatment, refer to the section on sores and ulcers of the Heart department.
Chao Yuanfang stated that the ears are the gathering point of the Zong Mai (Grand Vessels) and the pathway through which Kidney Qi communicates. In children, if the Kidneys are vigorous with heat, the hot qi rushes upward to the ears. The body fluids become congested and coagulate, producing pus. Alternatively, water may enter the ears during bathing and not be thoroughly drained; this retained dampness contends with blood and qi, generating heat through accumulation, which also causes pus discharge. These conditions are collectively referred to as "Er" (Ear diseases); if not healed over time, they may develop into deafness.
[Zhang Huan] Honglanhua San (Safflower Powder): Treats chronic ear disease that fails to heal.
Honghuahua (Flos Carthami Tinctorii, washed and dried), Huangbai (Cortex Phellodendri, sliced). Each 1 liang. Wuyu Gu (Os Sepiae), Huangqin (Radix Scutellariae). Each 0.5 liang. Grind the above into fine powder and insufflate into the ear.
Xionghuang (Realgar, ground with water to fine paste), 0.5 liang. Shexiang (Moschus, finely ground), 1 fen.
Grind all ingredients uniformly until fine. Wrap the medicine in cotton, insert into the ear, and change twice daily.
[Tian Shi] Hongyu San (Red Jade Powder): Treats pediatric suppurative otitis media.
Kubaijian (Alumen Exsiccatum), Ganyanzhi (Corallium Rubrum, dry), Shexiang (Moschus). Each 1 qian.
Grind together uniformly. First, clean the ear canal with a cotton-wrapped stick, then apply the powder.
[Tang Shi] Longhuang San (Dragon and Realgar Powder): Treats pediatric otorrhea with continuous discharge.
Kubaijian (Alumen Exsiccatum), Longgu (Os Draconis et Mammothis, powdered), Huangdan (Minium, stir-fried). Each 0.5 liang. Shexiang (Moschus), 1 qian.
Grind together finely. First, use a cotton swab to absorb all pus and fluid. Then, take an amount equivalent to one 'zi' and half a 'fen', divide it into two portions, and insufflate into the ears, twice daily.
[Dan] For otorrhea, apply Sulfur powder externally, once per day and night, which is effective.
Mi Yao (Secret Essentials) recommends insufflating Quchan (Gryllotalpa) ash powder for immediate effect; for those with ulcers, apply externally. (Same as Sun Simiao's prescription.)
Xue Ji stated: Various external treatments can only address mild cases of Fu organ syndromes. If the condition involves Liver channel wind-heat or blood dryness, one must follow the previous discussions to administer appropriate internal medicine. Using external methods to collect pus and dampness is acceptable as an adjunct, but relying solely on external attack while neglecting internal treatment is erroneous.
