A question was asked: "What if a *ju* (deep-seated abscess) appears on the upper part of the body, resembling a purple grape?" The answer was: "This is named *Xuan Yong* (Hanging Carbuncle), belonging to the Hand-Taiyin Lung Meridian and the Hand-Jueyin Pericardium. It causes severe alternating chills and fever, inability to extend or retract the tongue, inability to open or close the mouth, a desire to lie supine, and frequent discharge of bloody nasal mucus. This is caused by accumulated heat in the Hand-Shaoyin and Foot-Shaoyin meridians, as well as the Sanjiao (Triple Burner). It is appropriate to use *Huanglian Xiaodu Yin* (Coptis Decoction for Eliminating Toxins) with added *Jiegeng* (Platycodon Root) and *Xuanshen* (Scrophularia Root). Urgently puncture to drain foul blood, and apply *Xijiao Hupo Gao* (Rhinoceros Horn and Amber Ointment). For those with robust constitutions, administer *Yili Jindan* (One-Grain Golden Pill) to purge. If treatment is delayed, leading to inability to ingest food, unconsciousness, and chaotic pulse, death will ensue.
[Shi] For sores on the lips, apply the condensation dripping from the top of a steamer (*Zeng Han*), which has repeatedly proven miraculous.
[Luo] *Duoxiao San* (Many-Efficacy Powder) treats lip tightening with sores and rashes.
*Hezi Rou* (Areca Seed Flesh) and *Wubeizi* (Galla Chinensis) (equal parts).
Grind the above into a fine powder; apply dry and paste for efficacy.
[Dan] For yellow blisters and swelling on the lips, stir-fry *Wutou* (Aconite Lateral Root) until charred, grind it, and mix with sesame oil for application (Hu's Prescription).
For sores on the lips that do not heal for years, take one jin of blue leaves from August, crush them to extract juice, and wash the affected area. It will heal within three days (*Qianjin Fang*).
For sores on the lips, apply white lotus petals; this is highly effective. If cracked and bleeding, it stops immediately.
*Gui Yi Ke* states: In *Baimai Ju* (Hundred-Vessels Abscess), there is swelling around the neck and pain, high body heat, inability to move freely, and inability to eat. The patient experiences great fear and fright, qi rebellion with coughing, and the onset draws towards the ears. One must prevent the swelling from spreading. Within fifteen days, acupuncture and Daoyin (guiding and pulling exercises) can be performed. If acupuncture and Daoyin are not used to induce bleeding/drainage, death will occur within eighty days.
*Ling Shu* states: An abscess occurring at the neck is named *Tian Ju*. Its abscess is large, red, and black. If not treated urgently, the heat qi will descend into the Yuanye acupoint, damage the Ren Mai (Conception Vessel) anteriorly, and internally熏 (mingle/steep) the Liver and Lungs. After mingling with the Liver and Lungs for more than ten days, death will occur. A question was asked: "What about an abscess or *ju* growing on the neck?" The answer was: "This is *Jing Yong* (Neck Carbuncle), belonging to the Hand-Shaoyang Sanjiao Meridian, caused by stagnant fire, accumulated resentment, and fright. Upon initial sensation, perform garlic-separated moxibustion immediately, take *Mingdang Yin* (Life-Saving Decoction) with added *Xuanshen*, *Jiegeng*, and *Shengma*, as well as *Shengjin Dan* and *Duoming Dan* to induce sweating. For robust individuals, purge with *Yili Jindan*; for the elderly and weak, use *Shiquan Dabu Tang* (Decoction for Complete Great Tonification) or *Renshen Yangrong Tang* (Ginseng Nourishing Nutritive Qi Decoction). If ulceration occurs but does not close, accompanied by irritability, distension, fullness, urinary symptoms like strangury, and vomiting, death will ensue.
A woman had persistent neck swelling and pain. I administered six doses of *Shenxiao Gualou San* (Miraculous Trichosanthes Powder), resulting in slight reduction. Then, I switched to *Xiaochaihu Tang* (Minor Bupleurum Decoction) with added *Qingpi* (Aurantii Fructus Immaturus), *Zhike* (Aurantii Fructus), and *Beimu* (Fritillariae Bulbus) for several doses, reducing the pain and swelling by more than half. Subsequently, I combined *Siwu Tang* (Four Substance Decoction) with *Xiaochaihu Tang* for several doses, leading to resolution.
A man suffered from mental fatigue and anger, presenting with a lump on the neck that persisted despite various medications. I prescribed *Bazhen Tang* (Eight Treasure Decoction) with added *Chaihu* (Bupleuri Radix) and *Xiangfu* (Cyperi Rhizoma), along with several moxa cones of garlic-separated moxibustion daily. He drank two to three cups of *Yuanzhi Jiu* (Polygala Wine) daily, and the condition gradually resolved.
A woman with amenorrhea developed gradual fever and cough, emaciation, chest oppression, and a persistent neck lump. She was instructed to take *Xiaoyao San* (Free and Easy Wanderer Powder) for over a month, followed by *Bazhen Tang* with added *Mudanpi* (Moutan Cortex) and *Xiangfu* for another month. Then, *Huangqi* (Astragali Radix) and *Bailian* (Lilii Bulbus) were added. After two months, the fever subsided, swelling disappeared, menstruation resumed, and she recovered.
A man over thirty experienced neck swelling and fever due to excessive mental exertion. His condition worsened after taking *Baidu San* (Defeat Toxin Powder), but resolved after several doses of *Buzhong Yiqi Tang* (Decoction for Supplementing the Middle and Augmenting Qi).
Another man, due to sudden rage, had painful swelling under the neck, chest oppression, and fever. Two doses of *Fangmai Liuqi Yin* (Pulse-Qi Flowing Decoction) relieved the chest oppression, and two doses of *Jingfang Baidu San* (Schizonepeta and Saposhnikovia Decoction to Defeat Toxins) cleared the fever. The Liver pulse remained wiry and choppy. Four doses of *Xiaochaihu Tang* with added *Xiong* (Chuanxiong Rhizoma), *Guizhi* (wait, text says Xiong Gui Shaoyao - likely Chuanxiong, Angelica, Peony), specifically *Xiong*, *Gui* (Danggui), and *Shaoyao* reduced the pulse and symptoms significantly. One dose of *Sanzhong Kuijian Wan* (Pill for Dissolving Swelling and Hardening) nearly resolved it, except for one remaining nodule, which healed after two taels of *Shenxian Wubi Wan* (Immortal Peerless Pill).
An infant just one year old suffered from congenital toxin on the neck. I waited until pus formed and then punctured it. About a bowl of pus drained out. Milk intake remained normal. Using interior-supporting medicines for over a month led to recovery. Another infant with this condition waited for spontaneous drainage and almost died. Generally, superficial sores should be treated with Bian stone (lancet), while deep ones require needle puncture to remove stasis at the onset of toxin accumulation, making them easier to resolve. Moreover, children have weak Qi and Blood; if pus forms without needle or Bian treatment, death is rare to avoid.
A man with a toxic swelling under the neck had pus formation but feared needles. The infection spread to the chest, appearing red like rosy clouds. The pulse was slippery and rapid. He could not eat, had insomnia for over a month, and was very fatigued. I secretly punctured him. Immediately after pus drainage, he woke up and desired food. Treatment with *Tuoli San* (Support Interior Powder) for over a month led to recovery. Another man with this condition was punctured promptly and recovered in a few days. A man with a constitutionally deficient state did not undergo puncture; the ulcer penetrated the jaw and cheek, leading to further depletion of Qi and Blood and death.
A woman had neck swelling due to anger, followed by amenorrhea, limb edema, and urinary strangury-like symptoms. This is a Blood Fen (Blood Division) syndrome. First, several doses of *Jiaoren Wan* (Pepper Kernel Pill) were given. Once menstruation started and swelling subsided, *Liujunzi Tang* (Six Gentlemen Decoction) with added *Chaihu* and *Zhike* was administered for several doses, resolving the neck swelling.
There are cases where urinary difficulty precedes general edema, leading to amenorrhea, named Water Fen (Water Division), treated appropriately with *Tingli Wan* (Descurainia Seed Pill). *Liang Fang* states: In women with edema and fullness, if menstrual cessation precedes limb edema and urinary obstruction, it is named Blood Fen. Water transforms into Blood; if Blood does not flow, it re-transforms into Water. *Jiaohong Wan* (Red Pepper Pill) should be taken.
