According to the *Gui Yi Fang*: Whenever a carbuncle or ulcer presents with external heat and internal pain, it indicates the invasion of pathogenic factors (Ke Xie) and the accumulation of toxins within, signaling the impending stage of suppuration and emergence.
Hongbao Dan (Red Treasure Pill), also known as Jindan (Golden Pill), Cunjin San (Inch-Gold Powder), or Sihuang San (Four-Yellows Powder).
* Huafen (Trichosanthes Root, *Trichosanthis Radix*) - 3 liang
* Jianghuang (Turmeric Rhizome, *Curcumae Longae Rhizoma*) - 1 liang
* Baizhi (Angelica Dahurica Root, *Angelicae Dahuricae Radix*) - 1 liang
* Chishao Yao (Red Peony Root, *Paeoniae Radix Rubra*) - 2 liang
Grind the above into fine powder. Administer internally with tea or wine, or apply externally according to the degree of heat in the syndrome. For various heat syndromes, carbuncles, swellings, and metallic injuries (Jinchuang), this medicine acts by cooling; it can transform blood stasis into fluid, resolve blood accumulations, cool the skin to promote flesh regeneration, remove necrotic tissue and rotting flesh, invigorate blood to reduce swelling, regulate Qi stagnation, relieve pain, facilitate the closure of painful areas for pus formation, and expel pus. Through these alternating actions, the nature of this formula is such that it yields fewer effects in cold conditions but greater effects in heat conditions. Therefore, unless it is a clear Yang-heat excess syndrome, it should not be used lightly, lest it cause cold coagulation and make treatment difficult. However, for bleeding from metallic injuries, this is indispensable and the primary remedy. In other cases, it serves merely as an assistant to the previous two medicines (referring to warming/tonifying formulas); careful judgment is required. Generally, these three types of medicines (including warming ones) can cooperate synergistically, act independently with authority, divide responsibilities, surround and attack jointly, provide reinforcement, hold their ground bravely, serve as vanguard regular troops winning through surprise tactics, or vice versa. The divine skill and ingenuity of treatment are indeed no different from military strategy. Yet, as troops follow the seal and generals obey orders, success depends on the practitioner. Without understanding principles and flexibility, one cannot discuss this. Usage follows:
1. If the disease pattern involves intense heat, mix with hot tea for application. If the syndrome is slightly warm, mix with wine. To promote pus discharge, use 3 parts powder to 7 parts ginger juice, mixed with tea. Why? This medicine is very cold and causes blood flow to recede; ginger juice is hot and guides blood flow to surge. Thus, blood recedes under guidance and surges under expulsion, maintaining a balance until pus forms and breaks, forcing all pus out.
2. For broken sores where flesh remains hard and does not subside, it is due to wind attacking the wound. Add Duhuo (Notopterygium Root, *Notopterygii Rhizoma et Radix*) to dispel wind and mix with hot wine. If it still does not subside, the wind-toxin is deep and muscles are firm; add Zijingpi (Bauhinia Bark, *Bauhiniæ Cortex*) which ensures resolution.
3. This formula is best for metallic injuries and red/swollen heat syndromes, stopping bleeding at the root and preventing redness. Do not use lightly for carbuncles/ulcers, as it may prevent toxin dispersion, causing inward penetration: into bone forming osteomyelitis, into throat/neck causing toxic accumulation, into chest/back causing visceral decay, or into abdomen forming internal abscesses, leading to death. Be cautious! It is better to use Chonghe Gao (Harmony Paste) or Yulong Gao (Jade Dragon Paste) based on detailed differentiation.
4. For young people with robust blood or elderly with depleted blood, if splashing bleeding occurs and cannot be stopped, death ensues when blood is exhausted. For limbs, apply tea-mixed powder several chi up and down. For chest/back/waist/abdomen, apply over the whole body to block blood pathways, then use hemostatic agents (such as Wubeizi powder, see Metallic Injury section) or Shenxiao Junzhong Fang to achieve healing.
5. For severe metallic injuries with severed tendons/vessels and exsanguination, to stop bleeding, bind the arm with rope and silk bag. Apply this formula mixed with tea over the blood pathway, then use hemostatic agents to seal. Do NOT use internal tonics like Siwu Tang immediately, as they may cause vomiting, facial paralysis, irritability, fever, and lockjaw (Poshangfeng). Instead, use Duijin Yin with Chuanxiong, Baizhi, ginger, and jujube to stabilize, then slowly tonify blood. If lockjaw symptoms appear, use Poshangfeng medicines like Gegen Tang. Seal wounds with Junzhong Fang plus Jiulei Biejia (Nine-Rib Softshell Turtle Shell, *Trionycis Carapax*) roasted in butter and powdered.
6. For head/facial metallic injuries with unceasing bleeding, urgently apply tea-mixed powder around the neck to block blood flow, and apply to wound edges. Seal with Junzhong Fang. Severe cases improve in 10 days, mild in 3.
7. For metallic injuries exposed to water with everted flesh ("flowering"), mix with Scallion Juice (*Allii Fistulosi Juice*) and apply. Lightly cauterize sides with fire or smoke with early rice straw ash. Healing occurs when fluid drains. If no drainage, wind is present; apply Aconite Lateral Rootlet (*Aconiti Lateralis Radix Praeparata*, Nanxing) mixed with tea. Seal with Junzhong Fang.
8. For postpartum women or those with amenorrhea experiencing reversed blood flow, heart distress, hematemesis, epistaxis, or lingual bleeding, apply well-water-mixed powder to the neck. Mugwort juice is also effective. Blood stops immediately, then take internal medicine to address the root. For lingual bleeding with blisters, tie a thread around the tongue base and neck to block blood flow. Internally use Huangqin (Scutellaria Root, *Scutellariae Radix*) and Jingjie (Schizonepeta Spike, *Schizonepetae Spica*) to cool the heart. Apply honey-based astringent paste to the tongue. After removing the thread, swelling subsides. Take cooling heart medicine: Siwu Tang with Jingjie, Bohe (Mint, *Menthae Haplocalycis Herba*), and Zhusha (Cinnabar, *Cinnabaris*).
9. When applying this cold medicine for heat-toxin, pain may recur upon drying, and redness/swelling may persist. Use Egg White (*Ovi Album*) for mixing. Effective for stubborn heat-toxins and burns/scalds.
10. For chest/diaphragm trauma where blood circulation is blocked, swallow powder mixed with Mung Bean Water (*Phaseoli Radiati Decoctum*). Vomiting brings relief. For falls from height where blood-invigorating drugs fail after days, possibly due to cold weather and excessive use of Da Huang (Rhubarb Root, *Rhei Radix et Rhizoma*) causing ice-like coagulation, use hot wine mixed with Jiangjun (Ginger and Pepper mixture) to drink. Blood circulation resumes quickly, saving life, as heat activates dead blood allowing prior drugs to work. For early-stage Back Carbuncle (Fabei) with small size, before 5-7 days of high red swelling, apply poultice to disperse toxin, reduce pain, and prevent traction.

Xue Ji's Commentary: These two formulas and cold-cooling medicines are suitable for carbuncles/ulcers with redness/swelling/pain belonging to Yang patterns. If there is slight swelling/pain without redness, they are likely unsuitable. Qi and blood prefer warmth and dislike cold; if cold qi enters interior, it becomes difficult to treat.
